Book excerpt: Adolescent risky sexual behaviors such as early sexual debut, low rates of condom use, and high rates of partner change are associated with a number of negative health and social outcomes including contracting sexual transmitted infections and unintended pregnancy. Utilizing a sample of 529 African American adolescents, this study examines the contributions of parenting behaviors and family structure as factors associated with engagement in risky sexual behavior. Participants reported few differences in mother's parenting across family structures. While fathers and relatives provided greater responsiveness on average than stepfathers. Variation in parenting fully explained differences in risky sexual behavior for males. Yet family structure differences persisted for females, with females in mother step-father homes continually engaging in greater mean risky sexual behavior than those in mother-father households even after accounting for parental responsiveness and demandingness. Implications for prevention and intervention are discussed.

Book excerpt: The purpose of this study is to explore ethnic identity as a protective factor against sexual activity among a sample of African American adolescents. The study sample consisted of 1084 African American adolescents who were administered a survey comprised of questions about their health, risk behaviors, and their backgrounds. The variables examined were sexual intercourse, ethnic identity, parent-child communication regarding specific topics, mother-adolescent conflict and negative communication. The study also examined family structure, educational aspirations, attitudes about sex, self-esteem, refusal skills, lifetime substance use, perceived peer lifetime sexual behavior, age and gender. Analyses conducted examined the relationship between sexual activity and the independent variables as well as associations between two nominal variables. Relationships between interval/ratio variables were investigated. Multiple logistic regression analyses were used to assess whether the relationship between ethnic identity and lifetime sexual intercourse persisted, controlling for individual, family, and social factors. The findings indicate ethnic identity, and individual, family, and peer factors were related to lifetime sexual intercourse. The effects were stronger for substance use and the influence of peers on sexual behavior. When taking into account the other individual, family, and peer factors, the relationship between ethnic identity and lifetime sexual intercourse was non-significant. Additionally, ethnic identity did not moderate the risk factors explored in this study. Gender differences are clearly delineated in the study with more males having sexual intercourse than females. The results of this study recommend multifaceted interventions that begin as early as elementary school and involve families, schools, communities, and organizations working to promote responsible sexual behavior of adolescents. In addition, the findings of other studies provide support for intervention programs that facilitate the development of ethnic identity, such as youth mentoring programs with a focus on Rites of Passage programs. Interventions should also be developed to include peer groups that are comprised of friends, because of the influence peers have on sexual activity and the engagement in other risk behaviors such as substance use.-- Abstract.

Book excerpt: The enjoyment of physical contact and expressions of sexual feelings are normal experiences of adolescent development but have also been related to negative health consequences, with a higher prevalence among African Americans. Research has focused on reducing behaviors such as sexual activity among adolescents to avoid related health consequences but often fail to address the parental factor. A few studies on parental factors have noted that parent-child communication and parental monitoring are important influencers of adolescent sexual behavior. They also noted that mothers are the main communicators on topics of sex while fathers communicate about general topics. The purpose of this study was to examine the influence of mother-child communication and maternal monitoring on sexual behavior among African American high school students. Secondary data analysis was conducted on a cross-sectional sample of 424 adolescents between 14 and 17 years of age who participated in the Family Adolescent Risk Behavior and Communication Survey. Descriptive statistics, bivariate analyses and multiple regressions were conducted in order to draw conclusions about the influence of main predictors on outcome variables. The statistical significance for analyses was determined at a level of 0.05 alpha. In this study sample, 209 students had engaged in sexual intercourse, 125 students anticipated sexual intercourse in the next year, and 90 students reported to delay sexual intercourse in the next year. The results from multiple regression analyses showed that when different levels of African American adolescent sexual experience are considered (beyond the traditional sexually active/not sexually active dichotomy), the role of mother-child communication and maternal monitoring are not strong based on the three specific hypotheses that were tested. Results suggested that future studies may benefit from a longitudinal rather than a cross sectional design. The study concluded that the nurturing role of mothers can be greatly enhanced when they learn the appropriate skills to guide their adolescents on the path to adulthood with relevant tools to make healthy decisions.-- Abstract.

The research on adolescent sexual behavior finds that youth are more likely to engage in sexual activity at an earlier age and have more partners when they are not raised in two-parent households. This is a concern for African-Americans because they are less likely to live in two-parent families and are also more likely to be sexually active. Using the National Longitudinal Survey of Adolescent Health, we find that adolescents in cohabitating and step-parent households are more likely to engage in sexual activity and have more partners, compared to their counterparts in two-parent households. We also find differential effects by gender, with family structure and context having a stronger effect on young females. This is an important result because it shows the negative effects of family disruptions in the African-American community.

