Book excerpt: Résumé en français : Introduction: Le problème de la résistance aux antibiotiques est directement corrélé à leur consommation qui demeure trop élevée en France.L'objectif est de décrire les caractéristiques des prescriptions d'antibiotiques pour les principales pathologies infectieuses prises en charge aux urgences ainsi que leur adéquation avec les recommandations actuelles. Matériels et méthodes: Etude prospective monocentrique observationnelle réalisée sur 6 mois dans un service d'accueil des urgences adultes. Les critères d'inclusion étaient tous les patients de plus de 16 ans pour lesquels une antibiothérapie a été prescrite pour une de ces indications: pneumopathies, infections urinaires, dermo-hypodermites et fasciites nécrosantes, méningites, diarrhées aiguës fébriles, angines, neutropénies fébriles post chimiothérapie, infections sexuellement transmissibles. Le caractère approprié des prescriptions a été évalué en fonction des recommandations locales, nationales et internationales. Résultats : 219 patients ont été inclus. Le sex-ratio était de 0,8 et la moyenne d'âge de 57,7 ans. 114 (52%) patients étaient porteurs d'au moins une comorbidité. 162 (76%) prescriptions étaient réalisées par un médecin sénior et 51 (24%) par un interne.120 (55%) patients ont été hospitalisés. 14 (12%) ont présenté une évolution clinique défavorable au 5ème jour d'hospitalisation. Il existait une corrélation statistiquement significative entre mauvaise évolution clinique au 5ème jour d'hospitalisation et antibiothérapie inappropriée aux urgences (OR= 3,5 ; IC 95% : 1,03 - 16,26). Parmi les patients hospitalisés, 25 (21%) ont présenté un diagnostic final différent de celui posé aux urgences et 11 (9%) ne présentaient pas de processus infectieux nécessitant un traitement par antibiotique. Celle-ci est responsable de 25 000 décès et coûte 1,5 milliard d'euros par an en Europe. Les causes de cette résistance sont liées à une utilisation inappropriée des antibiotiques et à un ralentissement dans la découverte de nouvelles molécules. La mise en route d'une antibiothérapie entraîne des conséquences individuelles et collectives, il apparaît donc nécessaire que celle-ci soit optimale. L'objectif de cette étude est d'évaluer la qualité de l'antibiothérapie à partir de sa prescription aux urgences du centre hospitalier de Cambrai. Méthode : Il s'agit d'une étude prospective, observationnelle, menée durant quatre semaines consécutives, du 21 mars au 17 avril 2016, portant sur l'ensemble des patients ayant reçu au moins une dose d'antibiotiques aux urgences de l'hôpital de Cambrai. L'antibiothérapie à été classée selon la méthode Gyssens. Résultats : 124 dossiers ont été analysés. Ils ont été classés Gyssens 0, c'est-à-dire conformes aux recommandations dans 45.53 % des cas. Les erreurs d'antibiothérapie non acceptables (Gyssens IV ou V), celles ne justifiant pas de l'instauration d'un antibiotique (20,33 %) ou l'utilisation d'une molécule non adaptée (18,7 %), représentaient 39,03 % des cas. Les erreurs d'administration concernaient un mauvais timing lors de l'introduction de l'antibiothérapie (GYSSENS I) dans 9,76 % des situations. Une erreur de voie d'administration (GYSSENS IIc) a été retrouvée dans 1,63 % des dossiers. Les doses étaient incorrectes (GYSSENS IIa) pour 0,81 % des patients, et enfin, la durée de traitement était incorrecte (GYSSENS III) dans 2,44 % des cas. L'origine infectieuse était pulmonaire pour 49,59 % des patients, suivie principalement par les infections digestives (12,2 %) et cutanées (10,57 %). Un référentiel local était applicable dans 78,86 % des situations. Les principaux schémas antibiotiques utilisés concernaient l'association AMOXICILLINE-ACIDE CLAVULANIQUE, représentant 55,28 % des prescriptions, suivi par les Céphalosporines (12,1 %), puis par l'association CÉPHALOSPORINE-FLUOROQUINOLONES (9,76 %). Conclusion : Nos prescriptions d'antibiotiques ne sont pas optimales. Une amélioration apparaît nécessaire, et doit passer par un travail de réflexion multidisciplinaire afin de renforcer l'adhésion des prescripteurs aux référentiels locaux.
