Book excerpt: Objective: To evaluate the aesthetic outcome of immediate dental implantation and immediate non-functional loading utilizing definitive abutments in the aesthetic zone, with and without bony substitutes filling the peri-implant gap.Methods: in this prospective clinical trial, 12 implants were placed in 11 subjects (6 males, 5 females) utilizing a flapless immediate post extraction approach in the esthetic area of the maxilla (15 to 25). All participants were above 21 years old and medically fit. Atraumatic extraction was performed for teeth that were indicated for extraction keeping an intact facial bone plate. Implants were placed free hand with a gap of at least 2 mm from the inner surface of the facial plate bucco-lingually, and in the range of 3u20134 mm from the future gingival margin in a corono-apical direction (Buser et al. 2004). The gap between the implant and the inner surface of the facial plate was either left without grafting or filled with natural bovine bone mineral granules based on a coin toss method regardless of the size of the gap. Upon achieving a torque of uf0b3 30 Ncm, immediate non-functional loading was performed utilizing a definitive abutment as in a one-abutment-at-one-time protocol (Canullo et.al 2010). Definitive crowns fabrication was performed 12 weeks post implant placement. Pink Esthetic Score (PES) (Furhauser et al. 2005) was assessed preoperatively (PES0), 1 year after delivery of definitive crowns (PES1), and at 2 years follow up (PES2). Dental casts were made at those points of time as well. Those casts were scanned using an imes-icore table-top scanner, and the scans were registered using surface to surface registration and then signed closest point distances were measured. Surface models were cropped to include one tooth mesial and distal to the region of interest (implant site) in an attempt to remove the outliers that may result from differences else were on that cast. All image analysis steps were done using Slicer CMF. Appropriate statistical analyses were performed.Results: The range of the gap left between the implant and the inner surface of the facial plate varied from 2mm to 4mm. All implants remained integrated and restorations were functional at the end of a two year follow up period. The means of PES score were as following: PES0= 9.5uf0b11.68, PES1=9.5uf0b11.98, PES2=9.83uf0b12.89. There were no statistically significant differences in PES scores at different points of time both collectively (P=0.08 for PES0 vs PES1, P=0.14 for PES0 vs PES2, and P=0.61 for PES1 vs PES2), and when compared for the grafting group versus non-grafting group (P=0.24 for PES2 graft vs. PES2 non-grafting). As for casts scans, the surface distances between the 2 time points for all cases were less than 1 mm in all reference planes and there were no statistically significant differences. This reflects a stable surgical outcome of this approach.Conclusions: Immediate placement and immediate non-functional loading utilizing a definitive abutment in a one-abutment-at one-time protocol in the esthetic area of the maxilla appears to result in a favorable and stable result as far as esthetic outcome, and alveolar process sufficiency are concerned. It seems grafting the horizontal gap has no additional benefit in promoting better clinical outcomes given that the provisional restoration is meticulously fabricated and strict selection criteria are applied for such cases. Of notice is the limitation present in this study as the sample number is small as this is a pilot clinical trial.

Immediate prosthetic provisionalization not only is capable on its own of shortening treatment time, but also plays an important role in conditioning the soft tissues during healing with the provisional prosthetic restoration.AimThe presentation of a case report where one immediate and one delayed implant were placed in the same patient. Both implants were immediately restored with non- functional, provisional restorations. A 10-year follow up of the case will be presented.Materials and MethodsA female patient, aged 19 years old, underwent orthodontic therapy, during which teeth 12 and 22 presented with root resorption. Both teeth were scheduled to be extracted and replaced with dental implants. Tooth #12 was extracted at a prior date by her general dentist 6 months prior to implant placement. Tooth #22 was extracted on the day of surgery, and two implants were placed. Bovine bone substitute (Bio-Oss) was placed to cover the gap between the body of the immediately placed implant and the buccal bone. Subsequently, the implants were immediately restored with temporary crowns, non-functionally loaded. ResultsFinal restorations were placed 4 months later, after successful osseointegration of the implants was confirmed. Clinical and radiographical examinations revealed that functional and esthetic results have withstood the test of time. No soft tissue recession was observed at the 10-year follow-up. The appropriate preoperative evaluation of the morphology of the alveolar process and the periodontal biotype, the surgical planning which provided a guide for proper implant placement, management of the peri-implant gap and the peri-implant soft tissues led to a successful result.ConclusionImmediate implant placement along with immediate provisional restoration in the maxillary anterior is a predictable approach with good long-term results, as long as proper treatment planning is performed.

