Improving social conditions remains critical to improving health outcomes, and integrating social care into health care delivery is more relevant than ever in the context of the pandemic and increased strains placed on the U.S. health care system. The report and its related products ultimately aim to help improve health and health equity, during COVID-19 and beyond. The consistent and compelling evidence on how social determinants shape health has led to a growing recognition throughout the health care sector that improving health and health equity is likely to depend â€" at least in part â€" on mitigating adverse social determinants. This recognition has been bolstered by a shift in the health care sector towards value-based payment, which incentivizes improved health outcomes for persons and populations rather than service delivery alone. The combined result of these changes has been a growing emphasis on health care systems addressing patients' social risk factors and social needs with the aim of improving health outcomes. This may involve health care systems linking individual patients with government and community social services, but important questions need to be answered about when and how health care systems should integrate social care into their practices and what kinds of infrastructure are required to facilitate such activities. Integrating Social Care into the Delivery of Health Care: Moving Upstream to Improve the Nation's Health examines the potential for integrating services addressing social needs and the social determinants of health into the delivery of health care to achieve better health outcomes. As a result, many govern ment department and agencies are investigating the impacts of specific projects and are examining the role impact analysis could play in project planning. The process of environmental impact analysis has been developed, tested and institutionalized in several countries. The objective of the process is a prior identification and definition of likely environmental impacts of projects such as public works, industrial developments and tourist develop ments, as well as the impact of policies and legislative proposals. The environmental impact analysis process also includes the definition of alternative courses of action which would achieve comparable economic objectives while eliminatir .. g some or all of the detrimental environmental consequences. Identification of preventive or precautionary measures, which would minimize the unavoidable impacts, fonn an integral part of the process. The aim should be for a balanced appraisal in which economic, technical, social, environmental and health aspects are fully evaluated. But for many American communities, pre-disaster conditions are far from optimal. Large segments of the U.S. population suffer from preventable health problems, experience inequitable access to services, and rely on overburdened health systems. A return to pre-event conditions in such cases may be short-sighted given the high costs - both economic and social - of poor health. Instead, it is important to understand that the disaster recovery process offers a series of unique and valuable opportunities to improve on the status quo. Capitalizing on these opportunities can advance the long-term health, resilience, and sustainability of communities - thereby better preparing them for future challenges.
