Using a qualitative approach, in-depth semi-structured interviews were conducted with eight urban Aboriginal people with diabetes. The data generated were analyzed using content analysis. The findings indicate that urban Aboriginal people have difficulty accessing and navigating health care services as result of social, cultural, and economic circumstances. Further, lack of resources and information appropriate to participants' needs created difficulties in their making lifestyle changes for diabetes management. Emphasized was the need for greater diabetes awareness and availability of suitable resources for the Aboriginal community. In the discussion, the new paradigm of chronic disease management, with emphasis on self-management, is explored as an approach to change the health care system and make improvements to diabetes care services that positively contribute to Aboriginal people's health. Based on the results, implications for programs and services, health care providers, and further research are presented.

Book excerpt: Indigenous Peoples and Diabetes is a bold attempt to reframe the meaning of diabetes mellitus as a socio-political disorder from the perspective of Indigenous Peoples, community workers, medical anthropologists, and health professionals working and/or living in North America, Latin America, the Arctic, Australia, and the Indian Ocean. The anthology discusses the effects of social history on the etiology and epidemiology of type 2 diabetes within Indigenous experiences of cultural expansionism and colonial occupation. Indigenous narratives about the right to food, health, emotional experience, and the importance of networks of solidarity provide reflective critiques on community wellness, empowering individuals to regain control of their health, spiritual knowledge, and emotional liberty. The book is a paradigm-breaking endeavor because it challenges the widespread assumption that Indigenous Peoples all over the planet are inherently susceptible to sicken and die from degenerative ailments such as diabetes because of their faulty genotype, poor dietary habits, and sedentary lifestyle. Instead, the creative assemblage of chapters shifts the medical gaze from a potentially diseased body to a diseased colonial and post-colonial history of genocide practiced against Indigenous Peoples to this day. Innovative programs to combat the diabetes epidemic and promote physical and emotional wellness are discussed in detail, such as the Mino-Miijim 'Good Food for the Future' program on the White Earth Reservation in Minnesota; the Kahnawake School Diabetes Prevention Project developed in the Kanien'keha':ka (Mohawk) community of Kahnawake, near Montreal, Canada; and the Cultural Rebuilding Project at the Potawot Health Village in northern California. Book excerpt: This thesis explores the relationship between health access risk factors and diabetes in an urban First Nations population living in Canada. The proportion of Aboriginal peoples in Canada living in urban areas is increasing. Despite this, health data on urban Aboriginal populations in Canada is sparse and often unreliable. The Our Health Counts (OHC) study collected data from a self-identified urban First Nations population living in Hamilton, ON through respondent driven sampling. As statistical techniques for this data are not yet fully developed, advanced logistic regression modeling strategies were used to assess the relationship between health access risk factors and diabetes. Feeling health services were not culturally appropriate was significantly associated with an increased odds for diabetes (12.07, 95% 2.52, 57.91). A strong potential effect between diabetes was also found for the following barriers: not being able to locate a doctor in the area; feeling health care services accessed were inadequate and if health services were not being offered in the area. Furthermore, this study examined the sensitivity of results for various analysis strategies for data that were obtained using respondent driven sampling (RDS). This study could allow for the implementation of more targeted health interventions and the design of health policies that are more specific to target the urban First Nations population in Canada, as well as provide insight into the use of appropriate statistical methods for data collected through respondent driven sampling.

Investigation led the doctors to learn that something similar had become a medical issue among young people of the Pima Indian Nation in Arizona though, to their knowledge, nobody else. But these youth were just the tip of the iceberg. Over the next few decades more children would confront what was turning into not only a medical but also a social and community challenge. Diagnosing the Legacy is the story of communities, researchers, and doctors who faced—and continue to face—something never seen before: type 2 diabetes in younger and younger people. Through dozens of interviews, Krotz shows the impact of the disease on the lives of individuals and families as well as the challenges caregivers faced diagnosing and then responding to the complex and perplexing disease, especially in communities far removed from the medical personnel a facilities available in the city.

