Book excerpt: This case book will be based on real life scenarios of patient cases which have been encountered over several decades in one of the largest phototherapy centers in the United States. To our knowledge, there has not been anything similar published to date. By presenting various cases and how to manage such cases, we aim to educate dermatology trainees and practicing dermatologist who are trying to familiarize themselves with phototherapy. A recent study has shown that many dermatologists do not know how to administer phototherapy, and most graduating residents do not have adequate teaching and experience with administering phototherapy during their residencies. Therefore, the hope is that this book will fill a critical gap in the field of dermatology.

Book excerpt: The therapeutic use of ultraviolet radiation for medical purposes has a long history, whether it be sunlight in heliotherapy or artificial lamps in phototherapy, to name a few. The interest in ultraviolet radiation as treatment for various cutaneous diseases increased exponentially in the second half of the twentieth century. UV-based therapies, which include narrowband (NB) UVB, broad-band (BB) UVB, and psoralen and UVA (PUVA), are well-established treatment options for diverse dermatologic conditions such as atopic dermatitis, vitiligo, cutaneous T-cell lymphoma, and psoriasis either as monotherapy or as an adjuvant to systemic therapy. This monography focuses on the principal indications of phototherapy in dermatology.

Available in PDF, EPUB and Kindle. Book excerpt: Photodynamic therapy (PDT) is increasingly being used amongst health practitioners in combating a variety of diseases. One common strategy used during this light activated process is to induce the accumulation of significant amounts of a photosensitising agent in the diseased tissue. The naturally occurring amino acid 5-aminovulinic acid (ALA) is used to produce high levels of the photosensitizer, Protoprophyin IX, in the therapy known as ALA-PDT. Written by experts in the field Photodynamic therapy with ALA outlines a variety of clinical approaches to the topic, outlining current applications. These include the use of ALA-PDT and ALA-FD (fluorescence diagnosis) in the areas of dermatology, urology, brain, otorhinolaryngology, gynaecology and gastroenterology. Also included is a chapter that comprehensively reviews the scientific principles of ALA-PDT. Primarily aimed at a clinical audience Photodynamic therapy with ALA should provide a comprehensive review and useful insight into ALA-PDT.

Available in PDF, EPUB and Kindle. Book excerpt: As humans ventured into the twentieth century, the industrialized countries were confronted with the scourge of rickets. Although solariums were becoming common in the early 1900s and phototherapy was gaining popularity as a result of the awarding of a Nobel Prize to Finsen in 1903, it wasn't until 1921 when Hess and Unger demonstrated that rickets could be cured by exposure to sunlight that the healthful benefit of sun exposure appreciated. In 1941, Apperly (Cancer Research; 1: 191-195, 1941) noted that the occasional increased risk of skin cancer was associated with a decreased risk of many other more common and serious cancers. The alarming increase in the number of cases of skin cancer, especially melanoma, has caused great concern about the negative role of sunlight in health. The Sixth International Arnold Rikli Symposium on the Biologic Effects of Light was held in Boston, Massachusetts from June 16th - 18th, 2001. The goal of this Symposium was to focus on the very popular practice of tanning either by sunlight or by artificial light sources and the overall impact this practice has on health and disease. The program was organized by members of the Scientific Advisory Committee and my co-chair emeritus, Professor Ernst G. Jung. The Program Committee organized an outstanding state-of-the-art program that was enthusiastically received by the participants.

Available in PDF, EPUB and Kindle. Book excerpt: A consummate classic with a fresh approach to pediatric dermatology Children ́s skin is different. Maturation affects the epidermal barrier, the cutaneous microbiome, adnexal structures, vasculature, and transcutaneous absorption of drugs. The immature skin is more susceptible to pathogens and environmental disruption. Many genetic disorders are either present at birth or manifest early in childhood. Skin diseases thus present differently in children than in adults. Pediatric dermatology has seen significant advances over the last decade, particularly in the field of molecular genetics research, which has furthered our understanding of the pathogenesis of many skin diseases and the development of new approaches to treatment. This fourth edition of the Harper classic provides state-of-the-art information on all aspects of skin disease in children. It covers the diagnosis and treatment of all conditions - both common and rare - with a consistently evidence-based approach. Existing content has been refreshed and fully updated to reflect emerging thinking and to incorporate the latest in research and clinical data - especially at the genetic level. This new fourth edition includes: Greater focus on the genetics behind skin disease, including new genes/genodermatoses, progress in genetic analysis, and stem cell transplants Increased coverage of lasers and other technologies used to treat skin disease More summary tables, learning points, tables of differential diagnosis, and clinical algorithms for diagnosis and management Additional online features, including patient information links and multiple choice questions Harper's Textbook of Pediatric Dermatology delivers crucial clinical insights and up-to-date research information that spans the breadth of the field. As the most comprehensive reference book on this subject available, this revised fourth edition will support and guide the daily practice of both dermatologists and pediatricians across the world.

Book excerpt: This book reviews the role of each cell subset in the skin, providing the basics for understanding skin immunology and the mechanisms of skin diseases. The skin is one of the immune organs and is continually exposed to foreign antigens and external stimuli that must be monitored and characterized for possible elimination. Upon exposure to foreign antigens, the skin can elicit a variety of immune responses in harmony with skin components that include keratinocytes, dendritic cell subsets, mast cells, basophils, fibroblasts, macrophages, gamma-delta T cells, neutrophils, myeloid-derived suppressor cells, vascular and lymphatic cells, hair follicles, platelets, and adipose tissues, among others. In the past 10 years, knowledge of immunology has expanded drastically in areas such as innate immunity (Toll-like receptors, C-type lectins), and host defenses to bacteria and viruses, and this increased knowledge has led to the development of more effective treatment of psoriasis and other skin diseases. This book provides updates on the mechanisms of skin diseases including contact dermatitis, atopic dermatitis, psoriasis, urticaria, drug eruption, bullous diseases, anaphylaxis, graft-versus-host disease, rosacea, lymphoma, photodermatology, and collagen vascular diseases. Understanding the basics of skin immunology will help clinicians and dermatologists use new therapeutics such as biologics efficiently. Serving as an intermediary between basic science and clinical medicine, this book gives readers the opportunity to understand and marvel at the mystery and fascination of skin immunology.
