Also, history and research on sexual abuse and shame including etiology, symptomatology, and treatment considerations are discussed. the following research questions are tested: Do adult female survivors of childhood sexual abuse report significantly more shame than the non-clinical population? Since shame is suspected to be the core issue of much clinical pathology, do sexual abuse survivors differ from persons with other diagnosed disorders in regard to levels of shame? Sample data from the Internalized Shame Scale (ISS) administered to voluntary participants from clinical, college, and prison settings are analyzed and compared with established ISS normative groups. T-test results indicate that this sample of sexual abuse survivors report significantly higher levels of shame than the non-clinical normative group, and significantly less shame than eating-disordered women. When survivors are compared with other clinical normative groups there is no significant difference in shame--all exceed non-clinical norms and support the premise that shame is the core issue underlying clinical disorders. Frequency data pertaining to the respondents' abuse is presented. Demographic factors including ages of onset and cessation of abuse, frequency of abuse, and the relationship to the abuser are analyzed. An analysis of variance testing mean differences between the three age-of-onset categories failed to support theories which purport early childhood sexual abuse results in greater shame than later onset abuse. A relationship between the variables, frequency of abuse, and relationship to the abuser is suggested. Chi square results indicate a history of frequent family abuse episodes is statistically most common in this sample and occasional family abuse also occurs for a significant number of subjects. A concluding discussion incorporating results of this study with therapeutic intervention recommendations is presented.

Book excerpt: Sexual assault is associated with increased risk for physical and mental health challenges, specifically PTSD. Survivors of sexual assault experience PTSD at increased rates compared to those who have experienced other forms of trauma. Individuals who experience sexual assault are also less likely to seek mental health treatment following the assault. Trauma-related cognitions are associated with increased PTSD symptom severity, specifically trauma-related guilt, and shame. However, previous studies have rarely examined the impact of trauma-related guilt and shame simultaneously. Previous literature has suggested that trauma-related guilt is significantly related to PTSD symptom severity, however other studies examining trauma-related shame simultaneously have suggested shame may play a more central role. The proposed study utilized network analysis to explore the relations between PTSD symptoms, trauma-related shame, and trauma-related guilt. Networks were created from survivors of sexual assault and survivors of other trauma. Results suggested sexual assault survivors reported higher levels of trauma-related shame compared the other trauma survivors. However, there were not significant discernable differences in the connections between trauma related shame and guilt with the avoidance and negative alterations in cognitions and mood PTSD symptom clusters. Finally, results suggest that although trauma related shame is prevalent for sexual assault survivors, both trauma related shame and guilt signal high PTSD symptom severity regardless of trauma type.

The sample consisted of 114 children and adolescents (ages 11--18 years) who were wards of the Illinois Department of Children and Family Services (DCFS) and currently living in state-supervised residential treatment centers, apart from their families. This study assessed the relationship between varied histories of childhood abuse (sexual abuse, physical abuse, combined physical and sexual abuse), dissociative responses, and a variety of psychiatric outcomes using a multimethod design. Children were interviewed directly using several structured assessment tools, and their present caregivers (i.e., residential staff) were asked to complete questionnaires. A retrospective chart review was also used as an additional perspective on children's history and current functioning. Results indicated that sexual abuse was significantly associated with dissociation compared to other types of abuse history. Both sexual abuse and dissociation were independently associated with a several indicators of emotional and behavioral disturbance and risk-taking behaviors. Finally, dissociation was frequently a significant covariate for sexual abuse and psychiatric status. For instance, dissociation was a significant covariate for one measure of overall symptomatic functioning; previously significant main effects for sexual abuse disappeared after controlling for dissociation. These results were replicated across several assessment sources and different perspectives. Therefore, dissociation appears to serve as significant mediator function for emotional and behavioral sequelae among sexually abused children. The potential importance of more widespread assessment of dissociation among childhood populations and the implications of these findings are discussed.

Book excerpt: Prevalence of co-occurring posttraumatic stress disorder and social anxiety disorder (co-SAD/PTSD) ranges from 4.2-72% in clinical samples, suggesting exploration of moderating factors to better understand this comorbidity (Green et al., 1992, Orsillo, Heimberg, Juster, & Garrett, 1996). Previous reseach has found that shame (Orisllo et al., 1996) and history of childhood abuse (CA; Zayfert, DeViva, & Hofmann, 2005) are prevalent among those with co-SAD/PTSD. The present study examined shame and CA as potential moderators of co-SAD/PTSD in a sample of women exposed to intimate partner violence (IPV). A significant 3-way interaction was found. Contrary to prediction, high shame significantly increased the association between SAD and PTSD among CA-, but not among CA+. These findings sugest that heightened shame may be of more relevance among IPV survivors without history of CA. Results are discussed in light of moderating processes that may differentially influence co-SAD/PTSD among individuals with and without CA. .
