Book excerpt: Over the past several decades, the idea of transforming Medicare from its current structure to one known as "premium support" has been raised intermittently as an approach for reforming the Medicare program, often in the context of efforts to reduce the federal debt and deficit. The primary goals of a premium support system are to reduce the growth in Medicare spending, and rely more on a competitive marketplace. While the parameters of various premium support proposals differ, the general idea is for the federal government to make a predetermined contribution on behalf of each person on Medicare that would be applied toward the premium for a health insurance plan. This approach contrasts with the current Medicare program under which Medicare beneficiaries are entitled to a defined set of benefits, with the federal government contributing to the cost of these services, provided by either the traditional fee-for-service (FFS) program or Medicare Advantage plans.

Our society wishes to ensure quality health care for all older people, but there is growing concern about our ability to maintain and improve quality in the face of efforts to contain health care costs. Medicare: A Strategy for Quality Assurance answers the U.S. Congress' call for the Institute of Medicine to design a strategic plan for assessing and assuring the quality of medical care for the elderly. This book presents a proposed strategic plan for improving quality assurance in the Medicare program, along with steps and timetables for implementing the plan by the year 2000 and the 10 recommendations for action by Congress. The book explores quality of careâ€"how it is defined, measured, and improvedâ€"and reviews different types of quality problems. Major issues that affect approaches to assessing and assuring quality are examined. Medicare: A Strategy for Quality Assurance will be immediately useful to a wide audience, including policymakers, health administrators, individual providers, specialists in issues of the older American, researchers, educators, and students.

Available in PDF, EPUB and Kindle. Book excerpt: Medicare beneficiaries are rapidly moving into managed care, as attempts to restrain the growth of this costly entitlement program progress. However, advocates for patients question whether the necessary information and structures are in place to enable Medicare consumers to select wisely among private-sector managed care options. Improving the Medicare Market examines how to give Medicare beneficiaries the same choice of health plan options enjoyed in the private sectorâ€"yet protect them as consumers and patients. This book recommends approaches to ensuring accountability and informed purchasing for Medicare beneficiaries in an environment of broader choice and managed careâ€"how the government should evaluate and approve plans, what role the traditional Medicare program should play, how to help to elderly understand their options, and many other practical matters. The committee discusses the information requirements of Medicare beneficiaries and explores in detail how best to respond to their special needs. And it examines the procedures that should be developed to provide the necessary protections for the elderly in a managed care system.

Book excerpt: Medicare reform plans advocate reducing the capitation levels or determining these endogenously as a function of the premium bids, for example, the lowest, the second-lowest or a weighted average of the bids. Based on a price competition model tailored toward this market, and actual 2010 county-by county data, we estimate the impact such reforms would have on the plans's market shares, equilibrium premia, the government's and the beneficiaries' costs. We employ two methodologies to derive the model parameters. The predicted impacts are remarkably consistent and reveal, for example, that government costs could be reduced by 16.5%-21%.

Book excerpt: In response to concern about increases in the federal debt and deficit, prominent policymakers and commissions have proposed reducing federal spending on entitlement programs, including Medicare. One of the most frequently discussed proposals would transform Medicare from a defined benefit program, in which beneficiaries are guaranteed coverage for a fixed set of benefits, to a defined contribution or "premium support" program, in which beneficiaries are guaranteed a fixed federal payment to help cover their health care expenses. The idea of transforming Medicare into a system of premium supports, establishing fixed contributions per Medicare beneficiary, has been raised intermittently since the early 1980s, and more recently, a number of proposals have been advanced in the context of reducing the federal deficit and debt. Premium support proposals differ in a number of ways, including how federal payments on behalf of beneficiaries would be determined; whether Medicare fee-for-service would remain an option; what protections would be provided for low-income beneficiaries; and the extent to which the proposal would rely on a federal cap on Medicare spending to help constrain the growth in program spending. These differences have important implications for beneficiaries, Medicare program spending, health care providers, and health plans. This document provides a side-by-side comparison of the most recent proposals for transforming Medicare into a premium support program that have been put forth by policymakers and commissions as part of broad-based deficit- and debt-reduction packages.

Book excerpt: Medicare's Quality Improvement Organization Program is the second book in the new Pathways to Quality Health Care series. Focusing on performance improvement, it considers the history, role, and effectiveness of the Quality Improvement Organization (QIO) program and its potential to promote quality improvement within a changing health care delivery environment that includes standardized performance measures and new data collection and reporting requirements. This book carefully examines the QIOs that serve every state as well as the national program that guides and supports them. In addition, it highlights the important roles that a national program with private organizations in each state can play in promoting higher quality care. Medicare's Quality Improvement Organization Program looks closely at the technical assistance role of the QIO program and the need to encourage and support providers to improve their performance. By providing an in-depth assessment of the federal experience with quality improvement and recommendations for program improvement, this book helps point the way for those who strive to create higher quality and better value in health care. Intended for multiple audiences, Medicare's Quality Improvement Organization Program is essential reading for members of Congress, the federal executive branch, the QIOs, health care providers and clinicians, and stakeholder groups.

Those options cover many areas-ranging from defense to energy, Social Security, and provisions of the tax code. The budgetary effects identified for most of the options span the 10 years from 2014 to 2023 (the period covered by CBO's May 2013 baseline budget projections), although many of the options would have longer-term effects as well. Chapters 2 through 5 present options in the following categories: Chapter 2: Mandatory spending other than that for health-related programs; Chapter 3: Discretionary spending other than that for health-related programs, Chapter 4: revenues other than those related to health; Chapter 5: Health-related programs and revenue provisions. In addition to 11 options that are similar in scope to others in this volume, Chapter 5 includes 5 broad approaches for reducing spending on health care programs or revenues forgone because of tax provisions related to health care. Each would offer lawmakers a variety of possibilities for making changes in current laws. Chapter 6 differs from the rest of the volume; it discusses the challenges and the potential budgetary effects of eliminating a cabinet department.
