Book excerpt: Cause très fréquente de consultation pédiatrique, l'otite moyenne aiguë (OMA) est une surinfection bactérienne de l'oreille moyenne. C'est la première cause de prescription d'antibiotiques en pédiatrie et la première infection bactérienne de l'enfant tous âges confondus. Les bactéries les plus fréquemment responsables d'OMA sont le pneumocoque et l'Haemophilus influenzae. En France, depuis l'introduction des vaccins pneumococciques conjugués 7 et 13 valent, l'épidémiologie des OMA s'est modifiée. Ces changements sont liés à l'utilisation de ces vaccins et à la pression de sélection exercée par les antibiotiques sur les souches de pneumocoque. De plus, en 2011, la Société de Pathologie Infectieuse de Langue Française (SPILF) a révisé le traitement des OMA. Le groupe ACTIV a réalisé une étude entre Septembre 2013 et Mars 2014 dont le but était d'actualiser l'épidémiologie des OMA en France chez l'enfant. Parmi les 5 375 OMA enregistrées dans cette série, 76,6% étaient des OMA chez des enfants de moins de 3 ans et 23,4% des enfants avaient plus de 3 ans. Ces derniers présentaient moins souvent de syndrome otite-conjonctivite (10,4%) que les enfants plus jeunes (22,1%). Les otites fébriles (fièvre ≥ 38,5) et douloureuses représentaient 27,5% des cas chez les enfants de moins de 3 ans et 27,3% des cas chez les enfants de plus de 3 ans (p=0,86). La distribution des antibiotiques pris avant l'épisode d'OMA ou ceux prescrits le jour de l'OMA étaient conformes aux nouvelles recommandations ORL de 2011 (diminution de la prescription de cefpodoxime proxetil au profit de la prescription d'amoxicilline). Les taux d'échecs d'otite étaient plus fréquents chez les plus jeunes enfants par rapport aux plus grands (7,3% vs 2,5%). Les résultats de cette étude montrent comment sont suivies les recommandations et comment elles sont appliquées en pédiatrie ambulatoire.

Conclusion: Le diagnostic de l'otite moyenne aiguë semblait ètre fait par excès, si l'on considérait les critères cliniques de l'AFSSAPS. Le tabagisme parental était un facteur perturbateur dans le respect des recommandations. Pour les enfants de parents non-tabagiques, le jour de la semaine, la prise d'antibiotique récente et les antécédents d'otite devenaient prépondérants dans le non-respect des recommandations. Ceci doit ètre tempéré par le fait que l'enquète s'est déroulée sur un mode déclaratif. Seuls les internes en SASPAS ont participé à l'étude et ils étaient au fait des recommandations. Ces deux raisons rendent difficile l'interprétation et interdisent la généralisation de ces conclusions à l'ensemble de la profession.

En revanche, on assiste à une très franche augmentation de la résistance aux antibiotiques de ces germes et particulièrement pour le pneumocoque, qui représentait 65% des souches de sensibilité altérée à la pénicilline, en 1996, en France. Lors d’une étude rouennaise réalisée en 1995-1996, chez 81 enfants présentant une otite moyenne aiguë traitée par une antibiothérapie, un prélèvement bactériologique de l’oreille et du nasopharynx sont réalisés simultanément chez la moitié d’entre eux. L’analyse comparative des résultats montre que la situation d’échec clinique reste peu dépendante de l’efficacité bactériologique in vitro de l’antibiotique. La responsabilité d’une co-infection virale et/ou bactérienne pourrait être la cause. L’index inhibiteur de chaque antibiotique semble le mieux corrélé aux résultats bactériologiques in vivo. Le pneumocoque de sensibilité altérée à la pénicilline est le germe le plus souvent responsable d’échec bactériologique. L’étude des valeurs prédictives du prélèvement nasopharyngé ne permet pas une utilisation seule dans la documentation bactériologique des otites moyennes aiguës. Mais la similitude phénotypique des souches bactériennes retrouvées dans les deux sites confirme son grand intérêt épidémiologique.
