Book excerpt: Alors que la prévalence du diabète augmente chez les personnes âgées, peu d'études se sont intéressées à cette catégorie de patients. Notre travail a ainsi analysé les données du centre de Dijon (à l’inclusion et à 6 mois de suivi), à partir d’une étude nationale française multicentrique en cours sur les diabétiques de type 2 âgés de 70 ans et plus. Il s’agissait de décrire cette population prise en charge en hôpital de jour (HJ) gériatrique ou en HJ de diabétologie au CHU de Dijon. Vingt neuf patients âgés de 70 ans et plus, diabétiques de type 2 depuis au moins 1 an ont été inclus. Le suivi à 6 mois n’a concerné que 19 sujets. Le profil des patients inclus, soit 29 au total, correspondait à un sujet de sexe féminin, âgé de 81.3 ans en moyenne, non fumeur mais présentant un surpoids (indice de masse corporelle moyen : 28.5 kg/m2) avec un syndrome métabolique. L’ancienneté du diabète était de moins de 10 ans pour 53.6 % des cas et 75.8% des patients étaient sous antidiabétiques oraux. L’indépendance fonctionnelle ainsi que l’état nutritionnel étaient satisfaisants mais il existait une fragilité cognitive et thymique. Les objectifs lipidiques, tensionnels et de l’équilibre glycémique, définis par la Haute Autorité de Santé, étaient atteints pour 55 % des patients. Il présentait peu de complications évolutives du diabète. Enfin, à 6 mois, l’hémoglobine glyquée moyenne était meilleure (7.23 contre 7.37%, NS, 19 patients suivis). L’originalité de ce travail était de cibler des patients diabétiques de type 2, âgés de 70 ans et plus, avec une évaluation des facteurs de risque cardio-vasculaires, des complications et des syndromes gériatriques associés.

De nouvelles recommandations ont été publiées récemment concernant cette population dépendante. L'objectif de l'étude était d'évaluer l'équilibre glycémique chez les résidents diabétiques de type 2, atteints de démence sévère. METHODES : il s'agissait d'une étude descriptive, transversale, multicentrique, qui reprenait des données rétrospectives. Les EHPAD et USLD étaient recrutés par le biais d'une société de gériatrie. L'étude incluait les diabétiques de type 2, ayant un MMSE inférieur à 10, vivant en institution depuis au moins un an. Un investigateur unique complétait les questionnaires qui incluaient le dernière HbA1c et glycémie à jeun. RÉSULTATS : 53 résidents furent inclus. La moyenne de la dernière HbA1c était de 7,6 % ± 1,4. 83,7% des résidents traités pharmacologiquement se trouvaient strictement sous 9% d'HbA1c et celle-ci était inférieure ou égale à 7% chez 34,9% des résidents. La moyenne de la dernière glycémie à jeun était de 1,5 g/L ± 0,8. La prévalence du diabète de type 2 était de 17,3% sur 1445 résidents. Seulement 2 dossiers de soins sur 53 précisaient des HbA1c cibles. Il existait 13 prescriptions (29%) de metformine, dont 9 (69%) chez des résidents avec une HbA1c inférieure ou égale à 7%. 4 prescriptions (9%) d'inhibiteur de la dipeptidyl peptidase-4 étaient relevées. Une insulinothérapie était prescrite chez 31 résidents (69%). Une seule prescription de sulfamide était relevée. CONCLUSION : La prise en charge du diabète de type 2 peut être améliorée chez les sujets institutionnalisés, souffrant de démence sévère. Pour cela, nous proposons de sensibiliser les soignants quant aux risques d'hypoglycémies, de définir des objectifs glycémiques dans les dossiers et de privilégier les traitements à faible risque d'hypoglycémie.

Book excerpt: Le diabète de type 2 du sujet âgé est un problème important de santé publique pour lequel le médecin généraliste est souvent amené à intervenir en première ligne. Il s'agit d'une population hétérogène à haut risque de sur-traitement et d'hypoglycémies. Nous avons réalisé une étude dont le but était d'évaluer le niveau de contrôle glycémique (selon l'HbA1c : basse, cible ou élevée) et les facteurs associés aux hypoglycémies, chez des patients diabétiques de type 2 âgés de 70 ans ou plus suivis par le médecin généraliste. Cette étude, observationnelle et prospective a inclus sur 16 mois 100 patients dont 49 vivaient au domicile et 51 en EHPAD. Le contrôle glycémique intensif (HbA1c basse) était lié à une absence de réévaluation thérapeutique chez le patient âgé et comorbide, et associé à un état nutritionnel précaire. Il n'était pas associé à un niveau de pression thérapeutique intensif ni aux hypoglycémies. Dix-neuf pourcent des sujets avaient présenté des hypoglycémies au cours des 6 derniers mois (ressenties ou objectivées). Les facteurs associés de manière significative aux hypoglycémies étaient : le traitement par insuline, le niveau de pression thérapeutique intensif, la présence de complications du diabète et la survenue d'évènements aigus intercurrents. Au total, le traitement intensif du diabète semble être un facteur de risque d'hypoglycémie indépendamment du niveau de contrôle glycémique (taux d'HbA1c). Probablement que définir un taux minimal d'HbA1c favoriserait une désintensification de traitement, qui serait préférable et peu risquée (peu de bénéfices attendus d'un traitement intensif sur le risque de complications en regard de l'espérance de vie).