Are included in this study only from incisors to canine area in upper and lower jaw and evaluated each one after 60 months.Materials & MethodsAfter a minimally traumatic extraction of the tooth, a different surgical approach was used.In the first DEALAYED study group (10 individual cases), depending on the presence of thin or thick gingiva and on the residual volume of buccal plate, the implant was placed with conventional protocol and delayed loading. In the second IMMEDIATE LOADING study group (10 individual cases), after the tooth extraction the implant was immediately placed and loaded without functional loading. In both study groups, if bone loss occurred in the buccal plate, an integration with GBR techniques and the use of biomaterials were necessary to fill the buccal gap. Good primary stability of the implant and a non-functional load were achieved in all loading cases.ConclusionsThe follow-up after 60 months, performed by clinical probing and radiographic evaluation and also supported by TC scan, shows that the results are comparable and that post-extractive implants with immediate non functional loading can be used in selected cases.The results obtained are good in both thin and thick biotype cases and it seems evident that it is possible to obtain predictable long term functional and aesthetic success. An accurate previous evaluation and study of each single case as biotype, hard and soft tissue and as well as residual buccal plate are always considered essential requirement criteria to obtain this data.

The difference between these two means was not significant statistically (P=0.6882). Conclusions: Immediate implant placement and restoration appear to be a valid treatment option when using guided surgery to obtain satisfactory esthetic results.

All implants were immediately restored using zirconia Copy-abutments and followed-up for 1 yearof function.Results:No difference in median Pink Esthetic Scores (PES) were observed between the defect and non-defect group (11.5 vs. 12.0,p=0.336). A higher rate of alveolar deficiency (PES-5, +29%) and soft tissue discoloration (PES-7, +17%) in implants with bonydefects and a weak correlation betweeen PES and defect size (r=0.71, p=0.018) was seen. No difference regarding mucosalrecession (0.6 vs. 0.8 mm) as well as marginal bone loss (1.6 vs. 1.4 mm) could be observed.Conclusions and Clinical Implications:No differences regarding esthetic outcome and peri-implant bone remodeling could be substantiated between immediate implants insockets with vs. without buccal bone defects, however, implant esthetics may be less predictable in cases of bony deficiencies.

This procedure offers several advantages for the patient as well as for the clinician, including shorter treatment time and fewer surgical procedures. A correct three-dimesional implant position and a management of peri-implant gap are key factors in order to achieve successful functional and esthetic outcomes.The aim of the present 1-year prospective single cohort study was to evaluate the success rate, marginal bone level (MBL) changes and soft tissue stability of tissue-level implants placed in fresh extraction sockets.This prospective cohort study included a total of 10 patients (7 women and 3 men) ranging in age from 23- to 63-years-old (mean 40.1u00b113.3) with at least one tooth in need of tooth extraction and of immediate implant placement. Tooth extractions and implant placements were carried out with a flapless procedure. The tissue level implants (Prama, Sweden & Martina, Due Carrare, Italy) were placed with the machined-collar at the buccal bone wall level and the implants healed transgingivally. The peri-implant bone defects between the implant surface and bone walls were grafted with corticocancellous porcine bone particles and stabilized with a resorbable membrane. Five months after placement the implants were restored. Outcome evaluations were: implant success, MBL changes, width of keratinized gingiva (WKG), facial soft tissue (FST) levels, modified Plaque Index and modified Bleeding Index. The level of statistical significance was set at 0.05 for all analyses.10 Implants were placed in 10 patients, no complication occurred during the healing, no immediate post-operative infections were observed. No patient had implant failure within the first year of treatment. Therefore, a total of 10 patients with 10 implants were followed for the study period. The cumulative survival rate was 100% at 1 year-evaluation. The mean MBL was -0.6u00b10.49 mm at baseline and 0.3u00b10.36 mm at 1 year follow up. The FST Level was 0.4u00b10.69 mm at baseline and 0.02u00b10.70 mm at 1-year follow-up. The Width of Keratinized Gingiva was 3.8u00b1 0.47 mm at baseline and 4.1u00b10.42 mm at 1-year follow-up. Modified bleeding and plaque indexes had low values during the course of the study.The use of an implant with a machined-transgingival concave neck has shown satisfying results in terms of hard and soft tissue levels at 1-year evaluation. Particularly the increase of Width of Keratinized Gingiva can lead to a stable and esthetically successful results. Immediate implant placement can be successfully performed with a tissue-level implant according to a correct three-dimensional position and to a correct management of peri-implant soft tissue healing.

