Book excerpt: Mexico’s modern middle class emerged in the decades after World War II, a period of spectacular economic growth and social change. Though little studied, the middle class now accounts for one in five Mexican households. This path-breaking book explores the changing fortunes and political transformation of the middle class, especially during the last two decades, as Mexico has adopted new, market-oriented economic policies and has abandoned one-party rule. Blending the personal narratives of middle-class Mexicans with analyses of national surveys of households and voters, Dennis Gilbert traces the development of the middle class since the 1940s. He describes how middle-class Mexicans were affected by the economic upheavals of the 1980s and 1990s and examines their shifting relations with the ruling Partido Revolucionario Institucional (PRI). Long faithful to the PRI, the middle class gradually grew disenchanted. Gilbert examines middle-class reactions to the 1968 Tlatelolco massacre, the 1982 debt crisis, the government’s feeble response to the 1985 Mexico City earthquake, and its brazen manipulation of the vote count in the 1988 presidential election. Drawing on detailed interviews with Mexican families, he describes the effects of the 1994–95 peso crisis on middle-class households and their economic and political responses to it. His analysis of exit poll data from the 2000 elections shows that the lopsided middle-class vote in favor of opposition candidate Vicente Fox played a critical role in the election that drove the PRI from power after seven decades. The book closes with an epilogue on the middle class and the July 2006 presidential elections.

Currently, the richest 1% own 45% of the world’s wealth. The consequence is that some people are able to enjoy healthy, wealthy, happy lives whilst others continue to live in ill-health, poverty and sorrow. Rapid economic growth in Asia (particularly China and India) has lifted many people out of extreme poverty. Nevertheless, the wealth divide is steadily growing. According to Oxfam, between 2009 and 2018, the number of billionaires it took to equal the wealth of the world’s poorest 50 percent fell from 380 to 26. Those with extreme wealth have often accumulated their fortunes on the backs of people around the world who work for poor wages and under dangerous conditions. Women are scarce at the top and overrepresented at the bottom. Gender discrimination in the workplace contributes significantly to these persistent economic divides. There are also large differences in wealth across racial groups. Long-standing racial discrimination in many forms, including in education, hiring, and pay practices contribute to persistent earnings gaps. Inequalities have dramatically strengthened the economic and political power of those individuals at the top.

A prevalência de doenças não transmissíveis não ocorreu como esperado, tendo em vista a queda na mortalidade; o melhor acesso a serviços de diagnósticos entre as pessoas mais riscas pode estar mascarando mudanças na prevalência efetiva. Gastos catastróficos com saúde caíram na maior parte dos países. No entanto, o quadro com respeito à equidade é mais misto e indica limitações na medição. Embora a taxa de empobrecimento em razão de gastos com saúde seja baixa ou em declínio, 2 a 4 milhões de pessoas nos países estudados ainda ficam abaixo da linha da pobreza em razões de gastos com saúde. Esforços de monitoramento sistemático da qualidade do atendimento de saúde na região são ainda incipientes. Book excerpt: Over the past three decades, many countries of Latin America and the Caribbean have recognized health as a human right. Since the early 2000s, 46 million more people in the countries studied are covered by health programs with explicit guarantees of affordable care. Reforms have been accompanied by a rise in public spending for health, financed largely from general revenues that prioritized or explicitly target the population without capacity to pay. Political commitment has generally translated into larger budgets as well as passage of legislation that ring-fenced funding for health. Most countries have prioritized cost-effective primary care and adopted purchasing methods that incentivize efficiency and accountability for results, and that give stewards of the health sector greater leverage to steer providers to deliver on public health priorities. Evidence from the analysis of 54 household surveys corroborates that investments in extending coverage are yielding results. Though the poor still have worse health outcomes than the rich, disparities have narrowed considerably - particularly in the early stage of the life course. Countries have reached high levels of coverage and equity in utilization of maternal and child health services; coverage of noncommunicable disease interventions is not as high and service utilization is still skewed toward the better off. Catastrophic health expenditures have declined in most countries; the picture regarding equity, however, is mixed. While the rate of impoverishment owing to health-care expenditures is low and generally declining, 2-4 million people in the countries studied still fall below the poverty line after health spending. Efforts to systematically monitor quality of care in the region are still in their infancy. Nonetheless, a review of the literature reveals important shortcomings in quality of care, as well as substantial differences across subsystems. Improving quality of care and ensuring sustainability of investments in health remain an unfinished agenda.

