Book excerpt: Considerable effort has been expended particularly over the previous decade in the area of examining pre-crash factors in an attempt at developing appropriate highway safety countermeasures. This report summarizes the review of this field by a select panel of highway safety researchers and includes recommendations for NHTSA-sponsored research in accident causation over the next several years. After attempting to clarify the significance of the “cause” of an accident, the report outlines various approaches to accident causation research and indicates the relevant data needs. The extent and type of exposure information required is addressed. A detailed critique of the major research efforts in this field is provided. This critique centers mainly on the work done by Indiana University, Cornell Aeronautical Laboratory, Inc., Operations Research, Inc., and the University of Miami. Finally, short and long-range recommendations for NHTSA-sponsored research are presented. These include in general utilizing the NASS system to the fullest extent possible. Several specific studies recommended include: examination of data needs; survey of the possible existence of this required data; exploration of various alternative approaches to data collection (e.g. simulation); establishing appropriate quantifiers (e.g., what defines “following too closely”); determining exposure requirements; upgrading the statistical techniques utilized in this field; ascertaining the effectiveness of various countermeasures; and examining alternative information retrieval systems. In addition, several small open-ended research contracts for just plain “thinking about” accident causation methodology were recommended.

Aviation is, however, a complex system and as such suffers from being artificially manipulated into non-complex models and methods. The book’s new model offers many potential developments and some key areas are studied in this research. The application of Bayesian mathematics to historical data populations provides scope for studying error migration and barrier manipulation. The book also provides application of these predictions to a flight simulator study for the purposes of validation. Beyond this it also discusses the applicability of the approach to industry. Through working with a legacy airline the methods discussed are used as the basis for a new and prospective safety management system.

Book excerpt: Distributions of understeer coefficients and steering sensitivities are computed for (1) an OE vehicle population, (2) an at-risk vehicle population, and (3) an accident-involved vehicle population to determine if these vehicle handling parameters could be influential in accident causation. The influences of in-use tire factors (inflation pressure and tread depth) and in-use loading conditions (as reflected by number of occupants) upon understeer and steering sensitivity are reflected in the distributions calculated for the at-risk and accident-involved populations. It is concluded that no differences of any consequence exist between the distributions of either understeer or steering sensitivity as calculated for the at-risk and accident-involved vehicle populations. This conclusion is qualified by the following points: (1) knowledge of the effects of inflation pressure and tread depth on tire stiffness properties is imprecise, (2) the effects of mixing of tire sizes and constructions on understeer and steering sensitivity in the at-risk population is ignored, and (3) the sample of accident vehicles is somewhat small (a total of 218 vehicles).

Book excerpt: This book describes an orderly and science-based process of highway accident investigation, reconstruction and causation, beginning with the collection and interpretation of physical evidence, through the qualitative and quantitative reconstruction, to the determination of accident causation, with the ultimate goal of guiding the development of countermeasures. This process has evolved from the author's extensive teaching, research and reconstruction experience involving thousands of highway accidents, as well as the shared expertise of dozens of qualified investigators and engineers. Serious consideration is given to the role of police and civilian investigators in the process. Procedures are provided for the identification, collection, documentation and interpretation of the physical evidence upon which the entire process is dependent. Guidance is also provided in the qualitative reconstruction of accidents, thus providing critical information for subsequent quantitative reconstruction and causal analysis. Quantitative accident reconstruction proceeds from final rest backward through post-impact, impact and pre-impact phases of an event. The derivation and description of the vehicle and impact dynamics necessary for this reconstruction for a variety of crash configurations, as well as many example cases, are presented and discussed. An Accident Causation Model is offered as an orderly method for identifying human, vehicular, and environmental failures that cause a given highway accident. Analysis of the "second impact" includes a discussion of the methodology for determining occupant kinematics and injury causation, as well as suggestions for injury mitigation. Although this book is primarily directed toward investigators, reconstructionists and causal analysts, some topics will be of interest to professionals from a wide variety of fields such as attorneys, insurance adjustors, judges, and vehicle and highway engineers.
