Book excerpt: The goal of this research was to apply thick-film screen printing technology to produce a single-use, disposable, cost-effective electrochemical biosensor prototype in large scale. Iridium nanoparticles supported by carbon were selected to modify the electrode of the biosensor for its excellent catalytic effect towards the commonly detected electrochemical active species, i.e., hydrogen peroxide, nicotinamide adenine dinucleotide (reduced form, NADH), and nicotinamide adenine dinucleotide (oxidized form, NAD+). The development of this electrochemical biosensor prototype can establish a platform technology for various analytes of clinical importance. This study focused on the biosensing of the following analytes related to diabetes, i.e., 3-hydroxybutyrate (3HB), fructosyl valine, and the HbA1c, providing an analytical tool for diabetic patient management. The first part of this study discusses the importance of developing a point-of-care amperometric biosensor to detect 3-hydroxybutyrate (3HB) for diabetic patient management. Current electrochemical detection methods for 3HB require at least two stepwise reactions or a mediator. The detection method in this study only requires a single reaction step without any mediator, which can potentially also be more accurate, sensitive, cost-effective and stable over the long term. In this detection method, the enzyme D-3-hydroxybutyrate dehydrogenase (3HBDH, EC 1.1.1.30) was immobilized on the iridium0́3modified sensor prototypes which detect the NADH produced by the reaction of 3HB with NAD+ in the presence of 3HBDH. This microelectrode quantified the NADH electrochemically, which produced an electrical current that would then be used to quantify the concentration of 3HB. The interferences from uric acid, NAD+, and serum were measured. It was concluded that the level of 3HB could still be quantified well in the presence of these interfering species. Spectrometric measurements of NADH and 3HB were performed in both PBS and bovine serum and correlated very well with the electrochemical measurements of 3HB, using our biosensor prototype. The relationship between the sensing performance of this 3HB biosensor and the element properties in fabrication and the characterization parameters were assessed. As a result, the value for the element properties in fabrication and the parameters in the characterization were determined based on the experimental results together with the requirements for practical applications. Finally, inkjet printing technology was applied for the enzyme ink deposition for the mass-production of this biosensor. The biosensors from inkjet printing were characterized in both PBS and bovine serum. In the next part of this study, this biosensor prototype was used for the detection of NAD+, in order to quantify the interference from AcAc on the 3HB detection, and to quantify numerous biomarkers related to NAD+. This biosensor operated in the reduction mode at a relatively low electrochemical potential ( -0.4V vs Ag/AgCl reference electrode) in both buffer solution and bovine serum. The biosensor outputs showed high sensitivity, high linearity, and high reproducibility. The temperature effect and the interference from NADH were also assessed. As a separate study, the measurement of glycosylated hemoglobin (HbA1c) was considered to be most important for the long-term management of diabetic patients. Electrochemical detection of HbA1c was proposed involving multi-step decomposition of HbA1c to small molecules proportional to the concentration of HbA1c, then detection of the produced molecules amperometricly. This biosensor prototype was used for the detection of both HbA1c, and fructosyl valine (FV), one of the produced small molecules as a model compound for the HbA1c detection. Consequently, electrochemical biosensors for the detection of FV and HbA1c were developed. Quantifications of FV and HbA1c were carried out at a relatively low electrochemical potential (+0.25V vs Ag/AgCl) at an ambient temperature to detect the enzymatically produced H2O2. The enzymes, e.g., fructosyl amine oxidase (E.C. 1.5.3.x, FAO) and the lysis buffer were then co-immobilized onto the iridium-modified disposable sensor for HbA1c detection in whole human blood. This reagent-free electrochemical biosensor detected HbA1c in the physiological range with practical sensitivity and good linearity. Finally, a diffusion-reaction model was developed for the electrochemical detection of 3HB in solution which achieved reasonable agreement with the experimental results.

Cardiac troponin I (cTnI), as a subunit of troponin, is highly sensitive to cardiac injury; therefore, the cTnI level is used as an index to diagnose myocardial damage, particularly acute myocardial infarction. It can be also used in cardiospecific diagnosis, risk stratification therapeutic treatment and post risk management. In this research, an amperometric immunosensor was developed based on planar electrode and sandwich ELISA format. The electrical response corresponding to biological information was obtained via four main procedures, including electrode modification, immunoreaction, signal amplifications and amperometric detection. Enzyme labels such as horseradish peroxide (HRP) and alkaline phosphatase (ALP) were used for signals amplification. Since alkaline phosphatase works better in low background current levels and has great reproducibility, it was used for nanomaterials, chitosan, gold nanoparticle, carbon nanotube as electrode modification investigation. The anti-cTnI antibody is detectable by electrochemical technology. Necessary conditions and interferences of the experiment were examined. Detection range was from 0.001 ng ml-1 to 300 ng ml-1 on PDDA-MWCNT sensor, and from 0.02 ng ml-1 to 200 ng ml-1 on chitosan-AuNPs sensor. The detection range was investigated using cyclic voltammetry. The signal behavior recorded was linear to cTnI concentration. This behavior makes the developed biosensor be able to widely use in clinical practice. Likewise, two liquid substrates were catalyzed by hydroquinone and 3, 3', 5, 5'-teteramethylbenzidine respectively. Hydrogen peroxide (H2O2) is a product of glucose oxidizes catalyzing the oxidation of -D-glucose by oxygen. It is also used as an oxidizing agent in catalyzing HRP. Hence, an HRP-based immunosensor is important in integrating an immunosensor and an enzyme sensor for the purpose of achieving multianalyte detection compacted on one chip. The cTnI immunosensor developed here is rapid, easy-to-use, cost-efficient and robust.

