Book excerpt: Many of the trials taking place today are unregistered and unpublished, meaning that the information that they generate remains invisible to both the scientific community and the public. This undermines public trust, slowing the pace of medical advancement and potentially putting patients at risk. All trials conducted on NHS treatments-and all other trials receiving public funding-should be prospectively registered and their results published in a scientific journal. While the focus should be on implementing this change for future trials, the Government must also do what it can to ensure that historic trials are registered and published, particularly where they have been publically funded. The Government should also take steps to facilitate greater sharing of the raw data generated during a trial in a responsible and controlled way, with the knowledge and consent of patients. The report also draws attention to the recent fall in the number of trials taking place in the UK. It finds that the need for multiple governance approvals from participating NHS organisations remained the biggest barrier to setting up a UK trial, but that lack of public awareness was also a key issue. Recruiting participants can also be a challenge. The report calls on the Government to take its recommendations into account in ongoing discussions regarding the revision of European clinical trials legislation and in its response to the European Medicines Agency's consultation on the release of clinical trial data, which closes at the end of this month

Book excerpt: The currently influential book 'Nudge' by Richard Thaler and Cass Sunstein advocates a range of non-regulatory interventions that seek to influence behaviour by altering the context or environment in which people choose, and seek to influence behaviour in ways which people often do not notice. This approach differs from more traditional government attempts to change behaviour, which have either used regulatory interventions or relied on overt persuasion. The current Government have taken a considerable interest in the use of 'nudge interventions'. One aim of this inquiry, therefore, is to assess the evidence-base for the effectiveness of this approach. However it also examines evidence for the effectiveness of other types of policy intervention and asks whether the Government make good use of the full range of available evidence. The Committee's central finding is that non-regulatory measures used in isolation, including 'nudges', are less likely to be effective. Effective policies often use a range of interventions. A lot more could, and should, be done to improve the evaluation of interventions. Specific recommendations include: the Government must invest in gathering more evidence about what measures work to influence population behaviour change; they should appoint an independent Chief Social Scientist to provide them with robust and independent scientific advice; the Government should take steps to implement a traffic light system of nutritional labelling on all food packaging.

It builds on the case studies covered in three previous reports on MRI safety, the illegal drugs classification, and ID card technologies. The Committee recommends that the role of Government Chief Scientific Adviser (GCSA) be split from that of Head of the Office of Science and Innovation, to enable full attention to be given to the GCSA's cross-departmental functions. The GCSA would be better placed in a department, such as the Cabinet Office, which has cross-departmental responsibilities. The establishment of departmental CSAs is welcomed, but more needs to be done to ensure they maximize their contribution to strategic decision making and policy development within departments. The report also recommends the establishment of a Government Scientific Service to enhance scientific support in the civil service, and calls for greater involvement of learned societies and professional bodies in the advisory system. The Committee wants greater public investment in research to underpin evidence-based policy making, with the establishment of a cross-departmental fund to commission independent research. The short-term nature of the political cycle should be counterbalanced by embedding horizon scanning into the policy-making process. Transparency in policy making has improved, but more could be done. Valuable work on risk should be continued, and responsible coverage of risk by the media should be encouraged by greater involvement of departmental CSAs and greater clarity and consistency in public communication.

The exact level is to be agreed, but if it was 40p, it is estimated there would be 50,000 fewer crimes each year and 900 fewer alcohol-related deaths by the end of the decade. Consultations will also aim to end multi-buy promotions. Local areas and agencies will be given powers to challenge people's behaviour and make it easier to take action against, and even close down, problem premises. Other measures include early morning restriction orders and a late night levy so that businesses open late contribute to the costs of policing. The drinks industry has a crucial role to play in changing the drinking culture towards positive socialising. And the risks of excessive consumption will be widely circulated.
