Book excerpt: Introduction : La Maladie Rénale Chronique (MRC) de l'enfant évolue le plus souvent à bas bruit et tend inexorablement vers l'insuffisance rénale chronique, voire terminale. Toutefois, une prise en charge précoce permet de la ralentir et de stabiliser ses complications. L'attitude de dépistage reste non consensuelle pour de nombreux pays en raison notamment d'un rapport coût-efficacité incertain. Nous souhaitions explorer les pratiques et conceptions des médecins généralistes français vis-à-vis de ce dépistage. Méthodes : Nous avons réalisé à ce propos une étude non interventionnelle d'évaluation des pratiques professionnelles en envoyant en juillet et octobre 2020 un questionnaire informatisé à 5108 médecins généralistes. Les sujets inclus étaient des médecins généralistes thésés, installés dans le Grand Est et faisant partie de la liste de diffusion des messages de l'URPS-ML. Résultats : Le taux de participation a été de 1,3%. 54,4% des médecins interrogés ont déclaré avoir déjà réalisé un dépistage de la MRC au moins une fois dans leur pratique. 52,9% n'avaient pas connaissance de quelconque recommandation. 47,1% jugeaient correctement la prévalence de cette affection au sein de la population pédiatrique. Le terrain héréditaire a été le facteur de risque de MRC le plus souvent cité (60,3%), les oedèmes le signe d'appel le plus évocateur (57,4%) et la bandelette urinaire l'outil clinique privilégié (78,8%). Enfin, 75,4% des médecins ne maîtrisant pas ce sujet ont estimé le manque de formation comme le principal frein à sa réalisation. Conclusion : La méconnaissance des recommandations ainsi que la sous-estimation de la prévalence de la MRC de l'enfant contribuent notoirement à un mésusage d'un dépistage simple, accessible et efficace. Ainsi, l'utilisation appropriée, à grande échelle, d'outils de dépistage clinique validés comme la bandelette urinaire et le tensiomètre pédiatrique permettrait de repérer plus précocement davantage d'enfants atteints de MRC. Une sensibilisation régulière lors de formations médicales continues semble nécessaire afin d'optimiser les pratiques.

Chez l'enfant, de récentes études font le lien entre MRC à l'âge pédiatrique et insuffisance rénale terminale à l'âge adulte, et suggèrent que la prévalence de la MRC stade précoce non diagnostiquée chez l'enfant serait de près de 1%. Tout retard de diagnostic implique un retard de mise en oeuvre des moyens de néphroprotection nécessaires au ralentissement de l'évolution de la MRC. Suite aux recommandations de la société de Néphrologie Pédiatrique parues en 2016, nous avons cherché à faire un état des lieux des pratiques des médecins généralistes en ce qui concerne le dépistage de la MRC pédiatrique en région Rhône-Alpes et avons élaboré une plaquette d'aide au dépistage dans une démarche d'amélioration de la qualité des soins. Il s'agit d'une enquête descriptive transversale, par le biais d'un questionnaire informatisé diffusé par mailing listes ou sur les réseaux sociaux. La plupart des médecins (96%) ne se sentent pas compétents pour dépister la MRC de l'enfant et la quasi-totalité (96%) souhaite bénéficier de formations à ce sujet. Il existe une méconnaissance des techniques de dépistage de la MRC pédiatrique. Très peu de médecins connaissent la formule de Schwartz, seule formule utilisable en pédiatrie pour estimer le débit de filtration glomérulaire. Les facteurs de risque de MRC s'avèrent mal connus, notamment l'obésité, la prématurité et le petit poids de naissance. Une plaquette de dépistage intéresserait 98,5% des répondants. Dans les années à venir, du fait de l'accroissement de la prévalence des facteurs de risques (obésité, HTA,...) et de certaines populations à risque liées aux flux migratoires par exemple, la prévalence de la MRC de l'enfant devrait augmenter. Devant le coût humain et financier que représente le traitement de l'insuffisance rénale terminale en France, il paraît indispensable de dépister tous les enfants concernés. Notre plaquette se voulait une aide au dépistage et va être proposée pour financement.