Book excerpt: The development of rapid pathogenic detection continues to be at the forefront of the greatest challenges of our clinical settings to date. Sepsis is a serious systematic inflammatory response which is triggered by bacterial, viral, or fungal infections. It persists to be a primary cause of patient death despite the medical advances made in therapeutics and antibiotics development. The non-specific symptoms of sepsis are the utmost limitation of diagnosis of the condition. Delays in detection therefore leads to an increase in patient mortality. Current gold standards within microbiology laboratories are too slow, therefore a huge demand exists for a reliable method for early stage detection of sepsis. In this work, the development of biosensors and point-of-care systems for use with electrochemical, centrifugal lab-on-a-disc, Enzyme Linked Oligonucleotide Assay (ELONA), Polymerase Chain Reaction (PCR) and Recombinase Polymerase Amplification (RPA) techniques are demonstrated. A novel integrated point-of-care LoaD system has allowed a real-life application of the sensors to be studied for hospital samples. This innovative system allows pre-loading of reagents and samples which are systematically triggered using spin-frequencies where EIS techniques are adopted to give a sample to answer in under 15 minutes. These devices are single use, therefore are ideal for quick, infectious sample testing. A full optimisation of the device was required as fluid manipulation of various samples is a key-feature where samples ranging from buffer to whole blood can be tested within the same device without the need for sample pre-treatment. A label-free electrochemical biosensor, capable of capture and detection of pathogens, was produced by using a self-assembled monolayer and antibody capture layer was fabricated and characterised to compare with the single use biosensor developed for this work. Real-cultured samples from a microbiological clinic were studied to determine the capabilities of the device for real point-of-care settings. The device, which is capable of detecting the presence of pathogens, and furthermore the category in which they lie in, has proved to be an exciting and critical step-forward in early stage sepsis detection. Further analysis allowed a specific threshold to be determined, where a change in 300 Ohms to the system signifies the presence of a pathogen on the capture surface due to changes in the interfacial capacitance of the biosensor. Using confocal microscopy, the surface coverage of the captured pathogens was determined, where a directly proportional relationship exists between the number of pathogens captured and the change in the impedance response. This highly novel system is unlike any other commercially available technology for pathogen detection. Other methods have been explored, such as ELONA and DNA amplification. With very low detection limits of 23 CFU/ml for the ELONA, 5 CFU/ml for the PCR assay and finally 9 CFU/ml for the RPA assay. With varying assay times of 195-103 minutes, the assays were highly capable of detecting extremely low counts of E. coli in clean buffer samples. Although laborious they have also been proved to be able to detect specific E. coli pathogens which may lead the way for the development of a secondary analysis tool to specifically identify the causative pathogen for a highly precise diagnosis for patients.

Most of these biomarkers are present in the body with extremely low concentration and should be accurately and continually monitored to evaluate the dynamic physiological responses upon exposure to pathogens or drugs. However, conventional assays such as Enzyme linked immuno-sorbent assay (ELISA) and mass spectrometry can only take static and one time measurements, the process is long, expensive, and requires specialist equipment. Therefore, there is need for an advanced sensing system that is cost-effective, portable, and capable of simultaneously detecting very low levels of specific biomarkers with stable and reusable point of care technology (POCT). The goal of this thesis is to develop hyper sensitive biosensors based on electrochemical detection to recognize analytes a using nanowell array (NWA) electrode technology. NWA electrodes have been shown to enable faster detection of biomolecules with reproducibility much better than other approaches. In this thesis, I used fabricated a NWA on a 6-inch Si wafer by krypton-fluoride (KrF) stepper semiconductor process in order to develop NWA sensor through a combination of low cost and high throughput nano-pattern. Finally 57 uniform and homogeneous NWA electrodes were obtained on a 6-inch wafer. Second, to validate this NWA as a biosensor, several biomarkers were detected and monitored. In this part, first, NWA as immunosensor, antibody was immobilized on the NWA, and alkaline phosphatase (ALP) for bone monitoring and stress induced protein 1 (STIP-1) as ovarian cancer monitoring were used as antigen. Specific current and impedance were measured to quantify these analytes binding to the electrode. The Limit of detection (LOD) and the sensitivity & selectivity is highly increased compared with conventional micro-sized electrodes with reducing non-specific binding. Finally, these results suggest that wafer-scale NWA biosensor is useful for sensing application field for point of care technology because their structure provides for an advantage for detecting molecular binding events.