Book excerpt: "Health disparities research has indicated that urban, low-income African-American adolescents experience ecological and contextual factors like community violence, socioeconomic status, and limited sexual health knowledge which contribute to high rates of sexual risk among urban African American adolescents. However, protective factors like parental monitoring and parent-adolescent communication about sex may decrease sexual risk among this urban teen population. A sample of 1,102 African American adolescents aged 13-17 from urban Midwestern high schools were included in this study. The current study hypothesized that: (1) parent-adolescent communication about sex would be positively associated with adolescent sexual health knowledge, (2) there would be a significant negative association between parental monitoring and adolescent sexual risk, (3) both parental monitoring and parent-adolescent communication about sex would be negatively associated with adolescent sexual risk and (4) there would be gender differences in parental monitoring, (5) and parent-adolescent communication about sex. Results indicated no significant association between sexual health knowledge and parent-adolescent communication about sex or parental monitoring. Findings indicated a significant negative association between parental monitoring and adolescent sexual risk, with the association being stronger for boys than girls. There was a significant negative association between sexual health knowledge and adolescent sexual risk. Implications suggest that parental monitoring has greater influence on sexual risk in African American adolescents and thus, is more of a protective factor than parent-adolescent communication about sex."--Abstract from author supplied metadata.

Book excerpt: Risky sexual behavior continues to be a public health concern in the United States and studying its antecedents remains an important research endeavor. Utilizing longitudinal data from a sample (N=279) of African American youth and their families, this study examines the relationship between experiences of economic pressure in early adolescence and risky sexual behavior in late adolescence through the processes indicated in the Family Stress Model. Results showed significant relationships between economic pressure, primary and secondary caregiver depression, caregiver conflict, and hostile, inattentive parenting in early adolescence, consistent with the Family Stress Model. Hostile, inattentive parenting positively predicted risky sexual behavior in late adolescence, showing an indirect relationship between economic pressure and risky sexual behavior. Finally, no gender differences in the relationships outlined in this model were observed. This research has important implications for sexual health and prevention programs aimed at reducing risky sexual behavior among adolescents.

Book excerpt: Urban African American adolescents exhibit high levels of risk behavior, disproportionately high rates of sexually transmitted infections (STIs), and are at risk for acquiring Human Immunodefiency Virus (HIV). To date, interventions that address the high rates of STIs in this population demonstrate only moderate efficacy. As researchers strive to develop more effective interventions, social-ecological factors and developmental and gender issues are being considered. This study aimed to provide insight into the gendered characteristics of heterosexual urban African American adolescents' romantic relationships, and how these characteristics impact condom use. From a developmental standpoint, sexuality and romance are a normal part of adolescence. Thus, STI/HIV prevention targeted at urban African American youth can be improved if it considers romantic relationship dynamics, and how they influence sexual health behaviors including condom use. To this end, the current study examined the interconnection among interpersonal relationship factors including monogamy, commitment, trust, and respect, as well as intrapersonal relationship factors including gender and relationship history. Furthermore, this study examined how these inter- and intrapersonal factors impact youths' condom use behaviors. This research was guided by the existing literature, theoretical frameworks (i.e., the ecological perspective on health, Rusbult and Buunk's commitment theory, and Rotenberg and colleagues' trust framework), and the "directed perspective" (i.e., lens theory). A qualitative methodological approach was employed to collect data from 52 heterosexual, sexually experienced, African American adolescents (20 females; 32 males) ranging from 15-17 years in age at the time of enrollment. These youth resided in low-income neighborhoods of two large cities in the United States. Data from semi-structured interviews were analyzed using a phenomenological approach. Data analysis took place in four primary stages including: (a) development of case summaries; (b) development and refinement of codes and a codebook, and organization of data using matrices; (c) content analysis and reliability checks; and (d) reviews and critiques by, and discussions with, other research team members. Numerous themes and subthemes that provide insight into the multifaceted characteristics of urban African American adolescents' romantic relationships emerged. Findings show that study participants' romantic relationships were indeed characterized by gender-specific beliefs and behaviors, but that some beliefs and behaviors were similar across gender. For instance, males and females alike acknowledged positive feelings towards monogamy, trust, and respect, and they considered these dynamics as closely linked. That is, males and females viewed monogamy as indicative of commitment, and monogamous and committed relationships were those that were trusting and respectful. Youths' relationship experiences, however, contradicted their ideals to a great extent. For instance, despite the finding that males held positive perceptions of monogamy, they often engaged in multiple partnerships. This finding that shows young African American males often engage in multiple partnerships corroborates other research. Furthermore, these data provide insight into the social pressures experienced by urban African American males that encourage them to engage in multiple partnerships and remain uncommitted. According to the current findings, males' multiple partnerships set in motion a breakdown in relationship dynamics (i.e., lack of commitment, mistrust, disrespect), and thus youth experienced less-than ideal romantic relationships. Adolescents' less-than ideal experiences subsequently impacted their current and future romantic relationship beliefs and expectations. For instance, because males were frequently non-monogamous, females did not commonly express commitment to or trust in males. Despite this, only a few females talked about breaking up with a partner whom they mistrusted. Males, on the other hand, considered being monogamous and committed when they had negative experiences with being in multiple, uncommitted sexual partnerships. In addition, relationship dynamics, most noticeably trust (although trust was intricately linked to monogamy, commitment, and respect), impacted condom use. Although adolescents considered condom use important, males and females considered not using condoms when they were with a partner whom they trusted was monogamous and free from STIs. Furthermore, some male respondents talked about the need to balance the protection that condoms provided with the pleasure experienced when not using them. With casual and mistrusted partners, safety tended to take precedence; with more serious and trusted partners, pleasure tended to take precedence. Finally, these data elicited gender similarities and differences related to romantic respect perceptions. Across gender, respect was thought to be an important dynamic in romantic relationships, and one closely related to monogamy, commitment, and trust. Because beliefs about respect were interrelated with beliefs about other relationship dynamics that impacted condom use, respect may also impact condom use. The findings of this study suggest that youth have an understanding about how to develop healthy romantic relationships in adolescence and, in turn, healthy romantic relationships in adulthood. That said, respondents had little to no experience with being in healthy relationships. These study findings can help guide future research, and inform sexual health interventions. For example, investigators should explore further males' belief that monogamy is positive, as this belief frequently contradicted their related behavior (i.e., males often had multiple sexual partners). Finally, future interventions, including dyadic-level interventions, should focus on the dynamics of relationships. In doing so, the efficacy of STI/HIV prevention efforts targeted at African American youth may be enhanced.

