Book excerpt: The purpose of this thesis is to recommend a leadership approach in healthcare with a primary focus on two key strategies, transformational leadership and the application of lean methodologies. The implementation of such an approach, joining a proven leadership strategy with a proven production system, can potentially improve the quality of the work, reduce stress on team members and leaders, and reduce the costs. More importantly it should help reduce the number of healthcare associated infections (HAI’s) and the death and suffering they cause. The literature review included relevant areas of three primary topics, healthcare, leadership and lean methodologies. Points of special focus included HAI’s, partnership, trust, transformational leadership, and the Toyota Production System (TPS). The healthcare research revealed tremendous harm caused to patients by infections associated with healthcare and was a compelling call to action for this thesis. Tens of thousands of patients suffer and die each year due to HAI’s, the majority of which are preventable. The research on transformational leadership indicated it improved human performance and improved the capacity and quality of life of followers. Developing followers into leaders was a primary function and benefit of transformational leadership. It was evident in the literature that a culture of trust, transparency, and integrity, with a partnership mindset, and a compelling vision or purpose improved performance. Lean, particularly the TPS, produced outstanding results in quality, efficiency and reliability, all currently lacking in healthcare, and all contributors to reducing HAI’s. This thesis concluded with the recommendation that a leader led training approach should be designed to combine transformational leadership with lean methodologies. The primary focus of this training should be building team members into transforming lean leaders to ensure continuous improvement. The daily use of relevant stories and open-ended questions by leaders can inspire critical thinking by staff members and help create a learning culture. Such a leader led training program well designed and appropriately delivered, could create a healthy and nurturing human culture for staff members with a safer, higher quality, and lower cost service for patients in the high risk high pressure environment of healthcare today.

Recent research has shown that the quality of the principal is, among school-based factors, second only to the quality of the teacher in contributing to what students learn in the classroom. New programs to develop school leaders who can exercise vigilance over instruction and support effective teaching practices are not likely to succeed, however, if they are inconsistent with other state and district policies affecting school leadership. The Wallace Foundation, which focuses its grantmaking in education primarily on school leadership, has posited that well-coordinated policies and initiatives to develop leadership standards, provide high-quality training, and improve the conditions that affect principals' work will increase their ability to improve instruction in their schools. This study documents the actions taken by the Foundation's grantees to create a more cohesive set of policies and initiatives to improve instructional leadership in schools; describes how states and districts have worked together to forge such policies and initiatives around school leadership; and examines the hypothesis that more-cohesive systems do in fact improve school leadership. The study found that it is possible to build more-cohesive leadership systems and that such efforts appear to be a promising approach to developing school leaders engaged in improving instruction. Although the study did not find evidence that the full underlying theory behind this initiative is sound, it did find a correlation between improved conditions for principals and their engagement in instructional practices.
