Book excerpt: Although there are many books on angina, few are devoted to the important problem of how to evaluate drug efficacy in angina pectoris. Drug Evaluation in Angina Pectoris has two fundamental aims: the first is to review the methodological aspects of drug evaluation in stable and unstable angina pectoris; the second is to provide a logical and methodological background for future studies aimed at assessing the ability of medical treatment to improve prognosis. Part I updates the pathophysiology, clinical presentation and prognosis of angina pectoris. Part II aims to `put in numbers' the symptoms and signs of myocardial ischemia, starting from a revision of the currently used parameters. It is extremely important to quantify the variability of the disease for the correct design of clinical trial, a subject dealt with in Part III, which also discusses some clinico-pharmacological aspects of therapy. Part IV is a critical review of the drugs currently used for the treatment of stable and unstable angina, and particularly considers some important unresolved issues concerning their use. The current FDA and EC guidelines for the evaluation of antianginal drugs are briefly discussed in Part V, giving clinical investigators insight into how antianginal drugs are evaluated by regulatory agencies and what is considered as proof of a valid efficacy/tolerability ratio. Drug Evaluation in Angina Pectoris will assist discerning cardiologists, pharmacologists, and advanced students of cardiology as well as innovative pharmaceutical companies, all of whom need to understand what angina is, how to evaluate treatment, and how to judge the agents used in its treatment.

Available in PDF, EPUB and Kindle. Book excerpt: In the past two decades a number of studies have shown that abnormalities in the function and structure of coronary microcirculation can be detected in several cardiovascular diseases. On the basis of the clinical setting in which it occurs, coronary microvascular dysfunction (CMD) can be classified into four types: CMD in the absence of any other cardiac disease; CMD in myocardial diseases; CMD in obstructive epicardial coronary artery disease; and iatrogenic CMD. In some instances CMD represents an epiphenomenon, whereas in others it represents an important marker of risk or may contribute to the pathogenesis of myocardial ischemia, thus becoming a possible therapeutic target. This book provides an update on coronary physiology and a systematic assessment of microvascular abnormalities in cardiovascular diseases, in the hope that it will assist clinicians in prevention, detection and management of CMD in their everyday activity.

This book is divided into two sections including the introduction and the approach to chest pain. Our introductory chapter starts with the basic principles of statistics and its application in various diagnostic modalities of heart disease. Our authors present a nice approach to patients presenting with chest pain in various scenarios. We have also included a chapter describing GERD, which could present as chest pain and another chapter describing aortic dissection, which is a life-threatening disease presenting with chest pain. We hope that this book will serve as an accessible handbook on the differential diagnosis of chest pain.

Available in PDF, EPUB and Kindle. Book excerpt: The· first meeting of the cardiovascular drug therapy forum took place on April 13-15, 1988 in Montreux, Switzerland. In this meeting, which was closed to the public, approximately 150 leading investigators in the field of cardiovascular drug therapy, both academics and from the pharmaceutical industry, discussed the state of the art and future trends of this rapidly expanding field. The discussions concentrated on four major topics: (1) ischemic heart disease; (2) heart failure; (3) hyper tension; (4) arrhythmias. These topics were discussed by four committees in parallel, and thereafter the main points were reviewed by the whole forum. This framework allowed both intensive and interdisciplinary discussions. This book, the first publication of selected topics from the meeting, focuses mainly on nitrate therapy in ischemic heart disease and heart failure. Book excerpt: When I was asked some years ago by the editors of the Handbook of Experimental Pharmacology to edit a new volume on Antianginal Drugs, I agreed on the condition that, in accordance with my scientific background, primary emphasis be given to clinical pharmacology and therapeutics. It soon turned out that, due to rapid developments in this field, nothing of the previous volume on Antianginal Drugs by Charlier (Vol. 31, 1971) could be retained apart from its basic idea of devoting considerable space to methodology. Since editors must operate within certain limits, I had to abstain from dealing with acute myocardial infarction in detail despite the well-known overlap between unstable angina, the preinfarction syndrome, and acute myocardial infarction. It was only possible for acute myocardial infarction and the concept of reduction of infarct size to be briefly discussed within the chapter on pathophysiology of acute coronary insufficiency. The chapter on invasive methods provided an opportunity to touch on new approaches to early intervention in acute myocardial infarction. Here, intracoronary streptokinase therapy and PTCA are considered, again with attention to the overlap between mechanical and pharmacological interventions.

Book excerpt: This book reviews the latest findings on epicardial coronary spasm and coronary microvascular dysfunction (CMD), important diseases for understanding coronary artery vasomotion abnormalities. The book chiefly consists of two major parts, each of which explores the epidemiology, pathophysiology, diagnosis and treatment of the diseases. The chapters present a range of experimental and clinical studies, including the editor’s life’s work on coronary spasm; how Rho-kinase activation plays an important role in connection with the disease; how chronic adventitial inflammation is a central pathophysiology of the spasm; the importance of Rho-kinase activation in the pathogenesis of CMD; how epicardial spasm and CMD frequently co-exist; and much more. Coronary Vasomotion Abnormalities will help practicing cardiologists, cardiac surgeons, vascular biologists and radiologists understand the latest issues concerning the clinical, interventional, and surgical management of these conditions, and offer these readers valuable insights into the effective treatment of microvascular angina.