Cumplir con el compromiso de la cobertura universal de salud exigira esfuerzos concertados para mejorar la generaci6n de los ingresos fiscales de manera sostenible y de elevar la productividad y efectividad del gasto. En el re po rte Hacia la cobertura universal en salud v la equidad en America Latina v el Caribe: Evidencia de poises seleccionados. los autores sefialan que la evidencia tomada de un analisis de 54 encuestas de hogares corrobora que las inversiones en la ampliaci6n de la cobertura estan generando resultados. A pesar que los pobres aun presentan los peores fndices de salud en comparaci6n con los ricos, las disparidades se han reducido en gran medida, especialmente durante las primeras etapas de vida. Los pafses han alcanzado elevados niveles de cobertura y equidad en la utilizaci6n de los servicios de salud maternoinfantil. La cobertura de las intervenciones de enfermedades no transmisibles no es muy alta y la utilizaci6n de los servicios se inclina hacia aquellos que gozan de una mejor posici6n. La prevalencia de las enfermedades no transmisibles no ha mostrado el comportamiento esperado en vista de la cafda en las tasas de mortalidad: un mayor acceso a los servicios de diagn6stico por pa rte de los segmentos mas ricos podrfa encubrir cambios en la prevalencia real. Los gastos de salud provocados por situaciones catastr6ficas han ido en descenso en la mayorfa de los pafses. El panorama en torno a la equidad, sin embargo es mixto, apuntando hacia limitaciones en la medida. Si bien la tasa de empobrecimiento a causa de gastos de salud es baja, con tendencia decreciente en terminos generales, entre dos y cuatro millones de personas en los pafses estudiados aun caen por debajo de la lfnea de la pobreza despues de haber incurrido en gastos de salud. Los esfuerzos para darle un seguimiento sistematico a la calidad de la atenci6n en la region, apenas dan sus primeros pasos. Sin embargo, un repaso a la literatura revela graves deficiencias en la calidad de la atenci6n de salud, asi como sustanciales diferencias entre los diversos subsistemas. Book excerpt: There is no question that European colonization introduced smallpox, measles, and other infectious diseases to the Americas, causing considerable harm and death to indigenous peoples. But though these diseases were devastating, their impact has been widely exaggerated. Warfare, enslavement, land expropriation, removals, erasure of identity, and other factors undermined Native populations. These factors worked in a deadly cabal with germs to cause epidemics, exacerbate mortality, and curtail population recovery. Beyond Germs: Native Depopulation in North America challenges the “virgin soil” hypothesis that was used for decades to explain the decimation of the indigenous people of North America. This hypothesis argues that the massive depopulation of the New World was caused primarily by diseases brought by European colonists that infected Native populations lacking immunity to foreign pathogens. In Beyond Germs, contributors expertly argue that blaming germs lets Europeans off the hook for the enormous number of Native American deaths that occurred after 1492. Archaeologists, anthropologists, and historians come together in this cutting-edge volume to report a wide variety of other factors in the decline in the indigenous population, including genocide, forced labor, and population dislocation. These factors led to what the editors describe in their introduction as “systemic structural violence” on the Native populations of North America. While we may never know the full extent of Native depopulation during the colonial period because the evidence available for indigenous communities is notoriously slim and problematic, what is certain is that a generation of scholars has significantly overemphasized disease as the cause of depopulation and has downplayed the active role of Europeans in inciting wars, destroying livelihoods, and erasing identities.

Book excerpt: The author implements several inequality decomposition methods to measure the extent to which total household income disparities can be attributable to sectoral asymmetries and differences in skill endowments. The results show that at least half of total household inequality in Mexico is attributable to incomes derived from entrepreneurial activities, an income source rarely scrutinized in the inequality literature. He shows that education (skills) endowments are unevenly distributed among the Mexican population, with positive shifts in the market returns to schooling associated with increases in inequality. Asymmetries in the allocation of education explain around 20 percent of overall household income disparities in Mexico during the 1990s. Moreover, the proportion of inequality attributable to education endowments increases during stable periods and reduces during the crisis. This pattern is explained by shifts in returns to schooling rather than changes in the distribution of skills. Applying the same techniques to decompose within-sector income differences, the author finds that skill endowments can account for as much as 25 percent of earnings disparities but as little as 5 percent of dispersion in other income sources.

Book excerpt: In 2012, the Latin America and the Caribbean (LAC) region continued its successful drive to reduce poverty and build the middle class. Poverty reduction was accompanied by strong income growth of the bottom 40 percent of the population, the World Bank’s indicator of shared prosperity. However, the recent economic slowdown and stagnation in inequality decline suggest that future social gains may be more difficult to achieve. Given the modest prospects ahead, the region’s poverty reduction strategy needs to focus on restoring growth and preserving macroeconomic stability, while reinforcing the ability of less advantaged groups to participate in and contribute to growth. This report assesses two relevant policy areas: equity of fiscal policy and equal access to basic goods and services for children that open the opportunity for them to lead lives of their choosing. Drawing on results from the Commitment to Equity project, the report shows that fiscal policy remains an underused instrument in terms of level and incidence of taxation and spending. The Human Opportunity Index underscores that opportunities are expanding for children in the region, but large gaps remain in access and quality.
