Les constituants de ce concentré plaquettaire et leurs rôles biologiques seront détaillés et expliqués. On procédera ensuite à une évaluation de la littérature sur le sujet en présentant les différents cas et études disponibles. On conclut finalement sur l'absence de publications démontrant l'intérêt de l'utilisation de cet adjuvant en chirurgie osseuse pré-implantaire.

Book excerpt: La cicatrisation est un passage obligatoire et nécessaire en implantologie. Il s’agit d’une réalité clinique quotidienne qui concerne tous les praticiens. L’insuffisance d’os résiduel et une cicatrisation déficiente sont des situations cliniques défavorables que l’on rencontre trop souvent en implantologie orale. Au cours des dernières années, la compréhension des mécanismes moléculaires qui régissent les interactions des cellules osseuses et de leur environnement et la découverte de protéines hautement spécifiques (facteurs de croissance et cytokines), jouant un rôle majeur dans la cicatrisation et le remodelage osseux, a permis d’ouvrir un nouveau champ d’application en chirurgie implantaire. C’est la raison pour laquelle de nouvelles techniques sont apparues : l’utilisation de concentrés plaquettaires apporte sur le site une grande quantité de plaquettes, source autologue de facteurs de croissance qui interviennent à différents niveaux pour améliorer et accélérer la cicatrisation. Nous allons donc dans premier temps exposer des rappels sur la cicatrisation muqueuse et osseuse, avant et après chirurgie implantaire et montrer le rôle des facteurs de croissance lors de la cicatrisation. Puis nous exposerons les deux procédés qui permettent d’extraire ces précieux facteurs de croissance. Enfin, nous étudierons les différentes applications chirurgicales de ces concentrés plaquettaires décrites dans la littérature, illustrées de cas cliniques et nous tenterons de tirer une conclusion argumentée sur l'apport des facteurs de croissance dans la gestion de la cicatrisation après chirurgie implantaire.
