Book excerpt: This facilitators' guide for training community health workers (CHWs) and community volunteers (CVs) is intended for training in tuberculosis (TB) and integration of TB prevention and care services into community-based activities. The training lasts 3 days. It includes a PowerPoint slide set which is an integral part of the guide. Together these allow the facilitator to progress carefully from one idea to the next. This document is not for use directly by CHWs or CVs. The training includes six training modules. It starts with introductions objectives and norms followed by a brief presentation of the ENGAGE-TB approach which will enable CHWs and CVs to integrate TB activities into their existing work. The third module allows substantial time for CHWs and CVs to understand the basics of TB: its signs and symptoms and how it can be prevented and treated. The fourth module deals with integration of community-based TB services into community work. This is followed by a field visit to a TB clinic so that CHWs understand how clinical and laboratory aspects are handled. Finally the CHWs and CVs reflect on what they have learnt and describe how they will integrate TB services into their work on their return home.

It includes data on case notifications and treatment outcomes from 200 national TB control programmes, and an analysis of plans, budgets, expenditures, and progress in DOTS (Directly Observed Therapy - Short Course) strategies for 22 high-burden countries. It summarises progress on initiatives including the development of public-private partnerships in TB control, human resource development, the management of drug-resistant TB, and collaborations in TB and HIV/AIDS control. Findings include that in 2005, there were 8.8 million new cases of TB, and an estimated 1.6 million deaths from TB including those co-infected with HIV. The TB incidence rate was stable or falling in all six WHO regions and had reached a peak worldwide. However, the total number of new TB cases was still rising slowly, because the case-load continued to grow in the African, Eastern Mediterranean and South-east Asia regions.

The key audience for these guidelines are policy-makers in ministries of health or managers of national TB programs who formulate country-specific TB treatment guidelines or who plan TB treatment programs. In addition, health professionals - including doctors, nurses, and educators working both in government services and nongovernmental organizations, such as technical agencies that are treating patients and organizing treatment services - will find these guidelines to be useful.
