Méthode : Etude transversale, prospective, multicentrique des caractéristiques de trois populations de patients diabétiques de type 2 : 100 Camerounais, 98 Africains originaires d’Afrique Sub-Saharienne ayant migré en France à l’âge adulte et 199 Caucasiens. Les patients ont été recrutés sur la période de novembre 2005 à décembre 2006 de manière consécutive à l’hôpital de Yaoundé au Cameroun et à dans le service de diabétologie et d’endocrinologie de l’hôpital St Louis à Paris. Résultats : L’étude a montrée que l’âge au diagnostic du diabète était inférieur dans la population des patients Africains immigrés par rapport aux populations des Camerounais et des Caucasiens. Conclusion : Cette étude montre que l’âge au diagnostic du diabète est plus précoce chez les patients immigrés originaires d’Afrique Sub-Saharienne que chez les Caucasiens. Cette différence existe malgré un accès aux soins comparable. Cela signifie que l’âge au diagnostic du diabète chez les immigrés dépend d’autres facteurs tel que l'ethnie, l'environnement et le stress lié à la migration.

Book excerpt: Objectifs : La progression du nombre de diabétiques, n'a cessé d'augmenter partout dans le monde pour atteindre actuellement le chiffre alarmant de 392 millions. La France, terre d'immigration pour de nombreux migrants, détient un taux de prévalence de diabète de type 2 de 4,7 %. Celui-ci constitue une menace pour tous les migrants, exposés à un risque plus important à le développer. Dans ce contexte, il devient primordial, d'étudier leurs propres représentations afin d'améliorer la prise en charge de ces patients migrants atteints de cette maladie. Nous avons centré notre étude particulièrement chez les migrants originaires du Maghreb, d'Afrique subsaharienne et d'Asie. Méthode : Nous avons réalisé une étude qualitative basée sur des entretiens individuels semi-dirigés, chez des patients diabétiques de type 2 migrants de première génération. Ils étaient issus d'une patientèle de médecins généralistes exerçant à Villiers-le-Bel. Nous avons posé des questions relatives, à la perception et au vécu du diabète de type 2 et de ses traitements, en nous basant sur un guide d'entretien préétabli. Une analyse de contenu par analyse thématique et triangulation des données a permis l'extraction des résultats. Résultats : Notre échantillon se composait de douze patients (sept femmes et cinq hommes), dont l'âge moyen était de 57 ans, issus de la patientèle de sept médecins généralistes. Les représentations étaient globalement similaires au sein des trois populations de migrants, notamment, le stress et l'alimentation comme facteur déclenchant du diabète. La religion et la médecine traditionnelle avaient une place prépondérante dans la vie de ces migrants. Enfin le niveau de scolarisation avait un impact sur leurs connaissances. Conclusion : Il est primordial d'intégrer, à notre exercice quotidien, les représentations des patients ayant une culture différente de la notre, ceci afin d'améliorer la prise en charge de certains malades notamment celle du diabète de type 2 véritable fléau dans ces populations souvent précaires.

Book excerpt: Le diabète de type 2 est en pleine expansion dans le monde, particulièrement dans les pays en développement, tels l'Asie et Afrique. Les migrants ont une forte prévalence du diabète du fait d'une occidentalisation de leur mode de vie. L'éducation diététique a une place essentielle dans la prise en charge des diabétiques. Nous avons voulu évaluer le type d'alimentation des migrants africains diabétique de type 2 suivis dans le service d'endocrinologie de l'hôpital Saint-Louis, sur le plan quantitatif et qualitatif, et sa place par rapport aux recommandations, comparativement à un groupe contrôle de patients européens. L'hôpital Saint-Louis est situé dans le nord-est parisien avec un recrutement important de patients originaires d'Afrique subsaharienne. Durant 6 mois nous avons inclus 28 européens et 28 africains diabétiques consécutifs. Un profil médicosocial, des données anthropomorphiques et biologiques ont été recueillis pour ces patients, couplés à une enquête alimentaire. Les données qualitatives ont été apportées par un questionnaire alimentaire de fréquence et les données quantitatives par un rappel alimentaire des dernières 24h. Les résultats nous montrent que les patients de notre groupe africain sont plus jeunes, moins androïdes, ont une situation sociale plus précaire, un diabète plus récent, plus insulino traité et aussi compliqué que les européens. Leurs habitudes alimentaires sont aussi très différentes. Pourtant, l'équilibre glycémique (8.5%) est identique dans les deux groupes, de même que les apports nutritionnels quantitatifs, qui sont proches des recommandations. Les nutriments sont donc répartis et apportés en quantité identique mais par des aliments différents. 86% des diabétiques sont suivis en médecine générale où sont délivrés des messages diététiques. Un outil pratique tel le questionnaire de fréquence utilisé dans notre étude permet une évaluation rapide de la typologie alimentaire en consultation. Ainsi les messages diététiques délivrés en consultation peuvent être ciblés selon les particularités alimentaires de populations différentes.
