Book excerpt: Drug overdose, driven largely by overdose related to the use of opioids, is now the leading cause of unintentional injury death in the United States. The ongoing opioid crisis lies at the intersection of two public health challenges: reducing the burden of suffering from pain and containing the rising toll of the harms that can arise from the use of opioid medications. Chronic pain and opioid use disorder both represent complex human conditions affecting millions of Americans and causing untold disability and loss of function. In the context of the growing opioid problem, the U.S. Food and Drug Administration (FDA) launched an Opioids Action Plan in early 2016. As part of this plan, the FDA asked the National Academies of Sciences, Engineering, and Medicine to convene a committee to update the state of the science on pain research, care, and education and to identify actions the FDA and others can take to respond to the opioid epidemic, with a particular focus on informing FDA's development of a formal method for incorporating individual and societal considerations into its risk-benefit framework for opioid approval and monitoring.

Book excerpt: IRVING LEVESON The 19608 was a period of rapid social and economic change, coupled with spectacular growth in the role of government in dealing with social issues. The demands of public programs and policies created enormous pressures for improved information and analyses. In the field of drug abuse these pressures were compounded by the rapid rise of drug use and the absence of much critical information and analysis. The most elementary steps to develop a data base were just beginning, and many years of effort would be required before the accumulation of knowledge could produce. a strong foundation for public policy. There has been enormous progress in improving knowledge about drug abuse since the mid-1960s. However, as in many fields, research has concentrated on a few questions while others largely are ignored, and even where studies exist there are problems of assimilation. Information is widely dispersed, not always accessible and often not in the most useful form. Many analysts do not have adequate understanding of the ways in which studies might enter into policy development, while at the same time, persons responsible for policy formation often do not have sufficient knowledge of how to use research to help resolve policy issues. A major objective of this volume is to illustrate the formation of linkages which may bridge the gap between research and policy.

Book excerpt: Mental, neurological, and substance use disorders are common, highly disabling, and associated with significant premature mortality. The impact of these disorders on the social and economic well-being of individuals, families, and societies is large, growing, and underestimated. Despite this burden, these disorders have been systematically neglected, particularly in low- and middle-income countries, with pitifully small contributions to scaling up cost-effective prevention and treatment strategies. Systematically compiling the substantial existing knowledge to address this inequity is the central goal of this volume. This evidence-base can help policy makers in resource-constrained settings as they prioritize programs and interventions to address these disorders.

Book excerpt: The opioid crisis in the United States has come about because of excessive use of these drugs for both legal and illicit purposes and unprecedented levels of consequent opioid use disorder (OUD). More than 2 million people in the United States are estimated to have OUD, which is caused by prolonged use of prescription opioids, heroin, or other illicit opioids. OUD is a life-threatening condition associated with a 20-fold greater risk of early death due to overdose, infectious diseases, trauma, and suicide. Mortality related to OUD continues to escalate as this public health crisis gathers momentum across the country, with opioid overdoses killing more than 47,000 people in 2017 in the United States. Efforts to date have made no real headway in stemming this crisis, in large part because tools that already existâ€"like evidence-based medicationsâ€"are not being deployed to maximum impact. To support the dissemination of accurate patient-focused information about treatments for addiction, and to help provide scientific solutions to the current opioid crisis, this report studies the evidence base on medication assisted treatment (MAT) for OUD. It examines available evidence on the range of parameters and circumstances in which MAT can be effectively delivered and identifies additional research needed.

Available in PDF, EPUB and Kindle. Book excerpt: The Report continues to provide in depth trend analysis of the four main drug markets in its first section. In addition, to mark the one hundred year anniversary of the Shanghai Opium Commission, and one hundred years of international drug control, the Report contains an in-depth look at the development of the international drug control system. The Report also contains a small statistical annex which provides a detailed look at production, prices and consumption. As in previous years, the present Report is based on data obtained primarily from the annual reports questionnaire (ARQ) sent by Governments to UNODC in 2007, supplemented by other sources when necessary and where available. Two of the main limitations herein are: (i) that ARQ reporting is not systematic enough, both in terms of number of countries responding and of content, and (ii) that most countries lack the adequate monitoring systems required to produce reliable, comprehensive and internationally comparable data. National monitoring systems are, however, improving and UNODC has contributed to this process.

This book was released on 1995-02-01 with total page 251 pages. Available in PDF, EPUB and Kindle. Book excerpt: For nearly three decades, methadone hydrochloride has been the primary means of treating opiate addiction. Today, about 115,000 people receive such treatment, and thousands more have benefited from it in the past. Even though methadone's effectiveness has been well established, its use remains controversial, a fact reflected by the extensive regulation of its manufacturing, labeling, distribution, and use. The Food and Drug Administration regulates the safety and effectiveness of methadone, as it does for all drugs, and the Drug Enforcement Administration regulates it as a controlled substance. However, methadone is also subjected to a unique additional tier of regulation that prescribes how and under what circumstances it may be used to treat opiate addiction. Federal Regulation of Methadone Treatment examines current Department of Health and Human Services standards for narcotic addiction treatment and the regulation of methadone treatment programs pursuant to those standards. The book includes an evaluation of the effect of federal regulations on the provision of methadone treatment services and an exploration of options for modifying the regulations to allow optimal clinical practice. The volume also includes an assessment of alternatives to the existing regulations.

Available in PDF, EPUB and Kindle. Book excerpt: Heroin is a worldwide scourge and a seemingly intractable one. The Life of the Heroin User: Typical Beginnings, Trajectories and Outcomes is the first book to apply a biographical approach to the lifecycle of the heroin user from birth until death. Chapters address each stage of the user's life, including childhood, routes to use, the development of dependence, problems arising from addiction, death and options for treatment and prevention. Drawing on over two decades of experience in the field of opiate research, Shane Darke examines major theoretical approaches to the development of opiate dependence and the efficacy of treatment options for opiate dependence. Key points are presented at the end of each chapter. The most detailed review available of what is likely to happen to the dependent heroin user, this is an important book for clinicians, researchers and students in the fields of drug and alcohol studies and public health.

The annual Report, produced by the United Nations Office on Drugs and Crime, is a contribution towards that objective. It provides an overview of the illicit drug situation worldwide and regionally, analytical discussions of the key transnational drug markets and statistical trends for all major drug categories.
