Book excerpt: ABSTRACT: It has been shown that the interface between the prosthetic socket and residual limb (S-RL) interface is an important factor in determining acceptance and outcomes of upper limb prostheses. [1] Among the most common complaint from amputees is that the prosthesis is uncomfortable due to developing skin irritation which is usually attributed to poor fit (Nielson 1990). In order to understand why skin irritations can and do occur it is imperative to examine the biomechanical properties of the S-RL interface. A primary reason behind the development of skin irritation is instability of the socket upon the residuum. Alley (2009) asserts that excess slip, axial rotation, and translation are the facets of instability that cause skin irritations due to friction and shear. Measuring the motion at the S-RL interface is not commonly done and therefore there is still no valid and reliable method to quantify the motion clinically. A licensed prosthesis fabricated a transhumeral residual limb model to fit within a typical, harness suspended transhumeral prosthesis. A custom testing apparatus was built to hold the residual limb model and prosthesis for testing. Eight infrared markers were placed on the prosthesis and residual limb model: Two each respectively on the "wrist", elbow axis, socket, and on the residual limb model. Persons within these associated populations suffer from gait abnormalities and pain, which lead to a lower quality of life. Individuals with transfemoral amputation who are ambulatory don prosthetic sockets that often need frequent clinical refitting/reconfiguration and have few realistic alternatives, especially in individuals with obesity and/or vascular disease. To overcome these issues, osseointegration, or direct skeletal attachment of a prosthesis to a limb, has been proposed and shown to greatly improve functional mechanics of the lower-limb. But osseointegration surgery has only be used on patients with traumatic limb loss (representing the minority of the population) and can lead to infection of limb soft tissue and bone that requires additional amputation surgery and loss of limb length. Medial thighplasty, or excision and liposuction of adipose tissue in the residual limb, is a more conservative surgical alternative with higher eligibility than osseointegration and that may also improve prosthetic function. However, this limb “recontouring” procedure is often viewed as a surgery that improves limb cosmesis (i.e., its appearance), as opposed to its function. In fact, most medial thighplasty candidates are non amputees who are obese. This dissertation evaluates the influence of medial thighplasty surgery in the context of transfemoral limb loss and quantifies improvements in lower-limb and center-ofmass kinematics and kinetics during straight-line walking as well as during transient changes of direction. This surgical technique—modifying only the limb adipose tissue—improved biomechanical function and maneuverability characterized by greater path efficiency, faster movement completion times, smoother turning curvatures, tighter coupling between movement velocity and curvature, and increased shear ground reaction forces. This work suggests that confidence and fluidity of movement can be improved if medial thighplasty is applied in individuals with transfemoral limb loss and provides a missing link between the underlying structure of the residual limb and the function of lower-limb prostheses. Osteoarthritis has been shown to be secondary to developmental joint abnormalities, which are often expected to lead to higher contact stress of the articulating joint surfaces. Dysplasia and femoroacetabular impingement are two common hip deformities that lead to premature joint degradation. Yet, diagnosis is complex, and the course of treatment is different depending on the pathology. A robust treatment plan for therapy or surgery is vital to preserving the hip joint before the onset of osteoarthritis. This dissertation evaluated patients with diagnosed dysplasia or impingement prior to hip preservation surgery and uncovered significant differences in their gait biomechanics. The dysplasia cohort exhibited widespread gait deviations throughout the stride, while impingement group showed localized differences in mechanics that primarily occurred during peak hip extension (i.e., last stance phase of the stride). Furthermore, this dissertation investigated the use of dynamic simulation of static medical imaging from these patients. We argue, as an alternative to dynamic imaging modalities, that imposing normative motion of these static joint images in a simulation framework can lead to further insight about the underlying root causes of each deformity and calculate geometric properties of the hip center-of-rotation, as well as inadmissible variations of the relative motion between femur and acetabulum during normative level and sloped walking. Finally, continuous classification schemes were evaluated (linear and nonlinear discriminant analyses) to probe the separation between time-varying features of these multiple pathologies that occur throughout the stride. These final studies pertaining to hip preservation also deepen our understanding of the functional link between lower-limb structure and its function during widely varying ambulation scenarios and provide a potentially powerful tool to improve the diagnosis and treatment of a given patient.

Book excerpt: For people living with limb amputation, the prosthetic socket - the interface between the residuum and prosthesis - is the most critical component. When a socket is uncomfortable, especially due to poor fit, the quality of life for a patient is greatly hindered. However, conventional design of sockets is largely artisan, with limited input of quantitative data. Current computer-aided and manufacturing (CAD/CAM) designs are still not clinically applicable solutions. Due to model identification procedures that employ non patient-specific and incomplete data sets, today's finite element (FE) models of the residuum are not predictive, leading to suboptimal socket designs. As such, there exists a need for a comprehensive biomechanical model of the residuum for the quantitative design and computational evaluation of patient-specific prosthetic sockets. This thesis presents a combined experimental-numerical approach to evaluate and validate a transtibial residuum biomechanical model. The central hypothesis of the work is that a single biomechanical model can predict the large non-linear response at various sites on a residuum under load. To evaluate this hypothesis, a non-linear, two-tissue model was formulated where tissue geometries were defined using MRI data of the residuum. The non-linear viscoelastic material parameters of the model were identified through inverse FEA-based optimization using in-vivo indentation experimental data at four locations. Using optimized model tissue parameters, the mean percentage error (mean absolute error/ maximum experimental force) between the experimental and simulation force-time curves at 14 other locations across the evaluated transtibial residuum was 7 ± 3%. Using this same modeling methodology and a single set of material constants to describe the bulk soft tissue biomechanical response of seven distinct transtibial residual limb models, the average percentage error for indentations at multiple locations across all seven limbs was 7 ± 1%. From these predictive models of residuum limbs, one rigid novel socket and two multimaterial transtibial sockets were designed, fabricated and evaluated through an entirely quantitative, automated and repeatable methodology. In a preliminary clinical investigation, the novel sockets were shown to reduce peak contact pressures at the tibia and fibular head regions on the residuum by significant amounts during standing compared to a conventional socket interface designed and fabricated by a trained prosthetist.

Book excerpt: In this study, a non-linear Finite Element (FE) model was created and analyzed to determine the pressure distribution between the residual limb and the prosthetic socket of a transfemoral amputee. This analysis was performed in an attempt to develop a process allowing healthcare providers and engineers to simulate the fit and comfort of transfemoral prosthetics to reduce the number of re-fittings needed for the amputees. The analysis considered the effects of interference due to insertion of the limb into the prosthesis, referred to as donning, and also the effects due to the body weight of the amputee. A non-linear finite element static implicit analysis method was utilized. This analysis implemented multiple finite element techniques, including geometric non-linearity due to large deflections, non-linear contacts due to friction between the contact surfaces of the residual limb and the socket, and non-linear hyper-elastic material properties for the residual limb's soft tissue. This non-linear static analysis was carried out in two time-steps. The first step involved solving the interference fit analysis to study the pre-stresses developed due to the effect of donning. The donning process results in soft tissue displacement to accommodate the internal geometry of the prosthesis. In the second load application time-step, an additional load of half the person's body weight was applied to the femur. The maximum normal stress (contact pressure) of 84 kPa was observed due to the combined effect of the donning procedure and body weight application, comparable to the studies performed by other researchers. The procedure developed through this work can be used by future researchers and prosthetic designers in understanding how to better design transfemoral prosthesis.
