Les pédiatres des urgences sont impliqués dans cette mission difficile. L'étude du devenir à un an des enfants en danger ou en risque de danger consultant aux urgences nous a paru intéressante pour améliorer nos pratiques dans la prise en charge de cette population. Nos objectifs étaient : d'acquérir une meilleure connaissance de cette population, de connaître le "parcours" de l'enfant et/ou de sa famille après le passage aux urgences et de déterminer s'il existait des inadéquations entre la prise en charge attendue et celle réalisée. Matériel et méthode: la population était constituée de l'ensemble des enfants dits "en danger" ou "en risque de l'être" ayant consulté aux urgences pédiatriques du CRHU de Montpellier en 2010. Le recueil de données a été réalisé avec les services sociaux et judiciaires de l'Hérault. Résultats: 87% des 92 enfants inclus présentaient des facteurs de vulnérabilité. Une mesure de protection judiciaire a été nécessaire dans l'année dans 40% des cas avec un placement dans 21 % des cas. Pour 35% des cas, une inadéquation a été constatée entre la prise en charge attendue et celle réalisée. Conclusion: la singularité des situations et les contraintes propres à chaque institution ne permettent pas toujours d'assurer une protection optimale pour l'enfant. Un travail multidisciplinaire et une meilleure communication entre les partenaires paraissent nécessaires pour prévenir les conséquences préjudiciables au développement de l'enfant

La littérature épidémiologique est très pauvre, en particulier pour les enfants consultant aux urgences pédiatriques (UP). Objectifs : Décrire les situations de maltraitance rencontrées dans un service d’UP et analyser les procédures sociales débutées. Matériels et méthodes : Etude rétrospective d’enfants consultant aux UP du CHRU de Lille entre le 11 avril 2009 et le 10 juillet 2010, pour lesquels une maltraitance ou une situation à risque était notée. Résultats : L’étude portait sur 97 enfants : 43 % pour une suspicion de maltraitance sexuelle (MS), 57 % pour une suspicion de maltraitance autre (MA). Dans le cas des enfants ayant une suspicion de MS, 48 % consultaient pour des attouchements et 21 % pour un motif sans rapport avec une MS. L’examen génital était anormal dans 15 % des cas. Un diagnostic final de MS ou non était posé pour 48 % des enfants : 70 % avaient une MS certaine. Les procédures sociales et le devenir des enfants étaient identiques quel que soit le diagnostic final. Dans 90 % des cas, les enfants regagnaient le domicile parental après une simple consultation aux UP. Aucun enfant n’était confié à l’ASE. Dans le cas des enfants ayant une suspicion de MA, 31 % ne consultaient pas pour une MA. Des traces de violence physique étaient notées dans 30 % des cas. Le diagnostic final de MA certaine était posé dans 82 % des cas avec 33 % de maltraitance physique, 15 % psychologique, 24 % de négligence, 60 % d’environnement à risque. Les procédures débutées aux urgences étaient identiques quel que soit le diagnostic final. Dans 44 % des cas, les enfants bénéficiaient d’une simple consultation aux UP. Après hospitalisation, 18 % des enfants étaient confiés à l’ASE. Conclusion : Notre étude montre l’importance de l’activité sociale dans une unité d’UP et reflète les conditions de consultation des enfants maltraités. Elle montre également la difficulté à dépister les enfants maltraités et à poser un diagnostic certain de maltraitance.
