Human, veterinary, and crop health practitioners convinced themselves that disease could be controlled by medicating the sick, vaccinating those at risk, and eradicating the parts of the biosphere responsible for disease transmission. Evolutionary biologists assured themselves that coevolution between pathogens and hosts provided a firewall against disease emergence in new hosts. Most climate scientists made no connection between climate changes and disease. None of these traditional perspectives anticipated the onslaught of emerging infectious diseases confronting humanity today. As this book reveals, a new understanding of the evolution of pathogen-host systems, called the Stockholm Paradigm, explains what is happening. The planet is a minefield of pathogens with preexisting capacities to infect susceptible but unexposed hosts, needing only the opportunity for contact. Climate change has always been the major catalyst for such new opportunities, because it disrupts local ecosystem structure and allows pathogens and hosts to move. Once pathogens expand to new hosts, novel variants may emerge, each with new infection capacities. Mathematical models and real-world examples uniformly support these ideas. Emerging disease is thus one of the greatest climate change–related threats confronting humanity. Even without deadly global catastrophes on the scale of the 1918 Spanish Influenza pandemic, emerging diseases cost humanity more than a trillion dollars per year in treatment and lost productivity. But while time is short, the danger is great, and we are largely unprepared, the Stockholm Paradigm offers hope for managing the crisis. By using the DAMA (document, assess, monitor, act) protocol, we can “anticipate to mitigate” emerging disease, buying time and saving money while we search for more effective ways to cope with this challenge.

All too often, responses can become restricted by implicit assumptions about who or what is to blame that may not capture the dynamics and uncertainties at play in the multi-scale interactions of people, animals and microbes. As a result, policies intended to forestall epidemics may fail, and may even further threaten health, livelihoods and human rights. The book takes a unique approach by focusing on how different policy-makers, scientists, and local populations construct alternative narratives-accounts of the causes and appropriate responses to outbreaks- about epidemics at the global, national and local level. The contrast between emergency-oriented, top-down responses to what are perceived as potentially global outbreaks and longer-term approaches to diseases, such as AIDS, which may now be considered endemic, is highlighted. Case studies-on avian influenza, SARS, obesity, H1N1 influenza, HIV/AIDS, tuberculosis, and haemorrhagic fevers-cover a broad historical, geographical and biological range. As this book explores, it is often the most vulnerable members of a population-the poor, the social excluded and the already ill-who are likely to suffer most from epidemic diseases. Disease detection and diagnosis is the act of discovering a novel, emerging, or reemerging disease or disease event and identifying its cause. Diagnosis is "the cornerstone of effective disease control and prevention efforts, including surveillance." Disease surveillance and detection relies heavily on the astute individual: the clinician, veterinarian, plant pathologist, farmer, livestock manager, or agricultural extension agent who notices something unusual, atypical, or suspicious and brings this discovery in a timely way to the attention of an appropriate representative of human public health, veterinary medicine, or agriculture. The workshop examined case studies such as the specific case of Arctic region anthrax outbreaks, as a known, observed risk as well as other types of human and animal microbial health risks that have been discovered in snow, ice, or permafrost environments, or that could conceivably exist. The workshop primarily addressed two sources of emerging infectious diseases in the arctic: (1) new diseases likely to emerge in the Arctic as a result of climate change (such as vector-borne diseases) and (2) ancient and endemic diseases likely to emerge in the Arctic specifically as a result of permafrost thaw. Participants also considered key research that could advance knowledge including critical tools for improving observations, and surveillance to advance understanding of these risks, and to facilitate and implement effective early warning systems. Lessons learned from efforts to address emerging or re-emerging microbial threats elsewhere in the world were also discussed. This publication summarizes the presentation and discussion of the workshop.

The International Health Regulations (IHR) establish risk communicationâ€"the real-time exchange of information, advice, and opinions between experts or officials and people who face a threat to their survival, health, and economic or social well-beingâ€"as a core capacity that World Health Organization member states must fulfill to strengthen the fight against these threats. Despite global recognition of the importance of complying with IHR, 67 percent of signatory countries report themselves as not compliant. By investing in communication capacity, public health and government officials and civil society organizations facing health crises would be prepared to provide advice, information, and reassurance to the public as well as to rapidly develop messages and community engagement activities that are coordinated and take into account social and behavioral dynamics among all sectors. To learn about current national and international efforts to develop the capacity to communicate effectively during times of infectious disease outbreaks, and to explore gaps in the research agenda that may help address communication needs to advance the field, the Forum on Microbial Threats of the National Academies of Sciences, Engineering, and Medicine convened a 1.5 day workshop on December 13 and 14, 2016, in Washington, DC. Participants reviewed progress and needs in strengthening communication capacity for dealing with infectious disease threats for both outbreaks and routine challenges in the United States and abroad. This publication summarizes the presentations and discussions from the workshop.
