Book excerpt: La laryngectomie totale est un acte chirurgical mutilant à visée curative en cas de cancer du larynx dont l'une des principales séquelles est la perte définitive de la voix laryngée. L'orthophonie s'inscrit au centre de la rééducation post chirurgicale : son principal objectif est d'accompagner le patient dans la découverte et la maîtrise d'une voix de substitution fonctionnelle. La voix oro-oesophagienne, basée sur l'éructation contrôlée, est une méthode phonatoire alternative conseillée à tous les patients laryngectomisés. Cependant, son apprentissage se heurte à des manifestations anatomo-physiologiques et des comportements néfastes à son bon développement : tensions musculaires, troubles du schéma corporel, crispation, attitude de forçage. D'autre part, certains thérapeutes expriment leur réticence à assurer ces rééducations. Face à la nécessité d'améliorer et de démocratiser ce type de prise en charge, nous avons évalué les effets d'un travail de détente en supplément d'une rééducation ordinaire. Notre intervention visait à évaluer l'influence d'un protocole d'exercices de détente sur la progression des aptitudes vocales des patients et à déterminer son moment d'administration le plus bénéfique. L'étude a concernée 13 sujets pris en charge dans un cadre institutionnel et répartis en deux groupes. Notre protocole s'est déroulé en deux phases : durant chacune un groupe recevait le traitement expérimental et l'autre une rééducation classique. Chaque sujet a été évalué en pré, mid et post-test grâce à un questionnaire interrogeant son confort, la technicité et la fonctionnalité de sa voix. Nos résultats ne permettent pas de conclure à un effet de notre protocole sur l'efficacité de leur voix oesophagienne. Cependant, il a démontré une réelle motivation chez les sujets à investir la voix oro-oesophagienne et a objectivé des tendances de variation de leur rythme de progression au fil de la rééducation. Par ailleurs, il ouvre la réflexion sur les éventuels bénéfices d'approches thérapeutiques visant le relâchement musculaire et l'éveil proprioceptif dans l'éducation à la voix oesophagienne
