Cette étude rétrospective a révélé une incidence élevée du nombre de SARM importés, soulignant l'intérêt d'un dépistage à l'entrée, en particulier chez les patients provenant de secteur hospitaliers comme les réanimations, ou encore ayant eu des séjours hospitaliers récents et prolongés, des facteurs de risque généraux (diabète) et locaux (plaies cutanées). Cependant, les mesures de prévention de la transmission croisée dans les secteurs de gérontologie se heurtent à certaines difficultés spécifiques de ces secteurs, nécessitent des adaptations et deviennent très délicates à observer dès lors qu'une proportion importante de patients est porteur, l'accent sera alors mis sur les patients porteurs d'une infection symptomatique et l'hygiène des mains sera l'élément essentiel de la prévention. Les programmes de prévention de la diffusion du SARM doivent en permanence être adaptés aux réalités du terrain et faire l'objet de rappels constants par les membres Comité de Lutte contre les Infections Nosocomiales (CLIN) et les services d'hygiène des établissements de soins.

Book excerpt: Screening methicillin-resistant Staphylococcus aureus (MRSA) carriers at the time of hospital admission is justified by the necessity of an early identification of asymptomatic carriers in order to implement control measures as soon as possible. Nevertheless, screening programmes induce extra-costs and screening strategies are controversial, particularly in non-intensive care units. In the present work, we studied different aspects of screening strategies. In addition to conventional risk factors, we identified home care as an independent risk factor of MRSA carriage on admission, and the possibility of hospital-acquired MRSA transmission from carriers to households. Therefore, it seems that the definition of patients with risk of carriage should be revised. We demonstrated that screening on admission to two non-intensive care wards (internal medicine and emergency room) can be interesting for control. We also defined a strategy concerning the choice of anatomical sites for screening. The impact of the screening strategy on the values of certain surveillance indicators was also evaluated. Finally, we presented the evolution of MRSA indicators in two hospitals with contrasted screening strategies. Screening seems to be essential in MRSA control programmes. However, screening would not be effective without the implementation of efficient barrier measures, whose certain are still controversial or not sufficiently validated.

Le rôle de ces germes dans la pathogénie de l'atteinte respiratoire est controversé. Le but de cette étude est d'évaluer l'impact de SAMR et de SA SCV sur l'évolution des enfants atteints de mucoviscidose.Méthode : Il s'agit d'une étude de cohorte rétrospective de type exposés / non-exposés. Chacun des 9 patients exposés à SAMR et des 10 patients exposés à SA SCV a été apparié à 2 patients non-exposés aux germes, de même âge, même sexe, même statut bactériologique pour le pyocyanique, avec des mutations génétiques similaires et avec le même statut de tolérance glucidique. T0 correspond à la date de primoinfection à SAMR ou à SA SCV. Le volume expiré maximal par seconde (VEMS), l'indice de masse corporelle(IMC), le nombre de cures d'antibiotiques et le nombre d'hospitalisations ont été comparés entre les exposés et les non-exposés sur l'année avant T0 et sur l'année après. Résultats : Le déclin du VEMS sur les 2 ans étudiés est plus rapide chez les patients exposés à SAMR ou à SA SCV que chez les non-exposés (SAMR : -12,2% vs 1,6%, p=0,04 ; SA SCV : -12,2% vs 1,2%, p=0,003). Pour les patients exposés à SAMR ou à SA SCV, la variation du VEMS n'est pas plus importante après T0 (p=0,42 et p=0,30). L'IMC ne varie pas de façon significative après T0. Le pourcentage de patients recevant plus decures d'antibiothérapie orale après T0 est plus élevé chez les patients exposés (SAMR : 67% vs 28%, p=0,05 ; SA SCV : 88% vs 44%, p=0,02). Il n'y a pas d'augmentation significative du nombre de cures d'antibiothérapie intraveineuse ou du nombre d'hospitalisation après T0. Conclusion : Il existe une dégradation clinique et fonctionnelle respiratoire plus importante chez les patients infectés à SAMR ou à SA SCV mais les limites de l'étude ne permettent pas de conclure sur l'implication exclusive de ces germes.

Les taux d’importation et d’acquisition étaient respectivement de 12,3% et 44%. Les facteurs prédictifs d’acquisition de SARM retrouvés étaient : la présence d’un tableau infectieux à l’entrée et l’antécédent d’hospitalisation dans les six derniers mois

This is a major concern because pneumococcal infections are common in the community, being the le- ing cause of pneumonia, sinusitis, and meningitis. Resistant bacteria in hospitals are also becoming more prevalent.
