La voie inhalée est la principale voie utilisée mais aussi la plus complexe. Il existe une cinquantaine de dispositifs à inhaler, le choix de prescription dépend de multiples facteurs. Le pharmacien est un professionnel de santé primordial dans le suivi de l'observance.Une enquête concernant l'utilisation des dispositifs à inhaler s'est déroulée au sein d'une officine rurale de janvier à juillet 2020 avec un échantillon de 30 patients qui utilisent au moins un dispositif à inhaler. La moyenne d'âge des patients est de 65,8 ans. L'enquête comprenait trois étapes : une partie théorique qui évalue le ressenti du patient vis-à-vis de l'utilisation des dispositifs à inhaler. Une partie pratique qui permet d'évaluer les gestes nécessaires effectués par le patient afin de réaliser une inhalation (les erreurs constatées sont signalées au patient à l'aide d'une grille d'évaluation) et une troisième étape consacrée à la démonstration manuelle par le pharmacien qui reproduit les bons gestes indispensables à la réalisation d'une inhalation efficace.La plupart des patients pensent utiliser correctement les dispositifs à inhaler, mais lors de la pratique, des erreurs communes à tous les dispositifs à inhaler ainsi que des erreurs spécifiques à chaque système d'inhalation sont détectées. Ces erreurs de manipulation peuvent avoir des conséquences plus ou moins sévères dans la prise en charge de la maladie. L'observance dans les traitements à inhaler est primordiale. L'évaluation régulière des gestes indispensables à la réalisation d'une inhalation efficace est nécessaire afin de corriger les erreurs éventuelles. Le pharmacien a pleinement un rôle à jouer à ce niveau.

La mauvaise utilisation des traitements inhalés représente la très grande majorité des facteurs en cause. il existe très peu d'études menées spécifiquement chez des personnes âgées. Notre objectif principal était d'évaluer la qualité de la technique d'utilisation des dispositifs inhalés, chez des patients souffrant de maladies respiratoires chroniques, âgés de plus de 60 ans. Nous avons également tenté de mettre en évidence les facteurs prédictifs d'une mauvaise technique d'utilisation. Nous avons réalisé une étude chez les patients correspondant à nos critères d'inclusion, entre juillet 2012 et Septembre 2014. Après avoir accordé leur consentement éclairé, les patients recrutés ont montré leur technique d'utilisation de leur inhalateur, qui a été évaluée à l'aide d'une liste de contrôle standardisée. Les erreurs ont été classées comme critiques ou non. Les erreurs critiques étaient définies par l'absence de dépôt du médicament au niveau des poumons. Cent patients ont été inclus : 80 étaient hospitalisés au CHU de Rouen et 20 provenaient de 3 officines hautes normandes. De ces patients, 27 (21,1%) ont fait des erreurs critiques lors de la démonstration de la technique d'utilisation. Aucun dispositif ne rendait le patient sensiblement plus susceptible de faire des erreurs critiques. La présence de troubles cognitifs et de troubles moteurs étaient, quant à eux des facteurs prédictifs de mauvaise technique d'inhalation. En moyenne, 32,7 % des étapes d'utilisation d'un inhalateur ont été effectuées incorrectement. Parmi les patients, 76 % ont indiqué avoir déjà reçu des conseils sur la technique d'utilisation d'un inhalateur, mais seulement 26 % d'entre eux avaient reçu des conseils clans les 6 derniers mois. Dans la littérature, l'ADP est source d'erreurs fréquentes en comparaison avec les IPS. Notre taux d'erreurs retrouvées avec I'ADP est globalement sous-estimé du fait de la définition des erreurs critiques retenues. La simplification du mécanisme d'emploi des inhalateurs, et l'évaluation régulière de la technique d'inhalation par les professionnels de santé, au premier rang duquel se trouvent les pharmaciens, pourraient sensiblement améliorer ce constat.

Son contrôle optimal passe entre autres par une bonne manipulation des dispositifs inhalés. Notre étude a évalué l'utilisation de ces dispositifs inhalés chez les patients âgés de 2 à 20 ans en soins primaires ainsi que leurs principales sources d'éducation. Méthode : Notre étude a évalué l'utilisation de 4 dispositifs inhalés (les aérosols doseurs type Ventoline, les aérosols doseurs type Ventoline associés à une chambre d'inhalation, les Diskus et les Turbuhaler) chez 19 patients. Les scores d'utilisation ont été calculés à l'aide de grilles comprenant 8 à 9 étapes selon les dispositifs. Un questionnaire était rempli pour chaque patient reprenant ses différentes sources d'éducation. Résultats : Seulement 4 des 25 démonstrations réalisées par les patients (16%) sur leurs dispositifs ont été faites sans commettre d'erreur. Les erreurs principalement constatées étaient non-dispositifs-dépendantes : notamment l'absence d'expiration complète avant la prise du traitement et l'absence d'apnée de quelques secondes après sa prise. Lors de la prescription initiale, les médecins généralistes informent pour la plupart leur patient sur les modalités d'utilisation des inhalateurs prescrits (68%), mais très peu demandent une démonstration patient (31%). Lors des consultations ultérieurs, 68% des patients ne reçoivent plus aucune explication ou démonstration par leur médecin traitant et 74% des patients ne refont jamais eux-mêmes la démonstration. L'éducation fournie par les pharmaciens est encore moindre : seulement 15% des délivrances initiales donnent lieu à une démonstration du dispositif ; 10% des patients n'auront d'éducation par les pharmaciens après la première délivrance. Conclusion : Les patients de soins primaires ne maitrisent pas leur traitement par dispositif inhalé. L'éducation du patient lors de la prescription initiale de ces traitements n'est pas réalisée systématiquement, que ce soit par le médecin traitant ou par le pharmacien. Cette éducation, déjà précaire, n'est pas renouvelée chez environ 70% des patients.

Book excerpt: Introduction : L'asthme est une maladie respiratoire chronique potentiellement grave et entravant la qualité de vie des patients. Un contrôle satisfaisant passe par une bonne utilisation des dispositifs inhalés. Notre étude a évalué la connaissance d'utilisation des dispositifs inhalés de l'asthme chez les internes de médecine générale et étudiants en pharmacie. Matériels et méthodes : Nous avons évalué la pratique d'utilisation de 43 internes en médecine générale et 43 étudiants en pharmacie de Lille, de trois dispositifs inhalés (aérosol doseur, Turbuhaler, Diskus) lors d'entretien individuel. Les étudiants devaient respecter 8 points d'utilisation pour chacun. Résultats : Les étudiants en médecine et en pharmacie connaissent mal l'utilisation des dispositifs inhalés. Les étudiants en pharmacie ont toutefois des scores moyen supérieur pour chacun des dispositifs testés : 6,3/8 points respectés pour l'aérosol doseur contre 5/8 ; 5,3/8 pour le Turbuhaler contre 3,2/8 ; 6/8 pour le Diskus contre 4,3/8. Les étudiants en pharmacie réalisent plus fréquemment une démonstration à la première délivrance du dispositif ou lors de son renouvellement ; les internes de médecine font plus souvent réaliser une démonstration par le patient lui-même. Conclusion : La formation des étudiants en pharmacie et en médecine de Lille sur l'utilisation des dispositifs inhalés semble judicieuse avec des ateliers de « gestes pratiques », afin de favoriser l'éducation thérapeutique, permettre d'augmenter le contrôle de l'asthme et améliorer les conditions de vie des patients.
