Book excerpt: La mémoire autobiographique (Mau), composante fondamentale de la mémoire humaine, permet de conserver des souvenirs du passé propre à chaque individu. Cette mémoire très importante est altérée dans de nombreuses pathologies neurologiques notamment dans la maladie d'Alzheimer (MA). Nos objectifs visaient à développer un protocole expérimental permettant d'évaluer la mémoire autobiographique chez des patients présentant un déficit cognitif léger (MCl), sujets potentiellement à risque de développer une MA. Nous avons évalué cette mémoire chez 14 sujets MCl et chez 14 sujets contrôles au moyen d'une épreuve standard utilisant un matériel verbal standardisé (test de Kopelman, 1989) et d'une épreuve expérimentale utilisant un matériel visuel (test des photos familières). Ainsi, nous avons évalué cette mémoire chez les sujets MCl en étudiant notamment les processus cognitifs impliqués et les variables qui peuvent l'influencer (matériel verbal et visuel, charge émotionnelle). Sur l'ensemble des épreuves autobiographiques, les sujets MCl obtiennent des performances statistiquement inférieures à celles des sujets contrôles. Il apparaît également que les MCl mettent en jeu des processus cognitifs différents selon la nature du test utilisé: processus de récupération pour le test de Kopelman et processus de reconnaissance pour le test des photos familières. Par contre, ils ne semblent pas présenter de déficit du ressenti émotionnel. Cette atteinte de la Mau serait donc au moins en partie liée à un déficit des processus cognitifs de stockage et de récupération. Cette recherche a permis de révéler l'existence de troubles de la Mau chez les MCl ce qui permettra d'établir une prise en charge thérapeutique plus approprié.

Parmi elles, la maladie d'Alzheimer est la plus fréquente. La frontière entre le vieillissement normal et l'apparition des premiers troubles est floue. Dans ce contexte, le concept de Subjective Cognitive Impairment (SCI), désignant les personnes ayant une plainte mnésique non objectivée, a été créé. Plusieurs hypothèses peuvent expliquer cette difficulté de diagnostic. Les troubles sont-ils trop subtils ? Le SCI est-il une réalité clinique ? Les évaluations mnésiques classiques sont-elles adaptées ? Les intervalles de rétention usuels sont-ils assez longs ? Sur ce constat, le Groupe de Réflexion sur les Évaluations Cognitives a développé l'épreuve Épirêve. Elle évalue la mémoire épisodique autobiographique antérograde grâce à la réalité virtuelle. Le sujet doit explorer une ville où vingt événements remarquables auront lieu. Sans qu'il n'en soit informé, il devra rappeler ceux dont il se souvient, à vingt minutes puis à trois semaines. Notre travail consiste en une étude préliminaire portant sur l'intérêt de développer ce matériel dans l'optique de la problématique décrite. Pour ce faire, nous l'avons administré à trente-huit sujets de plus de 70 ans, âge à partir duquel les plaintes mnésiques deviennent plus fréquentes, et à deux sujets SCI, nos études de cas. Nos résultats montrent que l'épreuve Épirêve est valide cliniquement. Grâce à la réalité virtuelle, elle permet d'évaluer la mémoire autobiographique antérograde en générant des souvenirs plus riches, en accord avec la définition actualisée de la mémoire épisodique. Elle rend aussi compte des processus mnésiques sous-tendant le fonctionnement normal de la mémoire autobiographique dans le vieillissement. Par ailleurs, l'intervalle de rétention plus long contribue à révéler les difficultés mnésiques des sujets SU, corroborant l'hypothèse selon laquelle le matériel d'évaluation actuel serait inadapté à ce diagnostic.

A ces trois tâches, s'ajoute une tâche de reconnaissance visuelle (la DMS48) développée et étalonnée, en dehors du GREMEM, par Emmanuel Barbeau et ses collègues marseillais, et qui peut être utilisée pour le diagnostic précoce de la maladie d'Alzheimer mais aussi plus largement pour l'évaluation de patients présentant des troubles de la mémoire.

