Following a comprehensive review of the literature, David Chalmers and his co-authors detail a longitudinal, multidisciplinary study of over 1000 New Zealand children enrolled at three years and followed to age eleven. The changing prevalence of OME in the sample is described; evidence on a wide range of risk factors is examined; the problems of chronic perforation of the tympanic membrane, cholesteatoma, and ontological complications of surgical therapy are evaluated; and the developmental consequences of OME are traced over time. Early onset of bilateral OME is shown to have long-lasting consequences for language development, speech articulation, reading ability, and classroom behavior. The authors conclude from these results that there is an urgent need for the detection and management of early onset OME.

Book excerpt: OBJECTIVESWhether OME affects a child's educational achievement, behaviour, or speech and language development is unclear(1). Several researchers have addressed this question(1), and we aim to undertake a systematic review to analyse the findings of published studies. METHODSEmbase and MEDLINE search, including studies involving otherwise-healthy and typically-developing children. Data on baseline characteristics, confounding variables and outcomes were extracted. RESULTSAll together 84 papers fulfilled the inclusion criteria. Initial review indicates a wide range of reported outcomes and assessment methods. The studies also vary in the severity of OME included and whether or not children are affected bilaterally. CONCLUSIONSLarge methodological variation between studies makes interpretation of their findings difficult. In addition, the inclusion of children that are affected to different degrees makes any assessment difficult, for a child with short-term, unilateral OME would not be expected to be affected in the same way as one with significant, bilateral longer-term hearing loss.
