Book excerpt: Incipient caries lesions or white spot lesions are one of the most common clinical problems resulting from orthodontic treatment with fixed appliances. Literature has shown that infrared photothermal radiometry and modulated luminescence (PTR-LUM), used by The Canary System for caries detection, is capable of monitoring artificially created carious lesions and their evolution during demineralization. Recently, a water-based calcium aluminate glass ionomer luting cement, Ceramir, has shown bioactive surface apatite formation that may influence local remineralization. This in vitro study's objective was to evaluate effects of bioactive cement on enamel demineralization around orthodontic brackets compared to composite resin cement. A sample of 32 caries-free extracted human teeth was collected under an IRB-exempt protocol. Orthodontic brackets were cemented to each tooth with either Transbond XT or Ceramir. A 3x3 mm window adjacent to the bracket was created with acid-resistant varnish f.

Book excerpt: Currently there is no reason, in most cases of cavitated caries lesions, to remove affected tissue. This book presents evidence-based research on the topic and provides assessments of diagnostic devices. It offers new insights into how a dentine carious cavity can be managed by either tissue removal or restoration. Methods for preserving dental tissue are presented and ample evidence highlights the need to seal with a quality restorative material. An update on how to conduct a randomized clinical trial is followed by a chapter on agreed upon terminology for supporting improved communication among oral health professionals around the world. This is a must-read for general practitioners, restorative specialists, dental students, and oral hygienists/therapists.

Book excerpt: This volume brings together current concepts relating to the use of fluoride in dentistry. In contributions written by expert authors, data from this large and complex field have been assembled into a clear sequence and presented in a lucid fashion. The first section deals with the sources of fluoride intake and its metabolism, in order to fully understand fluoride toxicity and the importance of monitoring intake. The second section focuses in more detail on modes of fluoride application and the mechanisms by which this ion interacts with the oral environment to cause a remarkable reduction in dental caries. The role of fluoride in the prevention of dental erosion is also elucidated. The complex mechanisms by which fluoride exerts its effects are described with clarity and the entire text is accompanied by particularly useful illustrations. As a clear up-to-date summary of current thinking in the field, this book will be essential reading for research workers and postgraduate students. Established researchers and teachers in both clinical and basic sciences will find it to be a valuable addition to their libraries, and clinicians will be able to better evaluate the current scientific evidence on the advantages as well as the hazards of fluoride in dentistry.

The aim is to provide practitioners with thorough, up-to-date information that will improve their clinical practice. The opening chapters present a comprehensive overview regarding diagnosis of carious lesions, risk assessment, child behavior and development, and behavioral management. The importance of oral health promotion and prevention in controlling lesion progression and maintaining oral health is reviewed. The impact of various factors on clinician decision making is then explained in detail, examples including the type of dentition (primary versus permanent), the clinical and radiographic aspect of the dentine carious lesion (noncavitated or cavitated), and whether the lesion is associated with a developmental defect. Guidance is provided on selection of nonoperative versus operative interventions, and the restorative materials most frequently used in pediatric dentistry are fully described, highlighting their advantages and disadvantages. Readers will also find an informative series of cases, with explanation of the choices in terms of materials and approach.
