Book excerpt: INTRODUCTION : Pour diminuer la mortalité par cancer du sein, la France a mis en place un programme de dépistage organisé, nécessitant une participation suffisamment importante pour être efficace. Or, en raison d’une polémique remettant en cause son efficacité et de la persistance d’une démarche de dépistage individuel, celle-ci stagne depuis plusieurs années, sans atteindre l’objectif visé de 70%. METHODE : Une enquête qualitative par entretiens semi-dirigés a été réalisée afin de recueillir l’opinion et les pratiques des médecins généralistes concernant ce dépistage, et leurs avis sur les propositions du Plan Cancer visant à améliorer la participation et l’information aux femmes. RESULTATS : La majorité des généralistes sont convaincus de l’intérêt du dépistage organisé pour améliorer l’efficacité des traitements du cancer du sein. Les autres, évoquent un sentiment d’exclusion. Ils sont surtout confrontés à la peur des patientes et parlent davantage des bénéfices que des risques. Le dépistage individuel est conditionné par la présence d’antécédents familiaux, les médecins estimant alors que les patientes relèvent d’une surveillance précoce. CONCLUSION : Les médecins généralistes font partie des mieux placés pour convaincre leurs patientes de participer, même s’il existe une divergence de perception entre les médecins, qui voient dans le dépistage un intérêt préventif, et les femmes, qui l’associent à la maladie. Les coordinateurs des structures de gestion sont les interlocuteurs privilégiés pour renforcer l’adhésion des médecins au programme et améliorer leurs connaissances. L’utilisation d’outils d’aide à la décision médicale partagée permet la transmission d’une information objective et est à développer.

Murphy concentrates on the technoscientific means—the technologies, practices, protocols, and processes—developed by feminist health activists.
