Book excerpt: Peptic Ulcer: A New Approach to its Causation, Prevention, and Arrest, Based on Human Evolution focuses on the argument that the interference with the natural buffering of gastric acid by the food through the removal of protein in the refining of carbohydrates is the primary cause of peptic ulcer. The book first offers information on the law of adaptation and the role of hydrochloric acid in the production of peptic ulcer and the law of adaptation and the role of stress in the production of peptic ulcer. The text then elaborates on protein-stripping in carbohydrate foods as the fundamental cause of peptic ulcer. The publication takes look at racial incidence of peptic ulcer and explanation of the difference in the incidence of gastric ulcer and duodenal ulcer. The publication also underscores the disregard of appetite as an aggravating factor in the production of peptic ulcer, heredity and peptic ulcer, and application of the argument to the prevention and arrest of peptic ulcer. The manuscript is a dependable reference for readers interested in the study of causation, prevention, and arrest of peptic ulcer.

It provides easy-to-follow instructions accompanied by a range of pictures and illustrations, as well as a collection of interactive videos to aid the reader in developing a deeper understanding of each surgical procedure. Techniques covered include minimally invasive surgical treatment for esophageal and gastric cancer including different approaches such as thoracoscopic, transhiatal, laparoscopic, and robot-assisted resections. These chapters include different types of cervical and intrathoracic anastomoses after esophageal resections, and different anastomoses and reconstructions after gastrectomy. Moreover, the Atlas includes an extensive description of minimally invasive procedures in bariatric surgery including sleeve resection, gastric bypass, biliopancreatic diversion, and others. Minimally invasive approaches for other benign pathologies such as benign tumors and treatment of gastroduodenal ulcer complications are also depicted. All chapters, written by a renowned and experienced international group of surgeons and their teams, are focused on practical step-by-step description of the techniques. Atlas of Minimally Invasive Techniques in Upper Gastrointestinal Surgery systematically describes the most frequently performed surgical procedures of the esophagus and stomach and is a valuable resource for all practicing surgeons and trainee general surgeons dedicated to upper gastrointestinal surgery, such as bariatric and surgical oncologists.

All the diseases elegible for emergency laparoscopy are addressed, and for each disease recommendations, levels of evidence, and technical key points are discussed and analyzed. Diagnostic flow charts are included for cases in which laparoscopy turns out to be the final diagnostic step and the first therapeutic one. Furthermore, problematic and positive aspects of the laparoscopic approach from the anesthesiologic point of view are fully explored. Finally, a useful overview of current practice in hospitals across the world is provided, highlighting the varying applications in relation to different medical “cultures”, skills, resources, and healthcare systems.

Available in PDF, EPUB and Kindle. Book excerpt: Over the past decade a great deal of research activity has occurred on either side of the gastroduodenal epithelial interface, with the common goal of elucidating the mechanisms of mucosal protection and how these may be compromised in peptic ulcer disease. A prime stimulus to such research has been the realization that abnormal acid secretion or its delivery into the duodenum in excessive amount cannot fully explain ulcer pathogenesis in the majority of patients. One may envisage the stomach and proximal duodenum as being in a dynamic equilibrium, with aggressive luminal factors, such as acid and pepsin, being counteracted by protective mucosal mechanisms. Until recently, the mechanisms involved in mucosal protection had been poorly defined while the physiology and pathophysiology of acid and pepsin secretion had been elucidated. The impression that gastroduodenal mucosa was protected by a single mechanism has at last been replaced by a more realistic view, suggesting a number of protective zones acting in series. Some of these zones may act as first or second line defences against aggressive factors while certain zones may specifically protect against certain aggressors.
