The COA's say, "number of flights (per location, per aircraft)" & "total aircraft operational hours[.]" The columns only support one aircraft; however, recreational flyers could list multiple aircraft because they are currently not required to report. Disclaimer: Current FAA "blanket" COAs say "must submit the following information" and do not explicitly say scans are acceptable.
