The cause behind BPH is not well understood. Extracts of saw palmetto (Serenoa repens) are amongst the most widely used medicinal herbal preparations, used almost exclusively to alleviate urinary symptoms related to BPH. Given that little is known about the beneficial physiological role of saw palmetto as an alternative medication and which of the phytochemicals may be involved in improving the symptoms of BPH, this study investigated the effects of commercially available saw palmetto extracts on smooth muscle contractility in isolated preparations of rat prostate gland.

Abstract: Abstract of thesis entitled Combating Prostate Diseases with Ethnobotanical Drugs: Inhibition of Prostate Cancer Cell Proliferation by Saw Palmetto (Serenoa repens) Extracts Submitted by Chun Wai TAM for the degree of Master of Philosophy at The University of Hong Kong in August 2003 Saw Palmetto (Serenoa repens) is a plant found in the United States. The berries of Saw Palmetto have been used as medicine by native Americans for many years. Recent clinical studies revealed that Saw Palmetto extracts can help alleviate lower urinary tract symptoms arising from benign prostatic hyperplasia, a prostate disease commonly found in elderly males. In the present study, we first evaluate the potential of this ethnobotanical extract in future anti-prostate cancer drug development. We also attempt to characterize the bioactive ingredient(s) and elucidate the mechanism of action. We found that extraction of the berry powder with 2-propanol (either 90%, v/v; or 50%, v/v) resulted in a soluble fraction that could inhibit the proliferation of several prostate cancer cells. These include an androgen-sensitive LNCaP cells and two androgen-insensitive cells, DU 145 and PC-3. No significant changes in the viability of these human prostate cancer cell lines were observed, suggesting that Saw Palmetto extract is not cytotoxic. The inhibitory action seemed to be specific against prostate cancer cells because Saw Palmetto extracts had little or no inhibitory effects on the proliferation of colon, kidney, liver, and breast cancer cells. Preliminary chemical characterization studies revealed that the bioactive ingredient(s) in Saw Palmetto are hydrophobic and may contain aromatic functional groups. They also are quite heat-stable. The effects of Saw Palmetto extracts on several endogenous protein phosphorylation systems in the cytosolic and particulate fractions of LNCaP cells were also investigated. We found that treatment of the cells with Saw Palmetto extract for 4 days resulted in stimulation of the activity of cytosolic protein kinase C. The endogenous phosphorylation of a 92-kDa protein was enhanced 6- to 10- fold. Our results thus indicate that Saw Palmetto berry extracts may contain certain active ingredient(s) which can inhibit prostate cancer cell proliferation. The inhibitory effects may be mediated, at least in part, by some protein kinase C modulated protein phosphorylation systems.