Book excerpt: The longevity and functionality of dental implants depends on both, osseointegration around the implant and the establishment of a soft tissue barrier which protects the underlying hard tissue structures and the implant itself.1 The healing of soft tissues around the trans-mucosal portion of an implant after surgery starts with the formation of a blood clot and the induction of an inflammatory process which leads to tissue formation and remodeling.2 Taking into account that mucosae healing, in an oral environment, is challenging due to the consistent exposure to microorganisms, the rapid formation of an efficient soft-tissue seal is therefore crucial for the establishment and maintenance of peri-implant tissue health status.3 In the case of implants in the anterior region, the esthetics of the implant prosthesis becomes critical to the acceptance of the implant treatment, therefore, soft tissue control is absolutely fundamental.1 In this context, in cases of indication of extraction of anterior teeth, the insertion of immediate implants has become a clinical routine for the implant dentist. When the parameters for implant placement into fresh alveoli are respected, the esthetic results are extremely satisfactory, reducing consultation time, promoting a positive psychological impact on the patient and the maintenance of peri-implant mucosa.4Furthermore, immediate loading can achieve primary stability by using the implant insertion torque of at least 40 Ncm.5 This technique has a failure rate similar to conventional implants6 with less alveolar bone crest loss.7 To achieve these results, the alveolar bone must be intact in a site with thick gingival biotype and adequate three-dimensional positioning of the implant in the alveolus.Immediate loading directly influences the soft tissue framework around the implant. The maintenance of the peri-implant tissue through the manipulation of the temporary implant-supported prothesis is quite important for gingival health and esthetics of the region where the implant was inserted.8However, the needs of patients are diverse and esthetics is an increasingly important demand. In clinical practice, not all patients can be submitted to extraction with immediate implant placement and loading. In cases of low predictability, when the placement of a temporary implant-supported prothesis jeopardizes the success of the immediate implant, how should one proceed to maintain the gingival structure?This is a case report that describes an alternative treatment for the healing and maintenance of the peri-implant mucosa in an esthetically significant region, through the use of a customized abutment to obtain an emergency profile when there is insufficient torque for immediate implant loading.

Book excerpt: The comparison of dental implant surfaces and their effect on osseointegration in the aesthetic zone ; Clinical and Radiographic case reportBackground: Recent studies have encouraged a progressive shortening of the healing period for single-tooth implants and immediate loading has been proposed for the aesthetic zone in the maxilla. The biological fixation between the dental implant surfaces and jaw bones should be considered a prerequisite for the long-term success of implant-supported prostheses.Aim/Hypothesis : The aim of this case is to compare the effectiveness of immediate single implants with different surface in the aesthetic zone.Materials and Methods : Patients requiring a single tooth implant-supported restoration in the maxillary esthetic zone received the implant with rough surface and combine surface (rough surface + machined surface) placed in healed sites. The combine implant surfaces is also called hybrid implant. Implants that achieved an insertion torque of at least 35 Ncm were immediately non-occlusally loaded. The screw-retained provisional crown was delivered within 7 hours following the surgery and non functionally loaded. The definitive prosthesis was delivered within 2 months after implant insertion. Two months later the osteointegration progress was evaluated in all installed implants using: Dental X-rays and Osstellu2122 system.Results : After two months period of healing, high level of integration was observed. The stability of these implants was measured using Osstellu2122 system. These implants showed higher values ; ISQ 75 (hybrid) and 76 (rough surface), respectively. It also observed a good soft tissue adaptation clinically. No complications were observed. Radiographic bone changes were similar in both dental implants.Conclusions and Clinical implications :Both implants is a valuable treatment option for immediate implant placement and loading of single crowns in the maxillary esthetic zone.

Book excerpt: The introduction of cone beam computed tomography has promoted the realization of guided surgery in dental implant treatment.The aim of this study was to present clinical results of treatment with implants inserted by the technique of guided surgery and immediate loading in edentulous jaws.Fully edentulous maxilla patients were diagnosed by cone beam tomography and treated with 10 Galimplant 3D u00ae implants for rehabilitation by the technique of guided surgery and immediate loading. Immediately after surgery a full resin provisional immediate restoration was placed. At 6 months, the final restoration was performed. The clinical follow-up period was at least 60 months of functional loading.30 patients were treated with 300 implants. The clinical findings demonstrate an excellent clinical response of this type of protocol, free of postoperative complications in patients undergoing surgery. From an implantologic point of view, the results indicate a success rate of 98.4%. Five implants were lost during the healing period with the provisional prosthesis. After a mean functioning period of 77.8 months from the final restoration, there was no late complications.This study indicates that treatment with dental implants by guided surgery in the upper jaw is a high success rate therapeutic alternative.
