The aim of the study was to evaluate the effect of lifestyle modification on glycemic control of type 2 diabetic patients at Suez Canal University Hospitals at Ismailia city. A quasi-experimental design made up of a control group and a study group with pre- and post-test administration was applied. This study was carried out at the Family Medicine Outpatient Clinic and the Diabetic Outpatient Clinic of Suez Canal University Hospitals at Ismailia city in Egypt. 92 type 2 diabetic patients were included in this study. The Diabetes Knowledge Questionnaire; Health promoting lifestyle profile II Scale; and Physical assessment sheet were used for data collection in the two groups. After implementing of the program, those patients who received lifestyle modification intervention achieved better total score of knowledge & knowledge related practice about DM, health promoting lifestyle domains values and glycated hemoglobin, compared with the control group. Factors related to lower glycated hemoglobin in the present study were lower fasting blood sugar level and increasing physical activity. Overall, lifestyle modification program has a positive influence on blood glucose control of patients with type 2 diabetes mellitus. Therefore, it is recommended to that lifestyle modification interventions should be integral part of the curative management of type 2 diabetic patients, and further study in other places to investigate the effect of lifestyle modification on glycemic control of those patients.

Abstract: Objective: To systematically review the effect of self-management education on diabetic control in type 2 diabetes Research design and methods: PubMed was searched for English-language articles published between 2010 and 2013. All the studies were original articles selected manually and used randomized control trials generating results of self-management education in people with type 2 diabetes referring to diabetic control. Relevant data were divided and tabulated into factors of population characteristics, interventions and outcomes. Interventions were classified into three sections as collaborative information intervention, lifestyle intervention, and skills teaching intervention based on the patterns of education. Outcomes were categorized into glycemic control, cardiovascular disease (CVD) risk factors and nephropathy risk factors. Results: A total of 24 studies were identified of initial 41 articles for this review. Effects of self-management education on glycemic control were demonstrated to be positive both in short-term (10 months) follow-up, but more positive effects in short-term follow-up. The same effectiveness happens to CVD risk factors, including lipids, weight and blood pressure. On the other hand, with short-term follow-up, teaching skills intervention of self-management education is more effective than collaborative information intervention and lifestyle intervention on reducing glycemic control and CVD risk factors. Also with long-term follow-up, teaching skills intervention of self-management education had more effectiveness than collaborative information intervention and lifestyle intervention on reducing glycemic control. However, few studies including CVD risk factors in the long-term follow-up, so it is difficult to evaluate the effectiveness of on CVD risk factors with long-term follow-up. Conclusions: Evidences supports the positive effectiveness of self-management education with collaborative information intervention, lifestyle intervention and skills teaching intervention among type 2 diabetes patients on diabetic control, in both short-term follow-up and long-term follow-up, but short-term follow-up is more effective than long-term follow-up. Further research is needed to develop self-management interventions to maintain long-term follow-up effects on glycemic control, CVD risk factors and other diabetes complications. Diabetes self-management education has been involved in the prevention of diabetes-related complications among type 2 diabetes mellitus (T2DM) patients. Emphasis was on healthy food choices, increase in physical activities, and medication adherence. Purpose: The purpose of the evidence-based practice (EBP) project was to evaluate the effect of education on T2DM inmates 18 years and older on lifestyle changes such as weight loss. Methods: This was an EBP project conducted on 20 participants with T2DM. Project implementation lasted a total of 10 weeks. Interventions were individualized education sessions focusing on various aspects of diabetes control. Pre- and post-test utilized diabetes knowledge test 2 (DKT2) for interventions. Results: The mean percentage score on the pre-test was 63.65% with a SD of 8.35 compared to post-test score of 89.47% and SD of 3.64. There was a 25.82% increase in participants' knowledge post-intervention. The secondary outcome measured weight changes among the participants. Mean of pre-intervention weight was 234.31 pounds, median of 225 pounds, mode of 214 pounds, and a SD of 40.38. Post-intervention weight had a mean of 231.37 pounds, median of 219 pounds, mode of 256 pounds, and SD of 40.24. The difference between pre-and post-intervention weights was 1.25%. Implications for practice: Nurse practitioners play major roles in leading the interprofessional team and in providing health promotion policies and principles to various groups of patients. Successful planning, implementation, and sustaining continuity of change requires multidisciplinary involvement. Type 2 diabetes mellitus (T2DM) affects 29.1 million individuals in the United States. The vital need to address how diet can affect the T2DM patient glycemic goals is imperative. This direct practice improvement (DPI) project was intended to educate T2DM patients on diabetic dietary education and increase diabetes dietary knowledge on making healthy food options. The Diabetes Knowledge Test (DKT) questionnaire was utilized to analyze the existing knowledge level of the T2DM patient before the nutritional value label intervention. Kurt Lewin's force field change theory was used to evaluate the effect of increasing diabetes nutritional knowledge and how nutritious food selections can affect glucose levels. The specified population for this project consisted of 30 participants from a small family practice clinic in Converse, Texas. The data consisted of DKT questionnaire that was utilized both pre-and post-test during the 30-day duration of this project. The primary findings indicated that there was a significant difference in the pre-test and post-test response data on the DKT questionnaire, as evidenced by the pre-test standard deviation of 0.57 and post-test standard deviation of 0.21. The statistical significance meaning of the findings indicate that the diabetic dietary intervention was effective. The continuance of this project while utilizing the intervention of nutritional food label reading has the potential to promote an efficient transformation within this population of patients by choosing healthier options and reducing mortalities.

