Even though the success rate is high, the number of infections related to the placement of the implant is still remarkable and may impair the proper function of the device, leading to health and economic costs. The infections related to medical devices start with a bacterial adhesion and proliferation on the material surface, leading to the formation of a complex biofilm able to protect the bacteria from the host immune response and the treatment with antibiotic. Due to the difficulty of treatment of the implant site one the biofilm is settled, one of the strategies to avoid the infection is to deal with the initial bacterial adhesion. This PhD thesis deals with the development of polymeric antibacterial coatings on titanium for dental implants, focusing on the achievement of fast and cost-effective procedure. With this aim, different coating strategies have been developed, tested and compared. A pre-treatment of the titanium surface was optimized in the first part of the thesis in order to achieve a clean surface and to enhance the chemical reactivity of the titanium oxide. With this aim, low pressure plasma activation was the selected method. The use of plasma activation allows for the removal of organic contaminants while increasing the surface energy of the treated surfaces. For the preparation of the polymeric antibacterial coatings, two different antifouling polymers have been used, namely, polyethylene glycol (PEG) and poly-2-hydroxyethylmetacrylate (PHEMA). PEG coatings were prepared by three different techniques, a wet chemical technique (silanization), a plasma enhanced chemical vapor deposition and an electrochemical process (electrodeposition). The three methods rendered an ultra-thin coating able to resist the bacterial adhesion. On the other hand, PHEMA-like coatings were prepared in a novel set-up by treating the liquid monomer by a plasma jet. Moreover, the different coatings were biofunctionalized in order to achieve multifunctionality and enhance the performance of the coating. For instance, the combination of PEG with a cell adhesion peptide (RGD) reported a better human fibroblast adhesion while maintaining the antifouling properties of the coating. PEG was also used as a platform for the immobilization of antimicrobial peptides (AMP). The bonding of the polymer with the AMP was optimized, achieving a surface able to reduce the bacterial adhesion and to kill the bacteria still able to adhere to the surface. Finally, the combination of two different plasma polymerized coatings with antibiotics (either Doxycycline or Vancomycin) was used as a drug delivery system.

The surface topography and chemistry of UNCD coatings were characterized using scanning electron microscopy (SEM), Raman, and X-ray photoelectron spectroscopies (XPS) respectively. In conclusion, this thesis contributed to establish the optimal conditions to grow UNCD coatings on the complex 3-D geometry of Ti-based micro-implants, with geometry similar to real implants, relevant to developing UNCD-coated Ti–based dental implants with superior mechanical/chemical performance than current Ti-based implants.

Despite its favorable bone regeneration properties, HA is prone to brittleness, limiting its use in load-bearing applications. To overcome this drawback, HA can be applied as a coating on metallic or polymeric implants, with the aim of significantly enhancing their overall performance. Synthetic HA is typically produced by means of several chemical routes. Recent approaches have been reported for extracting HA from natural and sustainable resources. Biologically derived apatite is a carbonated HA, which differs from synthetic HA in terms of composition, stoichiometry, crystallinity degree, crystal size/morphology, and consequently, degradation rate and overall biological performance. This reprint is devoted to the synthesis and characterization techniques of HA-based coatings. The Special Issue's scope includes both synthetic and naturally derived HA materials. Dicationic imidazolium-based ionic liquids (ILs) were rationalized, synthesized and their molecular structure and thermal behavior were characterized. IL toxicity against pre-osteoblast (MC3T3-E1) and human gingival fibroblast (HGF-1) cells, as well as the antimicrobial activity against bacterial strains relevant to the oral environment, were evaluated. For some IL compositions it was established a safe range of concentrations that ILs can be used, in which the minimum inhibitory concentration (MIC) against bacteria was lower than the concentration that inhibits 50% of cellular enzymatic activity (IC 50). These compounds were used as coatings of commercially pure titanium (cpTi) samples and the coated and non-coated samples were characterized. X-Ray photoelectron spectroscopy was used to understand the intermolecular interactions between IL molecules and Ti surface. Atomic force microscopy (AFM) provided a morphological map of surfaces as well as adhesion force of coatings and UV-Vis spectroscopy was used to understand their release profile. ILs were also tested in regards to their anti-corrosive and lubricant activities and it was observed that these compounds form stable films on the surface providing corrosion protection and lubrication. Finally, the coated surfaces were observed to exert antimicrobial and anti-adhesive activities against bacterial strains while also preserving the compatibility of the surface with gingival fibroblast and bone-forming cells.

