Book excerpt: Abstract: The purpose of this study was to compare the effects of cold versus a combination of cold and passive stretching on the length of the hamstring muscles. One 22 year old female served as the subject for the study. The straight leg raise test (SLR) was utilized for the collection of data. Measurements of the pelvic angle were recorded with a standard plastic goniometer. The format chosen for this research was the multiple baseline single subject design, A-B-A-BC-A; A phases represented baseline measurements, B phase represented application of cold modality, and BC phase was the application of cold modality combined with passive stretching. Data were analyzed through utilization of visual, semi-statistical, and statistical methods. Results of these tests revealed no significance with the application of cold modality alone, however, the application of a cold modality combined with passive stretching had significant effects on increasing passive hamstring muscle length. The researchers concluded that the application of a cold modality may not be an effective treatment to increase hamstring muscle length independently; however, when combined with passive stretching, may result in significant increases in hamstring muscle length. Flexibility is an integral part of an individual's overall health. Clinicians use various modalities such as heat, cold, stretching, or combinations of these modalities to increase the muscle flexibility of their patients. Clinicians must continue to assess their treatments through research settings as well as in the clinical environment to continue advances in their professions. Further research can only enhance and improve treatments well established and currently practiced.

Book excerpt: The optimal duration of an acute static stretch of the hamstring muscles to improve knee extension ROM has not been determined and not many studies, to my knowledge, have tested the acute effects of a single bout of static stretch for longer durations greater than 60 seconds in older adults while also testing their physical function. The purpose of this study was to investigate the acute effects of a single bout of "low intensity" static stretching of the hamstring muscle for 0, 30, 60, 120 seconds to determine which duration of stretching will produce and maintain the greatest gains in knee ROM. Nineteen participants aged 55-80 years were volunteered for the study. Results of the study showed that a longer duration stretch of 60-120-seconds was more effective in producing acute gains in knee ROM than a shorter duration of 30 seconds within the older adults.

Although the quantitative parameters of training, duration, and frequency are important, it is the qualitative criterion of intensity (“how much”) that the author suggests is ultimately of greater concern. Intensity, the rate and magnitude of force, may be responsible for the proper recovery, regeneration, and adaptation of the musculoskeletal tissues from training, competition, or rehabilitation from injuries. Research suggests that too much force results in the stimulation of an inflammatory response, one associated with a biochemical feedback emerging from a mechanical stimulus. The intent of this manuscript is twofold: to initiate the discussion of the importance of stretching intensity with regard to proper recovery, regeneration, and adaptation, and to suggest that researchers need to explore its potential role in addressing numerous inflammatory (RA) and non-inflammatory (OA, recurrent tendinitis etc.) musculoskeletal conditions as well.
