Recognition of such signs offers the possibility of treatment with sufficient lead time to prevent irreversible organ damage and death. Pediatric hospitals currently utilize simple, human evaluated rubrics called early warning scores to detect early signs of patient deterioration. These scores comprise subjective (patient behavior, clinician's impression) and objective (vital signs) components to assess patient health and are computed intermittently by the nursing staff. At Boston Children's Hospital (BCH), early warning scores are evaluated at least every four hours for each patient. Many hospitals monitor inpatients continuously to alert caregivers to changes in physiological status. At BCH, each hospital bed is equipped with a bedside monitor that continuously collects and archives vital sign data, such as heart rate, respiration rate, and arterial oxygen saturation. Continuous access to these physiological variables allows for the definition of a continuously evaluated early warning score on a reduced rubric. This thesis quantitatively assesses the performance of BCH's current Children's Hospital Early Warning Score (CHEWS). We also apply several standard machine learning approaches to investigate the utility of automatically collected bedside monitoring trend data for prediction of patient deterioration. Our results suggest that CHEWS offers at least a 6-hour warning with sensitivity 0.78 and specificity 0.90 but only with a prohibitively large uncertainty (48 hours) surrounding the time of transfer. Performance using only standard bedside trend data is no better than chance; improvement may require exploiting additional intra-beat features of monitored waveforms. The full CHEWS appears to capture significant clinical features that are not present in the monitoring data used in this study.

In early 2004, when Dr. Michael DeVita informed me that he was cons- ering a textbook on the new concept of Medical Emergency Teams (METs), I was surprised. At Presbyterian-University Hospital in Pittsburgh we int- duced this idea some 15 years ago, but did not think it was revolutionary enough to publish. This, even though, our fellows in critical care medicine training were all involved and informed about the importance of “C- dition C (Crisis),” as it was called to distinguish it from “Condition A (Arrest). ”We thought it absurd to intervene only after cardiac arrest had occurred,because most cases showed prior deterioration and cardiac arrest could be prevented with rapid team work to correct precluding problems. The above thoughts were logical in Pittsburgh, where the legendary Dr. Peter Safar had been working since the late 1950s on improving current resuscitation techniques, ?rst ventilation victims of apneic from drowning, treatment of smoke inhalation, and so on. This was followed by external cardiac compression upon demonstration of its ef?ciency in cases of unexpected sudden cardiac arrest. Dr. Safar devoted his entire professional life to improvement of cardiopulmonary resuscitation. He and many others emphasized the importance of getting the CPR team to o- of-hospital victims of cardiac arrest as quickly as possible.

Book excerpt: The inability of the healthcare team to identify and report Early Warning System (EWS) patient events can result in further patient deterioration and delays in providing needed care. The purpose of this DNP Quality Improvement (QI) project was to initiate a QI solution using evidence-based practice (EBP) to address unreported EWS patient events on inpatient medical and surgical units at one healthcare institution. This DNP project instituted education and re-education of staff on EWS and utilized an EWS Communication Tool (ECT) to reduce unreported EWS scores >4 or a score of >2 for a single EWS parameter thereby improving EWS response times and positively influencing patient outcomes. The data collected involved reported and unreported EWS patient events representing pre- and post- QI implementation. Using Social Cognitive Theory (SCT) as a framework and the Model for Improvement, the DNP student provided the medical surgical staff EWS education, re-education, and the ECT. Data collected revealed that some EWS parameters were more frequently missed than others. Analysis also revealed a significant association between travel nurses in the post-QI implementation period and unreported EWS events. Both factors can be considered when planning future education. A positive finding was the increase in reporting for an EWS score of three in the post-implementation period. Continued implementation of the QI project's interventions can help sustain this and other encouraging findings.

Book excerpt: Early deterioration in adult medical-surgical patients is associated with increased intensive care unit and hospital mortality (Goldhill, 2001). Failure to recognize deterioration is a preventable patient safety and quality issue. To address this problem, since 2013, Kaiser Permanente Northern California (KP NCAL) has piloted Advance Alert Monitor (AAM) at two hospitals. This early warning system employs a set of predictive models developed by the KP NCAL Division of Research, which automatically predicts patient deterioration within the next 12 hours based on a complex algorithm of laboratory and clinical data points. Improvements in mortality and length of stay have been realized at the two pilot hospitals. In anticipation of expansion to additional NCAL facilities, major changes to the AAM workflows and processes were developed that increased the sensitivity of the patients identified at risk for clinical deterioration, as well as the timeliness and clarity of clinical response. Expansion to two additional pilot hospitals using these revised processes rely on the evidence-based implementation strategies found in this Doctor of Nursing Practice project. This paper examines the planning, assessment, and implementation of early warning systems at two NCAL facilities using Rogers' diffusion of innovation theory and Greenhalgh's extension of Rogers' theory. Key attributes need to be considered from a cultural and organizational perspective to both start and sustain an implementation. The success of AAM implementation is validated using specific outcome and process measures, including compliance with documentation and timeliness of workflows.

Book excerpt: Problem: Cardiopulmonary arrest in the pediatric population is a rare occurrence but has poor outcomes. In the inpatient setting, there is at least an hour during which the patient has signs and symptoms of clinical deterioration before the arrest occurs but in many instances these signs are ignored or not acted upon to prevent the deterioration. Metabolic derangement and abnormalities in fluid balance place pediatric burn-injured patients at high risk for deterioration. Use of a Pediatric Early Warning System/Score (PEWS) may help prevent or delay this clinical deterioration from happening. Methods: PubMed and Google Scholar were searched for peer-reviewed articles within the past ten years that showed the development, implementation and validation of pediatric early warning system (PEWS) scores. A total of 11 articles were reviewed. Results: The majority of the studies showed that development and implementation of the PEWS tool helps identify pediatric patients at risk for clinical deterioration. The PEWS tool can be used in conjunction with rapid response teams (RRT)/ medical emergency teams (METS) to help prevent or delay the deterioration. Conclusion: Further study is needed to validate the PEWS tool and assess its impact on patient outcome, especially in the pediatric burn-injured population, once implemented in the clinical setting.