Book excerpt: This study investigated the relationship between mother-adolescent sexual communication and adolescents' engagement of sexual behavior among a sample of 2,669 European American female adolescents, ages 13 to 18 years, and their mothers, from the first Wave of the National Longitudinal Study of Adolescent Health. Communication was assessed by the frequency that mothers discussed the negative consequences of intercourse with their daughters, one year prior to engagement of sexual risk. Additional family factors including mothers' knowledge of adolescent dating, family structure, and socioeconomic status were examined on the effectiveness of sexual communication and pregnancy risk. In addition to maternal influence, this study acknowledged the importance of adolescent predictors on sexual risk, including adolescents' dating experience, decision making, and awareness of the negative consequences of sexual intercourse. This study also determined if decision making/awareness of sexual consequences mediated the relationship between communication and pregnancy risk. Finally, the moderating effects of adolescents' age and mother-daughter closeness were examined in the associations between communication and sexual risk, and between decision making/awareness and sexual risk. Sexual communication influenced engagement of sexual risk. However, contrary to the hypothesis, communication around sexual risk contributed to adolescents' increased engagement of intercourse, and was ineffective in increasing condom use. Regarding adolescent predictors, decision making and awareness of sexual consequences decreased the likelihood of sexual risk, and decision-making partially mediated the relationship between communication and intercourse. There was no moderation of age or mother-daughter relationship quality in the above associations, indicating that mother and adolescent predictors had similar effects for younger and older adolescents, and across groups of mothers/adolescents who reported low and high levels of closeness. These findings suggest that discussing the negative consequences of intercourse does not prevent adolescents' engagement in sexual behavior, and adolescents may interpret this style of communication as controlling and dramatic, and rebel against parents' advice by becoming sexually active. Furthermore, adolescents' cognitive skills and ability to recognize consequences of sex played an important role in deterring sexual engagement. Thus, adolescent predictors may be more of a protective factor against engagement in sexual risk above and beyond that of maternal influence.

Book excerpt: Sex on television has nearly doubled since 1998 while sexual risk and responsibly messages have decreased. During this time frame, television watching has become one of the most popular pastimes in the United States, especially among Black/African Americans. Black youth are more likely to report higher levels of satisfaction with television images and view characters as more realistic than the average youth. Here, we examine how viewing higher amounts of television affects certain sexual behaviors of Black female adolescents using data from the 2007 Youth Risk Behavioral Surveillance Survey. Results indicate that the number of television hours viewed on an average school day was significantly related to initiating sexual intercourse before the age of 14, condom use, and contraceptive use to prevent pregnancy for all adolescent females in the sample. Though the number of television hours viewed on an average day was not significantly related to the sexual behaviors of Black adolescent females, this group was more likely to initiate sex before 14, report having 3 or more lifetime sexual partners, and more likely to report using condoms at last intercourse than their female counterparts in any other racial group. Black female adolescents who watched 5 or more hours of television, however, were almost four times more likely to report using any form of contraceptive to prevent pregnancy than their white counterparts. These findings suggest that the amount of television watched on an average school day does not influence the sexual behaviors of Black female adolescents in high school. More research is needed to determine the effect television has on Black female adolescents.

Understanding how sociocultural factors and beliefs contribute to Black women's risky sexual behavior is critical in understanding this health disparity. This study investigated the mediating influence of silencing the self (i.e., putting the needs and wants of a romantic partner before one's own needs) on the relationship between risky sexual behaviors and self-esteem, gender role beliefs, and gender ratio imbalance beliefs and behaviors (GRIBBs) in Black female college students. Participants included 99 female undergraduate students enrolled at a regional Mid-South University. Results demonstrated that risky sexual behavior was not significantly associated wtih the hypothesized variables. Higher scores on silencing the self was significantly associated with lower self-esteem (b = -1.42, p .001) and higher GRIBBS (b = .82, p .001). The results suggest that silencing the self can still adversely impact Black women, despite its non-significant relationship with risky sex. .
