Book excerpt: The ill effects of a baby's exposure to drugs and alcohol while in the womb are demonstrated in this essential reference with cutting-edge information from the fields of medicine, neuroscience, and child psychology. These findings posit that many of the learning and behavior problems seen in children--from poor school performance to patterns of impulsivity often diagnosed as ADHD--are both treatable and preventable. The book first traces the history of fetal malformation back to the time of Aristotle, then presents a resounding call for integrated systems of care for high-risk children and their families. Methods for applying behavior management and treatment techniques are included for health care practitioners, social workers, early childhood intervention specialists, special education teachers, and parents, whether for use at home, at school, or in the clinical setting.

Book excerpt: The Interfaces of Perinatal Addiction Ira J ChasnofT In the last few years, problems associated with drug use in pregnancy have become endemic. As cocaine has become the drug of choice for millions of Americans, including pregnant women, as AIDS has become more commonly recognized in women and infants, and as legal cases have begun to raise the question of fetal abuse, no professional group has come forward to serve as advocate for this special population of substance abusers. Meanwhile, however, physicians, nurses, social service agencies and public health officials have all been faced with increasing numbers of infants showing the detrimental effects of their mothers' drug use. Although problems of substance abuse in pregnancy have received increasing attention in the medical literature since the early 1970s, there has recently been a very rapid increase in the number of articles published related to this field. The reasons for this new interest are easily understood when current statistics from the National Institute on Drug Abuse are reviewed 1. Although patterns of abuse of alcohol, marijuana, heroin and other substances by women of childbearing age have changed very little over the last ten years, the incidence of cocaine use in this special population has been rising rapidly, a reflection of cocaine's increasing popularity among the general population of the United States.

Available in PDF, EPUB and Kindle. Book excerpt: Clinics in Developmental Medicine No. 188 This book documents the consequences of the exposure of infants to the influence of intrauterine chemicals. In setting out the evidence for these outcomes, the authors demonstrate that decisions about care and management can and should be made as early as possible. This should allow professionals to provide protective management and prevent the delays that are so often seen in this area of medical and social care. The international team of contributors sets out to inform the reader of the potential risks to infants exposed to a range of intrauterine chemicals that are potentially neuroactive, including medicinal drugs such as antiepileptics, antidepressants and antipsychotics, as well as drugs of abuse, including alcohol, opiates, and recreational drugs such as cannabis and tobacco. They review the teratogenic action of some of the chemical processes and the relationship of exposure to the stage of pregnancy. Some agents alter anatomic structure; others alter the chemical balance of neurotransmitters and may thus alter the regulation of brain function, with profound effects on the child's behaviour and propensity to behavioural disturbances. The book explores strategies to support these children and those who care for them, including statutory agencies. Readership Paediatricians, neonatologists, obstetricians, gynaecologists, child and adolescent psychiatrists and psychologists, those working in child care and child protection.

Book excerpt: Significant changes have taken place in the policy landscape surrounding cannabis legalization, production, and use. During the past 20 years, 25 states and the District of Columbia have legalized cannabis and/or cannabidiol (a component of cannabis) for medical conditions or retail sales at the state level and 4 states have legalized both the medical and recreational use of cannabis. These landmark changes in policy have impacted cannabis use patterns and perceived levels of risk. However, despite this changing landscape, evidence regarding the short- and long-term health effects of cannabis use remains elusive. While a myriad of studies have examined cannabis use in all its various forms, often these research conclusions are not appropriately synthesized, translated for, or communicated to policy makers, health care providers, state health officials, or other stakeholders who have been charged with influencing and enacting policies, procedures, and laws related to cannabis use. Unlike other controlled substances such as alcohol or tobacco, no accepted standards for safe use or appropriate dose are available to help guide individuals as they make choices regarding the issues of if, when, where, and how to use cannabis safely and, in regard to therapeutic uses, effectively. Shifting public sentiment, conflicting and impeded scientific research, and legislative battles have fueled the debate about what, if any, harms or benefits can be attributed to the use of cannabis or its derivatives, and this lack of aggregated knowledge has broad public health implications. The Health Effects of Cannabis and Cannabinoids provides a comprehensive review of scientific evidence related to the health effects and potential therapeutic benefits of cannabis. This report provides a research agendaâ€"outlining gaps in current knowledge and opportunities for providing additional insight into these issuesâ€"that summarizes and prioritizes pressing research needs.

It seems to divide everyone from psychologists and politicians to scientists and salesmen, leaving the parents themselves with a terrible headache as a result. How can anyone live up to such expansive and conflicting expectations? As Zoe Williams explores, the madness begins before the baby has even arrived: hysteria is rife surrounding everything from drinking alcohol and eating cheese to using a new frying pan. And it only gets worse. The list of things you need to consider (as well as the things you never realised you needed to consider) is ever-increasing, and questions of breastfeeding, buggies, staying at home, schooling - and what your mother-in-law thinks you're doing wrong - take over completely. The task of raising a child has been turned into a circus of ludicrous proportions. Combining laugh-out-loud tales of parenthood with myth-busting facts and figures, Zoe provides the antithesis of all parenting discussions to date. After all, parents managed perfectly well for centuries before this modern madness, so why do today's mothers and fathers make such an almighty fuss about everything?

Book excerpt: Concurrent Treatment of PTSD and Substance Use Disorders Using Prolonged Exposure (COPE) is a cognitive-behavioral psychotherapy program designed for patients who have posttraumatic stress disorder (PTSD) and a co-occurring alcohol or drug use disorder. COPE represents an integration of two evidence-based treatments: Prolonged Exposure (PE) therapy for PTSD and Relapse Prevention for substance use disorders. COPE is an integrated treatment, meaning that both the PTSD and substance use disorder are addressed concurrently in therapy by the same clinician, and patients can experience substantial reductions in both PTSD symptoms and substance use severity. Patients use the COPE Patient Workbook while their clinician uses the Therapist Guide to deliver treatment. The program is comprised of 12 individual, 60 to 90 minute therapy sessions. The program includes several components: information about how PTSD symptoms and substance use interact with one another; information about the most common reactions to trauma; techniques to help the patient manage cravings and thoughts about using alcohol or drugs; coping skills to help the patient prevent relapse to substances; a breathing retraining relaxation exercise; and in vivo (real life) and imaginal exposures to target the patient's PTSD symptoms.