Book excerpt: The mechanical and morphological properties of titanium as well as its biocompatibility and osteoinductive characteristics have made it the material of choice for dental implant systems. Although the success rate of titanium implants exceeds 90% in healthy individuals, a large subset of the population has one or more risk factors that inhibit implant integration. Treatments and coatings have been developed to improve clinical outcomes via introduction of appropriate surface topography, texture and roughness or incorporation of bioactive molecules. It is essential that the coatings and associated deposition techniques are controllable and reproducible. Currently, methods of depositing functional coatings are dictated by numerous parameters (temperature, particle size distribution, pH and voltage), which result in variable coating thickness, strength, porosity and weight, and hinder or preclude biomolecule incorporation. Silk is a highly versatile protein with a unique combination of mechanical and physical properties, including tunable degradation, biocompatibility, drug stabilizing capabilities and mechanical properties. Most recently an electrogelation technique was developed which allows for the deposition of gels which dry seamlessly over the contoured topography of the conductive substrate. In this work we examine the potential use of silk electrogels as mechanically robust implant coatings capable of sequestering and releasing therapeutic agents. Electrodeposition of silk electrogels formed in uniform electric fields was characterized with respect to field intensity and deposition time. Gel formation kinetics were used to derive functions which allowed for the prediction of coating deposition over a range of process and solution parameters. Silk electrogel growth orientation was shown to be influenced by the applied electric field. Coatings were reproducible and tunable via intrinsic silk solution properties and extrinsic process parameters. Adhesion was modulated over a 10-fold range and implant insertion into bone mimics demonstrated that the coatings were able to withstand delamination forces experienced during these mock implantations. Antibiotic release from coated implant studs inhibited bacterial growth and dexamethasone release was shown to stimulate calcium deposition in mesenchymal stem cells.

Book excerpt: Osseointegration and soft tissue seal formation between a dental implant surface and host tissues are important steps in achieving successful implantation. However, factors such as bacterial biofilm adhesion, excessive stresses experienced by an implant during insertion and mastication, and other health-related factors may affect implant stability. Dicationic imidazolium-based ionic liquid (IL) coatings containing amino acids were previously developed for implant surface functionalization. These multi-functional ionic liquid coatings have demonstrated excellent results as surface coatings of titanium in terms of providing the material with anti-biofilm activity, lubrication and corrosion resistance while being compatible with host cells in vitro. The aim of this study was to investigate the possibility of using this technology on the surface of a ceramic material, zirconia, which has been recently introduced in the design of one and two components dental implants. In this work, the physical and biological performance of IL coatings on the surface of zirconia was investigated. In summary, zirconia surfaces coated with two IL compositions were assessed for intermolecular interactions and coating morphology using X-ray photoelectron spectroscopy and optical microscopy. Coating stability was verified by release profiles using UV-vis spectroscopy. Mammalian and bacterial cell activity were studied on the surface of IL-coated zirconia using osteoblasts and fibroblasts cells and S. salivarius and S. sanguinis, respectively. Finally, wear tests were performed in simulated physiological conditions to determine coefficient of friction and wear volume loss in the presence of IL coatings. Results showed that ILs formed stable coatings on zirconia surfaces. IL containing phenylalanine demonstrated excellent anti-biofilm activity and sustained the conditions for growth and proliferation of host cells. The results of this study indicate that the investigated dicationic imidazolium-based IL coatings constitutes a potential technology for surface enhancement of zirconia dental implants.
