Book excerpt: Le plan de périnatalité 2005-2007 a établi des objectifs nationaux d’amélioration des indicateurs périnataux par l’intermédiaire, entre autres, de l’entretien prénatal précoce (EPP) dès le quatrième mois de grossesse. L’EPP devait permettre d’améliorer la sécurité émotionnelle de la femme enceinte et du couple à travers une prise en charge globale et cohérente de l’anténatal au post-natal. La Haute Autorité de Santé française recommandait que l’EPP soit proposé systématiquement lors de la déclaration de grossesse. L’Agence Régionale de Santé bas-normande a fait de sa mise en place une priorité. L’évolution de la démographie médicale a conduit les médecins généralistes à se réapproprier le suivi de grossesses à bas risque. L’objectif principal de cette étude était d’évaluer le taux de connaissance de l’existence de l’EPP par les médecins généralistes de Basse-Normandie, premiers acteurs, en théorie, de sa promotion sur le terrain. Nous avons réalisé une étude quantitative descriptive par questionnaire téléphonique sur un échantillon randomisé stratifié de médecins généralistes libéraux bas-normands. Sur 80 médecins interrogés, 27,5 % connaissaient l’existence de l’EPP [IC95 %, 18,9-38,1]. Ce résultat était comparable aux taux de connaissance retrouvés dans la littérature compris entre 23,0 % et 39,4 %. Les femmes interrogées connaissaient significativement mieux l’EPP que les hommes et les médecins connaissant l’EPP était significativement plus jeune que ceux qui ne le connaissaient pas. Ainsi, l’EPP semblait méconnu des médecins généralistes bas-normands malgré la promotion et les formations assurées par le réseau régional de périnatalité auprès de ces professionnels.

Leurs conditions de rémunération et d’exercice libéral exclusif, ainsi que la non possibilité de se former à l’EPP sur leur territoire rendent difficile l’intégration de cet entretien dans leurs pratiques quotidiennes.
