La recherche de facteurs prédictifs préopératoires peut permettre d’identifier les patients vulnérables à une DPO aigue intense. Récemment, Pan et al ont proposé un test simplifié de 3 échelles (cf infra) permettant une prédiction satisfaisante de l’intensité douloureuse après césarienne. L’objectif de notre étude était d’évaluer un ensemble de facteurs dont ces 3 items dans une population variée d’opérés. Matériel et Méthodes : Nous avons mené une étude de cohorte prospective monocentrique de 13 semaines. Après accord du CPP, les patients majeurs opérés en chirurgie programmée au CHU de Reims étaient éligibles. Les principaux critères de non-inclusion étaient la neurochirurgie et les troubles cognitifs. Les questionnaires ont été réalisés la veille de l’intervention par des étudiants infirmiers anesthésiques formés. La prise en charge anesthésique et antalgique a été laissée à l’appréciation de l’équipe médicale selon les protocoles habituels et en aveugle du questionnaire. Les facteurs prédictifs évalués étaient les 3 échelles citées en introduction (EVA sur 100mm du niveau d’anxiété liée à la chirurgie, du niveau de douleur pressentie en post opératoire et enfin de l’anticipation du besoin en antalgiques par rapport à la moyenne), une EVA de l’anxiété générale et une évaluation de la douleur chronique, du terrain et les données peropératoires. Le critère de jugement principal était la douleur maximale à 24h post-opératoire (DPO 24h) mesurée par une échelle numérique au repos ou à la mobilisation par un investigateur en aveugle des données pré et per-opératoires. Un recueil de l’intensité de la douleur à 3 mois postopératoire par appel téléphonique permettait une évaluation de la douleur chronique post chirurgicale (DCPC). L’analyse statistique a consisté en l’analyse multivariée des facteurs en rapport avec la DPO 24h d’une part, la DPO 24h≥5/10 et ≥7/10 par la suite et enfin de l’intensité de la DCPC. Ces analyses ont été ajustées au caractère douloureux attendu de la chirurgie d’après le référentiel des chirurgies la SFAR. Résultats : 1371 patients âgés de 57±16 ans dont 51% de femmes ont été inclus dans les secteurs orthopédique 35%, digestif 20%, urologique 11%, vasculaire 9%, cardiothoracique 9%, gynécologique 8%, ORL 5%. L’anxiété générale était de31±28mm et l’anxiété liée à la chirurgie de 35±30mm. L’anticipation du niveau de douleur était de 43±27mm et la consommation anticipée d’antalgique était égale à la moyenne dans 55% des cas, 19% des patients pensant avoir une consommation plus élevée que la moyenne. La DPO 24h maximale était de 4,8±2.9 et ≥5/10 dans 57% des cas. L’analyse multivariée de la DPO retrouvait 9 variables indépendantes (i) l’absence de diabète, (ii) une prise d’antalgiques au long cours, (iii) un sommeil non réparateur, (iv) l’existence de douleur chronique quotidienne, (v) une EVA de la douleur attendue lié à la chirurgie plus importante, (vi) une consommation d’antalgiques attendue supérieure à la moyenne, (vii) une chirurgie à douleur attendue importante, (viii) l’absence de dexaméthasone, (ix) une durée d’intervention chirurgicale prolongée. Le R2 de ce modèle est de 0,14. Discussion : Notre étude confirme, sur une large population, les résultats d’une étude récente menée en obstétrique sur l’intérêt d’utiliser des mesures simplifiées des paramètres d’anxiété et de douleur attendue en préopératoire. Des éléments du terrain tels que les douleurs chroniques, le diabète ou les troubles du sommeil sont des données simples qu’il serait intéressant d’associer à l’évaluation préopératoire du risque de DPO intense.