Dans la maladie d'Alzheimer, l'altération de la mémoire autobiographique peut mener à une dégradation du self, c'est-à-dire de l'image de soi. La mise en place d' une intervention cognitive ciblant la mémoire autobiographique apparaît essentielle afin d'aider au maintien du sentiment d' identité. Le but de notre travail est d'évaluer l'impact à court, moyen et long terme (2 mois) d'une version adaptée à la pratique orthophonique libérale du programme de réminiscence autobiographique (RE Mau) conçu par Lalanne, Gallarda et Piolino en 2015. L'objectif étant un maintien des effets sur les composantes épisodique et sémantique de la mémoire autobiographique, l'humeur et la relation de communication avec les aidants. Cette étude de cas multiples compare les résultats obtenus avant et après l'intervention cognitive chez trois patients atteints de la maladie d'Alzheimer à un stade léger. L'analyse des résultats met en évidence une amélioration des performances en mémoire autobiographique et de l'humeur après le programme chez les trois patients. Ces bénéfices se maintiennent après un délai de deux mois pour deux patients sur trois et diminuent légèrement pour le dernier patient. De plus, la charge ressentie par les aidants a diminué après le programme pour deux aidants sur trois. Enfin, la relation de communication entre le malade et son proche s'est légèrement améliorée. Ces bénéfices ressentis par les aidants se sont maintenus deux mois après le programme. Ainsi, le programme REMau semble être une piste de prise en charge des personnes atteintes de MA intéressante pour les orthophonistes. Néanmoins, pour certains cas, la stimulation devrait être poursuivie au-delà du programme. Par ailleurs, il serait intéressant d'adapter le programme REMau aux patients atteints de trouble du langage et notamment de trouble du discours.

En particulier, la mémoire autobiographique, dans ses aspects rétrogrades et antérogrades, pourrait être relativement préservée chez les sujets jeunes (ou« préséniles ») en comparaison avec les sujets plus âgés.Dans ce travail, nous avons évalué les effets de l'âge sur la mémoire autobiographique et spatiale dans la MA. Dix-neuf sujets MA, répartis en trois groupes selon l'âge de début des troubles (avant 65 ans, entre 65 et 75 ans, et au-delà de 75 ans), et 27 sujets contrôles ont été soumis à un protocole mesurant : i) la mémoire autobiographique dans ses composantes sémantique et épisodique à travers quatre périodes de vie, à l'aide d'un questionnaire semistructuré et d'une épreuve de fluence ; ii) la mémoire spatiale, à l'aide d'une épreuve reposant sur l'apprentissage d'un parcours numérisé représentant un trajet en voiture. Le rappel du parcours a été proposé à deux reprises, une heure après l'apprentissage et huit jours après. Les résultats de notre étude sont en accord avec les données de la littérature. Les patients présentent une altération de la mémoire autobiographique quelle que soit la période de vie testée, selon un gradient temporel obéissant à la loi de Ribot. Cependant, les effets de l'âge sont globalement modestes, les sujets les plus jeunes présentant un déficit plus marqué sur la composante sémantique de la première période de vie. Par ailleurs, la mémoire spatiale est très déficitaire, apparaissant dès la phase d'encodage du parcours. Aucune différence sur les performances en mémoire spatiale entre les trois groupes de patients n'a été observée. En conclusion, ce travail ne permet de pas de soutenir l'hypothèse selon laquelle l'âge influe sur la mémoire autobiographique et spatiale dans la MA. En revanche, il confirme l'atteinte sévère de ces systèmes mnésiques, dès les stades légers de cette affection.

The ebernist often first detects a compound, or even a particular functional group in its molecule, by inference from a color reaction, a revealing X-ray diffraction pattern or the formation of a characteristic precipitate. The physician must first suspect the presence of a microbe through certain clinical signs and symptoms before he can verify his diagnosis by looking for a particular organism. It is perhaps not too daring to hope that in our first efforts to clarify the role of hormones in resistance, simple, directly visible indicators might also serve us best.
