Book excerpt: Partie théorique : Dans notre pratique médicale, nous sommes fréquemment confrontés à la présence de comorbidité dépressive dans de nombreuses pathologies chroniques. Le diabète de type 1 en est un exemple. Dans ce contexte, la littérature scientifique a développée des recherches pour peu à peu établir des liens de causalité pouvant expliquer cette prévalence élevée. La psychologie de la santé propose divers modèles mettant le focus sur l'influence de certains « processus transactionnels » du stress secondaire à la maladie chronique, entre autres. Parmi ces modulateurs, un semble avoir un rôle prépondérant : le « coping », développé au départ par les auteurs Lazarus et Folkman et abondamment étudié depuis. Matériels et Méthodes : Le but principal de ce travail est d'essayer de proposer un modèle à la survenue d'état anxiodépressif dans la maladie diabétique de type 1, celui-ci pouvant être médié par les stratégies de coping utilisées par l'individu. Nous émettons aussi l'hypothèse secondaire d'une influence de ces stratégies sur l'équilibre glycémique. Pour cela, nous avons inclus 68 patients diabétiques de type 1 au CHU de Nice Pasteur entre octobre 2013 et juin 2014 en leur faisant remplir les échelles HADS, MINI et CISS. Résultats : Notre étude a pu démontrer l'influence significative du coping centré sur l'émotion sur les états anxieux et dépressifs. Le coping centré sur la diversion sociale est un facteur protecteur de dépression. L'équilibre glycémique mesuré via le dernier taux HbA1c est corrélé au coping centré sur la distraction de manière positive et au coping centré sur la tâche de manière négative. Conclusions : Ces résultats montrent l'intérêt de l'étude du coping, concept multidimensionnel, pouvant avoir un impact bénéfique au plan thérapeutique chez les patients diabétiques de type 1. Ce sujet demande à être encore approfondi par des études ultérieures.
