Similarly, the mental health of family members can influence the experiences of mothers with postpartum depression. This study sought to understand the factors that impact the mother-infant relationship during the postpartum period. Specifically, the study was interested in how the father’s mental health indirectly influenced maternal positive affective involvement. An integrative theoretical approach, consisting of ecological theory and stress process theory, was used to conceptualize how familial factors influenced maternal positive affective involvement during mother-infant interactions. Method The current study used archival data collected to examine the efficacy of a mother-infant treatment group (M-ITG) as an intervention for mothers experiencing postpartum depression (Clark et al., 2003). The study consisted of 147 families whose mothers met the criteria for postpartum depression and had an infant that was less than 6 months old. Participants primarily identified as White, but also identified as Native American, Black, Asian, Other, and Multiethnic. Parents ranged in age from 19 to 44 years old. Data was gathered on mothers’ perceptions of marital satisfaction using the Dyadic Adjustment Scale (DAS), mothers’ experiences of parenting stress with the Parenting Stress Index (PSI), maternal postpartum depression through the Hamilton Depression Rating Scale (HDRS), paternal mental health from the Symptom Checklist – 90 (SCL-90), infant temperament using the PSI, demographic factors, and maternal positive affective involvement during mother-infant interactions with the Parent-Child Early Relational Assessment (P-CERA). Data was analyzed via descriptive, correlational, and predictive statistics. Results and Discussion There were four main findings within the study. The fathers’ levels of education predicted both marital satisfaction and the severity of maternal depression, household income predicted mothers’ experiences of parenting stress, and the fathers’ mental health predicted maternal positive affective involvement. Conclusion The study highlights the importance of assessing and treating each member of the family during the postpartum period. Factors that are associated with fathers impact mothers’ experiences during the postpartum period and indirectly influence mother-infant interactions. It is therefore essential for mental health professionals to develop programs and treatments to address each family member.

Book excerpt: Perinatal psychiatric disorders are a leading cause of maternal morbidity and mortality, with significant adverse consequences for mothers and infants, including negative effects on maternal health and well-being, maternal-infant interactions and attachment, and infant and child development. Accumulating evidence suggests that stressful reproductive events, such as adverse birth outcomes resulting in admission to a neonatal intensive care unit (NICU), may trigger the onset of or exacerbate pre-existing psychiatric symptoms. Currently, in the United States, approximately 10-15% of infants born each year are treated in a NICU, and mothers of NICU infants have been found to have high levels of distress when compared to other postpartum mothers. Maternal-infant attachment, maternal attitudes and adjustment, perceived social support, and healthpromoting behaviors have been shown to predict postpartum depressive and anxiety symptoms, but these constructs have not been well documented in mothers with NICU infants. Additionally, many mothers of NICU infants do not pursue mental health treatment, even when it is indicated, and it remains unclear what barriers or facilitators to engagement in mental health care exist for mothers during this time. The current study examined the relationships between depression, anxiety, maternal-infant attachment and attitudes, perceived social support, and health-promoting behaviors in mothers of NICU infants. Additionally, the current study assessed the attitudes of NICU mothers toward psychological treatment, and explored which of these variables may be associated with barriers or facilitators to engagement in mental health treatment during the postpartum period. One hundred twenty-seven women were recruited from their infants’ NICU bedside at three hospitals in the Philadelphia area and completed self-report measures at enrollment and 8-12 weeks later. Findings revealed that depressive, anxiety, and stress symptoms were negatively correlated with maternalinfant attachment, maternal attitudes and adjustment, perceived social support, and health-promoting behaviors, while depressive, anxiety, and stress symptoms were positively correlated. Results indicated a significant decrease in depressive and anxiety symptoms from initial assessment to follow-up. Moreover, depressive symptoms during NICU admission significantly predicted depressive symptoms at follow-up. Barriers to psychological treatment and attitudes towards treatment were negatively correlated, such that as favorable attitudes towards psychological treatment increased, perceived barriers to treatment decreased. Implications for future research and clinical applications, such as identifying specific targets for intervention within the NICU environment, are discussed.

Based on evidentiary studies, major depression in either parent can interfere with parenting quality and increase the risk of children developing mental, behavioral and social problems. Depression in Parents, Parenting, and Children highlights disparities in the prevalence, identification, treatment, and prevention of parental depression among different sociodemographic populations. It also outlines strategies for effective intervention and identifies the need for a more interdisciplinary approach that takes biological, psychological, behavioral, interpersonal, and social contexts into consideration. A major challenge to the effective management of parental depression is developing a treatment and prevention strategy that can be introduced within a two-generation framework, conducive for parents and their children. Thus far, both the federal and state response to the problem has been fragmented, poorly funded, and lacking proper oversight. This study examines options for widespread implementation of best practices as well as strategies that can be effective in diverse service settings for diverse populations of children and their families. The delivery of adequate screening and successful detection and treatment of a depressive illness and prevention of its effects on parenting and the health of children is a formidable challenge to modern health care systems. This study offers seven solid recommendations designed to increase awareness about and remove barriers to care for both the depressed adult and prevention of effects in the child. The report will be of particular interest to federal health officers, mental and behavioral health providers in diverse parts of health care delivery systems, health policy staff, state legislators, and the general public.

Book excerpt: Research has shown that maternal depression can have serious physical health effects on the developing fetus as well as later cognitive, behavioral, and affective problems in children. One area of clinical significance is the effect of maternal depression across time, including the differential effects of depression on early child development during the transition to motherhood. The present study explored trajectories of maternal depression from pregnancy through 2 years postpartum and their relation to infant affect expression. Data for the study were collected as part of a larger 5-panel longitudinal study on women's transition to motherhood. The present study will us data from the third trimester of pregnancy (T1) and 3 months (T2), 1 year (T3), and 2 years (T4) postpartum. The sample is composed of 120 primarily low-income women and is diverse in terms of ethnicity (62% minority), marital status (64% single), and maternal age (18 - 42 years, M = 26, SD = 5.7). Maternal depression was measured at T1 and T2 using the Edinburgh Postpartum Depression Scale (Cox et al., 1987; Wisner et al., 2002) and at T3 and T4 using the Beck Depression Inventory-II (Beck et al., 1996). Infant affect expression was assessed at T3 and T4 using coded observations from videotaped mother-infant free-play interactions. It was hypothesized that different subsamples or trajectories of maternal depression would emerge, having differential effects on infant affect expression at each time point. Results indicated that a 4-class model best fit the data, including stable-low, stable-high, increasing, and decreasing trajectories. These trajectories of depressive symptoms were not found to have differential associations with infant affect expression at age 1 or age 2. Results from this study further inform clinicians about possible patterns of maternal depression and aid in the planning of interventions directed at preventing or reducing cases of maternal depression and problematic child affect development.