The goal of the research was to first, document the voices of Urban Aboriginal people with Type 2 diabetes and secondly, to bring together these voices into an updated oral format using the medium of radio. Four principles that grounded this research were Indigenous research paradigm, drawing from Indigenous scholars, critiquing the Euro-Western biomedical worldview of health, and reciprocity. Using an Indigenous research paradigm meant situating who I am as a Nahayowak (Cree) woman who used prayer, medicines and talking with Elders to carry out the research. Second, I ensured the bulk of the reference sources were Indigenous writers. Third, the research was undertaken knowing that health is situated in a colonial Euro-Western biomedical worldview and if the health of Indigenous people is to improve I must assert Indigenous ways of doing research; and finally, any work I do must have a component of reciprocity where knowledge and pragmatic tools, podcast of radio documentary, must be given back to not only the academy but to students and Indigenous community members. Reciprocity must be seen as the 'gold standard' in any work that Indigenous scholars do when working to change the situation of Indigenous peoples. The theoretical underpinnings of this research are four aspects of self, visually represented in the Nahayowak Medicine Model by Ghostkeeper as the mind, body, emotions and spirit. I was motivated to do this research when I found out 50% of my relatives had diabetes and when the Canadian Diabetes Association in 2002, and again in 2015, stated there is still a stigma associated with diabetes that prevents Aboriginal people from disclosing their diabetes. For this study there were seven Indigenous people who agreed to participate because they also wanted to create a space for other Aboriginal people to be able to talk about diabetes without shame or guilt. They hope their story will allow others to share their diabetes story. The interviews became a radio documentary that ensured views were broadcasted about Indigenous people living with diabetes from an Indigenous perspective that was directed toward other Indigenous people.

Book excerpt: While previous health research with Aboriginal populations focused almost exclusively on Aboriginal Peoples of First Nations descent living on reserves or in isolated rural communities in Canada, this study focusing on diabetes aimed to engage Aboriginal Peoples of First Nations and Métis descent living in an urban Ontario setting. Type 2 diabetes mellitus is a progressive metabolic disorder that affects Aboriginal Peoples of Métis and First Nations descent disproportionately compared to the rest of the Canadian population. To understand this disparity in diabetes incidence and to address issues with existing diabetes prevention and management strategies, this study: a) explores the perceptions surrounding Type 2 diabetes and its prevention from First Nations and Métis community people and health service providers and policy makers; and b) informs the existing diabetes prevention, management and care strategies in light of these perceived understandings. Primary data was collected through 40 in-depth one-on-one narrative interviews with First Nations and Métis people, health service providers and policy makers. Thematic codes that emerged through the narrative analysis of this data revealed that to fully understand the social determinants of diabetes in an urban First Nations and Métis people's context required the application of intersectionality theory, since production of First Nations and Métis diabetes is socially determined and deeply intersectional. By combining the concepts of the social determinants of health and intersectional approaches, narrative analysis of the primary data revealed that diversities in socio-economic, cultural, legal and spatial contexts determine First Nations and Métis people's life choices and have a strong bearing on their health outcomes. First Nations and Métis participants' narratives revealed that dimensions of marginalization were reflected not only through inadequate material resources, but also through intersections of multiple factors such as colonial legacies, stereotyping, legal statuses, and the pan-Aboriginal nature of government policies and services. First Nations and Métis community members indicated that preventive programming aimed at avoiding or managing diabetes should be grounded in balancing and restoring the positive aspects of physical, mental, spiritual and emotional health and should also balance their diverse needs, lived realities, and social circumstances. The views of health service providers and policy makers captured in this thesis tended to reflect an understanding of diabetes causation grounded in both biomedical and intersecting social determinants of health. At the pragmatic level, however, the solution to this health issue presented by health service providers and policy makers addresses only the measurable individualistic biomedical risk factors of diabetes. Policy makers also discussed the need for developing qualitative indicators of the success of presently implemented health programs. Overall, the results of this study indicated that effective diabetes prevention and management strategies for urban First Nations and Métis people must recognize and address the diversities in their historical, socio-economic, spatial and legal contexts as well as their related entitlement to health services. A comprehensive diabetes prevention strategy should target the social determinants of health that are specific to urban First Nations and Métis people and must build on community strengths.
