Book excerpt: La prise en charge de la douleur post-opératoire s'est nettement améliorée depuis quelques années avec la généralisation des techniques d'analgésie balancée et d'analgésie loco-régionale. En ce qui concerne la douleur post-opératoire immédiate, la méthode de référence est la titration morphinique. Cette méthode permet un administration adaptée à la variabilité inter et intra-individuelle des besoins en morphinique des patients, elle repose sur une auto-évaluation de la douleur par EVA ou EN. Cependant des problèmes persistent : le délai de soulagement des patients est encore trop long et les effets secondaires de la morphine empêchent une prise en charge optimale de la douleur en SSPI. Il nous a paru intéressant de mettre en évidence les facteurs prédictifs de la douleur sévère et de la titration de morphine en SSPI afin de mieux comprendre les facteurs pouvant influencer cette variabilité et éventuellement déboucher sur une amélioration de la prise en charge de la douleur des patients en SSPI. Nous avons réalisé pour cela une étude prospective de ces facteurs prédictifs incluant tous les patients admis en SSPI en post-opératoire de chirurgie orthopédique, urologique, gynécologique et digestive. Les caractéristiques pré-opéraoires des patients (antécédents, information sur la douleur), les caractéristiques per-opératoires (type d'anesthésie de chirurgie, antalgiques reçus) et post-opératoire (douleur, titration, durée de séjour) étaient collectées de manière prospective. L'analyse multivariée de ces paramétres a permis de mettre en évidence trois facteurs prédictifs indépendants : les fortes doses de sufentanyl per-opératoire, la prise d'antalgique pré-opératoire et la rachi-anesthésie. Ces paramètres laissent envisager des solutions thérapeutiques, généraliser l'utilisation des anti-NMDA qui limitent la tolérance aiguë au morphiniques et encourager les techniques d'analgésie loco-régionale . Ceci afin d'améliorer le confort des patients dans une démarche d'assurance qualité.
