Those disparities are caused not only by fundamental differences in health status across segments of the population, but also because of inequities in factors that impact health status, so-called determinants of health. Only part of an individual's health status depends on his or her behavior and choice; community-wide problems like poverty, unemployment, poor education, inadequate housing, poor public transportation, interpersonal violence, and decaying neighborhoods also contribute to health inequities, as well as the historic and ongoing interplay of structures, policies, and norms that shape lives. When these factors are not optimal in a community, it does not mean they are intractable: such inequities can be mitigated by social policies that can shape health in powerful ways. Communities in Action: Pathways to Health Equity seeks to delineate the causes of and the solutions to health inequities in the United States. This report focuses on what communities can do to promote health equity, what actions are needed by the many and varied stakeholders that are part of communities or support them, as well as the root causes and structural barriers that need to be overcome.

The case studies explore the context that explain this divergence and its implications. One of the distinguishing features of the left in Britain is that it holds to a remorselessly bleak and miserabilist view of our recent political history — one in which Margaret Thatcher's election in 1979 marked the start of a still-continuing fall from political grace made evident by the triumph of a free market get-what-you-can neoliberal ideology, dizzying levels of inequality, social decay, rampant individualism, state authoritarianism, and political corruption. The left does not like what has happened to us and it does not like what we have become. Andrew Hindmoor argues that this history is wrong and self-harming. It is wrong because Britain has in many respects become a more politically attractive and progressive country over the last few decades. It is self-harming because this bleak history undermines faith in politics. Post-Brexit, post-Grenfell, and post the 2010, 2015, and 2017 general elections, things may not, right now, look that great. But looked at over the longer haul, Britain is a long way from being a posterchild for neoliberalism. Left-wing ideas and arguments have shaped and continue to shape our politics.
