Book excerpt: To ensure that countries can effectively benefit from digital health investments, “digital adaptation kits” (DAKs) are designed to facilitate the accurate reflection of WHO’s clinical, public health and data use guidelines in the digital systems that countries are adopting. DAKs are operational, software-neutral, standardized documentations that distil clinical, public health and data use guidance into a format that can be transparently incorporated into digital systems. For this particular DAK, the operational requirements are based on systems that provide the functionalities of digital tracking and decision support (DTDS) and include components such as personas, workflows, core data elements, decision-support algorithms, scheduling logic and reporting indicators. Web annexes provide certain components in additional detail including: data dictionary (Web Annex A), decision-support logic (Web Annex B), indicator definitions (Web Annex C), and functional and non-functional requirements (Web Annex D). Data elements within the DAK (Web Annex A) are mapped to standards-based terminology, such as the International Classification of Diseases (ICD), to facilitate interoperability. This DAK focuses on providing the content requirements for a DTDS system for HIV care used by health workers in primary health care settings. It also includes cross- cutting elements focused on the client, such as self-care interventions.

Book excerpt: WHO's pre-exposure prophylaxis (PrEP) for HIV infection implementation tool contains modules for a range of stakeholders to support them in the consideration, planning, introduction, implementation and roll-out of PrEP services. The modules can be used on their own or in combination. In addition, there is a module for individuals interested in or already taking oral PrEP. The STI integration module compiles existing WHO recommendations, systematic scientific literature reviews and other relevant publications to provide suggested implementation approaches. It is the product of collaboration among many experts, community organizations and networks, providers, implementers, researchers and partners from all regions. The STI integration module is divided into 2 parts: the first focuses on programme managers and other decision-makers, and the second on health care workers. People who use PrEP will also benefit from it.

Book excerpt: Globally, women of reproductive age face two overlapping issues that have a significant impact on their health and well-being: unintended pregnancy and sexually transmitted infections (STIs), including HIV. A growing body of research indicates that the majority of women across geographies, ages, racial and ethnic backgrounds would prefer a multipurpose prevention technology (MPT) that combines protection against pregnancy and HIV/STIs versus individual products for contraception and disease prevention. Currently, male and female condoms are the only available MPTs. A wider range of MPT options will help women select methods that they are less apt to discontinue, as well as increase uptake by first-time users. A number of MPT candidates – intravaginal rings (IVRs), oral tablets, vaginal fast dissolving inserts (FDIs), implants, injectables and microarray patches (MAPs) are in various stages of development. The goal of this Research Topic is to invigorate continued development on MPTs by providing the most up-to-date research on specific products in development, identifying research gaps related to the field overall, and prepare for introduction into programs and healthcare systems. Our broad approach will cover formulation science, clinical trials, sociobehavioral research, and implementation science to present the latest thinking in all areas of MPT development from “bench to bedside.”

Book excerpt: The HIV/sexually transmitted infection (STI) syndemics and the unmet need for modern contraceptive methods continue to pose significant health risks for women and other people worldwide. As awareness of the need to address these interlinked risks has increased, the need for new technologies that combine protection against unintended pregnancy, HIV and other STIs is a growing research priority. Multipurpose Prevention Technologies (MPTs) are products that simultaneously prevent HIV, other STIs, and/or unintended pregnancy. They have power to revolutionize women's health by providing prevention for multiple indications.

Book excerpt: The Consolidated guidelines on HIV, viral hepatitis and STI prevention, diagnosis, treatment and care for key populations outline a public health response for five key populations (men who have sex with men, trans and gender diverse people, sex workers, people who inject drugs and people in prisons and other closed settings). They present and discuss new recommendations and consolidate a range of recommendations and guidance from current WHO guidelines. Particularly for key populations, social, legal, structural and other contextual factors both increase vulnerability to HIV, viral hepatitis and STIs and obstruct access to health and other essential services. These guidelines highlight the critical importance of addressing structural barriers in all settings as a priority. In most countries, inadequate coverage and poor quality of services for key populations continue to undermine responses to HIV, viral hepatitis and STIs. All countries should prioritise reaching key populations and supporting key population communities to lead the response and provide equitable, accessible and acceptable services. In most countries, inadequate coverage and poor quality of services for key populations continue to undermine responses to HIV, viral hepatitis and STIs. All countries should prioritise reaching these key populations and supporting key population communities to lead the response and provide equitable, accessible and acceptable services to these groups.

These include expanded and simplified treatment criteria for adults but now also for adolescents; expanded eligibility for antiviral prophylaxis for pregnant women to prevent mother-to-child transmission of HBV; improving HBV diagnostics through use of point-of-care HBV DNA viral load and reflex approaches to HBV DNA testing; who to test and how to test for HDV infection; and approaches to promote delivery of high-quality HBV services, including strategies to promote adherence to long-term antiviral therapy and retention in care. The 2024 guidelines include 11 updated chapters with new recommendations and also update existing chapters without new recommendations, such as those on treatment monitoring and surveillance for liver cancer.

Book excerpt: Digital technologies have rapidly changed how we bank, borrow and lend, commute, or order food. The scale of these changes, and the relatively low barriers for individuals to drive such systemic change, have raised great expectations for digital technologies to also impact health and healthcare globally. The COVID-19 pandemic has further exacerbated the need for improved health data from low- and middle-income countries (LMICs), and the expectation for digital technologies to provide solutions.

This book was released on 2010-12-21 with total page 163 pages. Available in PDF, EPUB and Kindle. Book excerpt: Randomized clinical trials are the primary tool for evaluating new medical interventions. Randomization provides for a fair comparison between treatment and control groups, balancing out, on average, distributions of known and unknown factors among the participants. Unfortunately, these studies often lack a substantial percentage of data. This missing data reduces the benefit provided by the randomization and introduces potential biases in the comparison of the treatment groups. Missing data can arise for a variety of reasons, including the inability or unwillingness of participants to meet appointments for evaluation. And in some studies, some or all of data collection ceases when participants discontinue study treatment. Existing guidelines for the design and conduct of clinical trials, and the analysis of the resulting data, provide only limited advice on how to handle missing data. Thus, approaches to the analysis of data with an appreciable amount of missing values tend to be ad hoc and variable. The Prevention and Treatment of Missing Data in Clinical Trials concludes that a more principled approach to design and analysis in the presence of missing data is both needed and possible. Such an approach needs to focus on two critical elements: (1) careful design and conduct to limit the amount and impact of missing data and (2) analysis that makes full use of information on all randomized participants and is based on careful attention to the assumptions about the nature of the missing data underlying estimates of treatment effects. In addition to the highest priority recommendations, the book offers more detailed recommendations on the conduct of clinical trials and techniques for analysis of trial data.
