Book excerpt: Introduction : le cancer une pathologie du sujet âgé. 23% des nouveaux cas de cancer du sein sont diagnostiqués après 74 ans en France. Au stade localisé, la chirurgie est le traitement de référence associée ou non à la chimiothérapie, la radiothérapie et l'hormonothérapie. Les patientes âgées ont souvent des comorbidités, un état général altéré, des troubles cognitifs et un risque accru de toxicités liées au traitement pouvant être responsables d'un sous traitement, d'une déterioration de leur qualité de vie et d'une difficulté dans le choix des options thérapeutiques. Par ailleurs les données de la littérature sont peu nombreuses, les patientes âgées de plus de 65 ans étant jusqu' à il y a peu exclues des essais thérapeutiques. L'objectif de ce travail était d'évaluer le devenir des patientes atteintes de cancer du sein localisé hormonodépendant, la tolérance des traitements notamment de l'hormonothérapie, la notion de sous traitement et nos pratiques. Méthodes : les données de patientes de 75 ans et plus prises en charge au Centre Jean Perrin entre 2011 et 2016 pour un premier cancer localisé, hormonosensible, HER2 négatif ont été recueillies de manière rétrospective. Résultats : cent vingt-quatre patientes répondant aux critères d' inclusion ont été traitées dans notre centre dont 17 ont reçu une chimiothérapie. Une consultation oncogériatrique a été réalisée dans 35 (28,2 %) cas. Trente et une patientes (25 %) ont reçu une hormonothérapie exclusive et 9 (7,2 %) une hormonothérapie néoadjuvante. Quatre-vingt-une (65,3 %) patientes ont reçu une radiothérapie au mins mammaire. Les patientes traitées par chimiothérapie étaient significativement moins âgées (p=0,011), avaient moins d' antécédents cardiovasculaires (p=0,012) et plus de tumeurs de grade III (p=0,002). Les taux d'institutionnalisation, curage ganglionnaire et rechute étaient plus importants dans le groupe chimiothérapie. Il n' y avait pas de différence significative sur la tolérance de l'hormonothérapie (p=0,588). Vingt-deux (17,4 %) patientes n'ont pas reçu le traitement préconisé en RCP parmi lesquelles 17 (77 %) ont refusé le traitement préconisé. Trente-deux (25,8 %) patientes sont décédées dont 5 (15,6 %) d'une évolution de leur cancer du sein. La survie sans progression médiane et la survie globale médiane ne sont pas atteintes après un suivi médian de 55,6 mois. Conclusion : la prise encharge était plus agressive chez les patientes moins âgées ayant moins de comorbidités et qui présentaient plus de facteurs d' agressivité tumorale. Le taux de survie spécifique est sensiblement similaire aux données décrites dans la littérature. La tolérance au traitement semble acceptable sans franche détérioration de la qualité de vie. Les patientes sont impliquées dans le choix thérapeutiques avec un refus de traitement qui semble surtout en lien avec une crainte des effets secondaires. L'accès aux essais cliniques prospectifs, la réalisation d'une évolution gériatrique et un traitement personnalisé semblent nécessaires afin d'optimiser la prise en charge de cette population.