Medical nutrition therapy and exercise are effective as oral antidiabetics and/or insulin treatment and must be in combination with pharmacological intervention. We aimed to study the effect of lifestyle intervention in obese type 2 diabetics using insulin. Patients above BMI>30 kg/m2, and using more than >60 iu insulin per day were enrolled in the study. Their daily calorie requirments were calculated and medical therapy were arrenged. In collabaration with Physical Rehabilatation Clinic, patients were ordered to do physical exercises in the hospital. Before the therapy; patients were monitorised in terms of ECG and blood pressure. Maximum heart rate was calculated and 60-80 % of it was calibrated during the exercise. Bicyle warm up for 5 minutes, 20 minutes of active exercise, 5 minutes of cooling was done during the exercise for 20 consecutive days. After discharge from hospital, patients were told to do active exercise for at least 45 minutes every other day. After 6 months, weight, BMI, A1c changes and daily insulin doses were compared. There were 21 females (64%) 12 males (36%), mean ages were 60.52u00b17.45 years. After six months, mean weight loss was 8.44u00b11.22 kgs (p=0.002), mean BMI decrease was 1.45u00b10.71 kg/m2 (p=0.49), mean A1c decrease was 2.36u00b11.68 % (p=0.001) and mean daily insulin dose reduction were 24.18u00b113.96 iu (p=0.001). It should be kept in mind that, unnecessary high daily insulin doses could be the reason for poor glycemic control and obesity especially in patients who are obese and have high A1c levels. Lifestyle intervention is the first and maybe the most effective treatment for obesity and insulin dose reduction, so patients must be motivated and should be followed up regularly.

This concept posits that the holobiont (host plus all of its associated microorganisms) and its hologenome (sum of the genetic information of the host and its symbiotic microorganisms), acting in concert, function as a unique biological entity and therefore as a level of selection in evolution. All animals and plants harbor abundant and diverse microbiota, including viruses. Often the amount of symbiotic microorganisms and their combined genetic information far exceed that of their host. The microbiota with its microbiome, together with the host genome, can be transmitted from one generation to the next and thus propagate the unique properties of the holobiont. The microbial symbionts and the host interact in a cooperative way that affects the health of the holobiont within its environment. Beneficial microbiota protects against pathogens, provides essential nutrients, catabolizes complex polysaccharides, renders harmful chemicals inert, and contributes to the performance of the immune system. In humans and animals, the microbiota also plays a role in behavior. The sum of these cooperative interactions characterizes the holobiont as a unique biological entity. Genetic variation in the hologenome can be brought about by changes in either the host genome or the microbial population genomes (microbiome). Evolution by cooperation can occur by amplifying existing microbes, gaining novel microbiota and by acquiring microbial and viral genes. Under environmental stress, the microbiome can change more rapidly and in response to more processes than the host organism alone and thus influences the evolution of the holobiont. Prebiotics, probiotics, synbiotics and phage therapy are discussed as applied aspects of the hologenome concept.
