Book excerpt: Although the physiology of the menstrual cycle appears clear and easily explained by a balance in the concentration of various sex steroid hormones, numerous details of its mechanism are still poorly understood and little is known about the relationship among clinical events, plasma hormone concentrations, molecular impacts on target tissues and their regulation. In the following chapters, the authors have attempted to establish a correlation between endometrial histology and well-understood physiologic events of the menstrual cycle. They have provided up-to-date information on the effects of various hormones and combinations of hormones on the endometrium. The interdependence of endometrial morphology, molecular biology, endocrinology and physiology, provides grounds for a better understanding of the complex mechanism of the menstrual cycle, and sheds some light on its pathophysiology. Book excerpt: The incidence of endometrial cancer rose sharply in the United States in the early 1970s, paralleling changes in the use of postmenopausal estrogens by American women. A sizeable body of evidence supports the role of both excessive endogenous estrogen and exogenous estrogen in the etiology of endometrial cancer. There is growing evidence that inadequate progesterone has the opposite effect, in that progesterone supplementation of postmeno pausal estrogen therapy reduces the incidence of endometrial cancer. Despite this new awareness of the hormonal role that is played in carcino ma of the endometrium, the disease still plagues the oncologist. The general approach to carcinoma of the endometrium in the United States is that of primary surgical staging. This provides the maximum amount of information to best plan postoperative radiation therapy and/or chemotherapy for these patients. In general, patients who are considered candidates for surgical staging are those with advanced disease or high-risk stage I endometrial carcinoma. High-risk endometrial carcinoma is defined as those patients with moderately differentiated lesions with deep myometrial invasion, poor ly differentiated carcinoma of the endometrium, and the high-risk histolo gies such as papillary carcinoma and clear-cell carcinomas. The surgical staging has extended in most institutions to patients with occult stage II carcinoma of the endometrium, i. e.

It is unfortunate to note that the global 5-year survival rate for patients with advanced or recurrent endometrial carcinoma has improved only slightly. Therefore any complacency regarding this 'benign malignancy' should be reconsidered. There is a growing awareness of the nature of end ometrial carcinoma, with advances in our knowledge ranging from its etiology through its epidemiology to its clinical findings. This volume has been designed to fill a hiatus in the literature in China. Multiple major medical societies recommend fertility preservation counseling prior to starting any gender-affirming therapies, but data are limited on the reproductive effects of common gender-affirming hormone regimens. The burden of fertility counseling falls to the hormone providers and surgeons that are encountering these patients, many of whom will not have had adequate training or resources to provide evidence-based recommendations and options. Additionally, many reproductive health care providers are not trained in how to care for gender minorities. The purpose of this book is to be a reference for clinicians and researchers in the field of transgender medicine, to provide up-to-date data and resources to properly counsel transgender and nonbinary patients about the reproductive consequences of gender-affirming interventions and their options for family-building, and to educate providers about appropriate and culturally competent reproductive health care.
