L'ischémie myocardique silencieuse est un facteur prédictif d'évènements cardiovasculaires mais son identification reste difficile chez les patients diabétiques. En effet, les critères de sélection des patients à haut risque cardiovasculaire relevant d'un dépistage de l'ischémie myocardique silencieuse (IMS) doivent être améliorés. L'objectif principal de notre étude était d'identifier les facteurs prédictifs d'ischémie myocardique silencieuse afin de mieux cibler les patients à dépister. Les objectifs secondaires étaient de déterminer la prévalence de l'ischémie myocardique silencieuse et d'évaluer la prise en charge thérapeutique des patients dépistés. Méthode: Nous avons inclus de façon rétrospective, les patients diabétiques (de type 1 ou 2) ayant été dépistés pour l'IMS dans le service d'Hypertension Artérielle et de Médecine Vasculaire du CHRU de Lille entre janvier 2011 et décembre 2013. Ces patients étaient asymptomatiques et à haut risque cardiovasculaire selon les critères de la SFC/ALFEDIAM. Le dépistage de l'IMS a été effectué par des tests d'ischémie non invasifs. Une coronarographie était réalisée en cas de test positif. Résultats: Cent quarante-neuf patients ont été inclus dans notre étude. Le diagnostic d'IMS a été établi chez 24,2% des patients. Parmi ces patients, 63,6% présentaient au moins une sténose coronaire significative. Une revascularisation percutanée ou chirurgicale a été réalisée chez 12,1% des patients dépistés. Nous avons identifié 2 facteurs prédictifs indépendants d'IMS en analyse multivariée: les sténoses artérielles des membres inférieurs > 50% (OR= 3,59IC 95% [1,33-9,72]) et les troubles de la cinétique segmentaire à l'échographie cardiaque de repos (OR=9,44 IC 95% [3,18-28,0]). Le dépistage de l'ischémie myocardique silencieuse a permis d'optimiser la prise en charge des patients puisque nous avons constaté une augmentation significative de prescription d'antiagrégants plaquettaires (p0,001), de statines (p
