Il a permis de montrer qu'il n'existe manifestement pas de relation simple et transparente entre les performances des personnes DTA dans la réalisation de ce type de tâche et les troubles cognitifs, mais que des processus spécifiques (planification et organisation de l'action, stockage des buts et des représentations en mémoire de travail) et non uniquement un facteur général (les ressources attentionnelles disponibles) sont impliqués dans la réalisation des activités courantes.

Book excerpt: La maladie d’Alzheimer (MA) est une maladie neurodégénérative insidieuse qui entraîne l’apparition de déficits cognitifs menant à un déclin graduel de l’autonomie fonctionnelle. Ce déclin fonctionnel peut affecter négativement le bien-être des patients vivant avec la MA, qui est dépendant du sentiment d’autonomie, de compétence et d’affiliation, selon la théorie d’autodétermination. Des interventions ciblant les atteintes fonctionnelles dans la MA ont donc été développées. Le type d’intervention démontrant le meilleur potentiel auprès de cette population est la réadaptation cognitive (RC). Il s’agit d’une approche individualisée selon les incapacités du fonctionnement quotidien et le profil d’atteintes cognitives. Les études existantes sur la RC ont démontré des améliorations de la performance sur les activités de la vie quotidienne qui étaient ciblées par la RC. Cela dit, ces études présentaient des limites méthodologiques, telles que des mesures subjectives de la performance et l’utilisation d’interventions non individualisées et non liées à des tâches de la vie quotidienne. Thivierge et collaborateurs (2014) ont pallié certaines de ces limites en élaborant un programme de RC qui a démontré l’amélioration objective de la performance pour une activité instrumentale de la vie quotidienne (AIVQ) altérée chez un groupe de participants vivant avec la MA en stade léger. Le programme de RC incluait les techniques cognitives de la pratique répétée avec rappel espacé et apprentissage sans erreur. Malgré les effets bénéfiques sur la performance, la RC n’avait pas amélioré la qualité de vie des participants et un effet secondaire indésirable avait été enregistré (Brunelle-Hamann, Thivierge & Simard, 2015). Avant de poursuivre la recherche sur la RC, il était donc primordial de préciser certains aspects. C’est donc dans ce cadre que s’inscrit la présente thèse. Cette thèse a trois objectifs principaux. D’abord, elle vise à justifier le développement d’interventions de RC ciblant les activités de la vie quotidienne auprès de la population MA. Une revue critique de la littérature a été effectuée afin de préciser les atteintes fonctionnelles des activités instrumentales de la vie quotidienne (AIVQ) dans les premiers stades de la MA. Dans un second temps, cette thèse vise à vérifier si un programme de RC pour réapprendre deux AIVQ est sécuritaire auprès des patients dans les stades léger et modéré. Finalement, le troisième objectif est d’évaluer l’efficacité d’un programme de RC pour améliorer la qualité de vie, l’estime de soi et la satisfaction de soi et pour améliorer la performance à deux AIVQ altérées et significatives pour les patients en stades léger et modéré de la MA. Cette thèse est novatrice, car elle est la première à évaluer objectivement à l’aide d’une mesure valide, fidèle et écologique, l’efficacité d’une RC individualisée à domicile, chez les patients MA en stade léger et modéré, pour améliorer deux AIVQ significatives et altérées. Elle est la première à évaluer l’innocuité d’un programme de RC pour deux AIVQ en contexte de MA. Elle introduit des mesures du bien-être, dont l’estime de soi et la satisfaction de soi, mesures importantes lorsque l’on considère l’implantation d’une telle intervention à plus large échelle et mesures supportées par un modèle théorique et recommandées pour les interventions dans la démence. Les résultats de la présente thèse ont démontré que les atteintes fonctionnelles sont présentes de façon précoce dans les stades léger et modéré de la MA et affectent en premier lieu les activités complexes de la vie quotidienne (AIVQ). Les interventions ciblant ces atteintes sont pertinentes afin de favoriser l’autonomie et le bien-être des patients MA. Les résultats recueillis dans le cadre de cette thèse ont permis d’établir qu’un programme de RC ciblant deux AIVQ était sécuritaire et bien toléré pour les patients MA et pour leur proche aidant. Finalement, la valeur surajoutée de la réadaptation de deux AIVQ altérées, comparativement à une seule AIVQ, a été identifiée à l’aide d’une étude randomisée contrôlée. La RC pour deux AIVQ permet d’améliorer objectivement la performance à ces activités et de prévenir le déclin fonctionnel global jusqu’à trois mois après la fin de l’intervention et entraine une amélioration de l’estime de soi. Et ce, malgré les déficits de la mémoire épisodique des participants et l’évolution du déclin cognitif.

La préoccupation de l'aidant naturel est alors de préserver la sécurité de son proche en supervisant, identifiant et anticipant les difficultés auxquelles il s'expose. Cette responsabilité implicite, qualifiée de vigilance, fait partie intégrante de l'accompagnement des aidants. Le premier objectif de cette thèse est d'évaluer les situations à risque dans un échantillon de patients français vus en consultation. Les résultats indiquent que tous les patients de l'échantillon sont confrontés aux risques à domicile. Les risques les plus fréquents sont liés au feu, à l'alimentation et aux médicaments. La Grille d'Evaluation de la Sécurité s'avère être un outil simple, rapide et pertinent à introduire au sein des consultations gériatriques bien que son utilisation requière certaines précautions. Elle permet d'orienter l'entretien avec l'aidant vers une prévention des accidents domestiques et d'améliorer la qualité de vie à domicile. Le deuxième objectif empirique de ce travail est d'explorer la perception de vigilance des aidants accompagnant leur proche en consultation. Les résultats mettent en évidence des sous-groupes d'aidants avec des niveaux de vigilance très élevés, se déclarant "en garde" 24H/24. Les motifs de la surveillance à domicile évoqués par les aidants concernent souvent la sécurité de leur proche. Le troisième objectif est d'explorer les facteurs associés avec la vigilance de l'aidant. Le niveau de vigilance est associé avec certaines caractéristiques du patient (niveau de dépendance dans les actes de la vie quotidienne, risques encourus) et avec des caractéristiques propres à l'aidant (sentiment de fardeau, troubles de l'humeur)... Des pistes sont proposées afin d'améliorer la sécurité à domicile des patients déments, mais également pour soutenir les aidants repérés comme étant particulièrement vulnérables au niveau psychologique.
